their body temperature to some extent, prematureinfants have thinner skin, which allows surface bloodvessels to more readily lose heat to the environment; alarge ratio of surface area to volume, resulting ingreater heat losses from radiation and convection; andalmost no subcutaneous fat to either metabolize intoheat or act as an insulator. Prolonged cold stress inneonates can cause oxygen deprivation, hypoglycemia,metabolic acidosis, and rapid depletion of glycogenstores; therefore, energy conservation provided bythermal support is critical.

Principles of operationThe neonate lies on a mattress in the infant com-

partment, which is enclosed by a clear plastic hood.Most incubators have hand access ports with doorsthat permit the infant to be handled while limiting theintroduction of cool room air. The clinician can raise orremove the plastic hood or open a panel to gain greateraccess to the infant. Some units feature an air curtainthat causes warm air to sweep past the opening.

Most incubators warm the infant by a forced ornatural flow of heated air. At least one unit supple-ments air convection by actively warming the incuba-tor walls to reduce radiant heat loss. Another unit usesa mattress of warm water, rather than a convectiveairflow, to warm the infant.

Heating and humidification systems are located be-neath the infant compartment. A fan or natural flowcirculates air past the heater and the temperaturemeasuring device, over a water reservoir used to hu-midify the air (if desired), and up into the infantcompartment. Most incubators are equipped with pro-portional heating controls that provide electricalpower to the heating coil in response to the differencebetween the actual temperature and the desired tem-perature. Most units have two modes of operation:air-temperature control and skin-temperature control.With the air-temperature (manual) control, the opera-tor sets the temperature of the air in the incubator;changes in infant body temperature are usually meas-ured periodically with a thermometer, and adjust-ments in air temperature are made accordingly. In theskin-temperature control mode, also called the servo(automatic) mode, a sensor is taped to the infant’s skin,and the heater responds to changes in the sensor tokeep the skin temperature at the preset level.

Most units allow the user to vary relative humidityfrom either a built-in reservoir or an outside source(e.g., a humidifier that attaches to one of the inletports). Although increasing the relative humidity in anincubator can reduce evaporative heat loss, many cli-nicians avoid supplemental humidification because of

concern that infectious organisms may proliferate inthe water reservoir.

Many incubators have one or two oxygen inlet portsand can be equipped with optional oxygen controllers.The incubators can also provide support and protec-tion for oxygen cylinders when oxygen must be deliv-ered to the infant in the incubator.

Because the room temperature of the nursery isnearly always lower than that inside the incubator,radiant heat loss through the incubator walls accountsfor as much as half of the infant’s total heat loss. Insome nurseries, a plastic heat shield is placed over theinfant inside the incubator to minimize radiant heatloss. In addition, some incubators have double wallsseparated by an air space to prevent excessive heatloss. However, in a study comparing heat loss fromservo-regulated single- and double-walled incubators,Bell and Rios (1983) reported that, although the dou-ble-walled incubator decreased radiant heat loss, itincreased convective heat loss; total heat loss andmetabolic heat production were the same as in single-walled incubators.

Incubator/radiant warming units

One manufacturer’s device combines the thermalcapabilities of an infant incubator with the accessadvantages of an infant radiant warming unit. Thishybrid unit provides a centralized control and moni-toring system for both the incubator and the warmingunit; it can be converted automatically from a closedincubator to an open bed radiant warming unit andvice versa with little effort in a few seconds. This deviceeliminates the need to transfer an infant between anincubator and a radiant warming unit.

Transport incubators

During transport within a hospital or to anotherfacility, infants require thermal support. Transportincubators can be operated on a variety of powersources found in hospitals or transport vehicles (e.g.,120 VAC, 12 VDC, 24 VDC). Most units also carry theirown backup power supplies (e.g., 12 VDC rechargeablenickel-cadmium [Ni-Cd], gel cell, or lead-acid batter-ies) that power the unit for short periods in the absenceor failure of other power sources. In addition to havinga reliable portable power source, transport incubatorsalso need to meet strength requirements for air andground travel, provide isolation from external noiseand vibration, and have limited electromagnetic emis-sions to allow for aircraft use. Transport incubators aretypically smaller and lighter than mobile or stationaryincubators, facilitating easy maneuvering in and outof emergency vehicles.

Reported problemsDeaths and injuries to neonates in incubators have

been linked to thermostat failure that caused incuba-tor overheating and infant hyperthermia and to mal-functions or design defects that produced fires andelectric shock hazards.

If a surface temperature sensor does not make ade-quate thermal contact with the infant’s skin, incorrectlow temperature readings may result, and the incuba-tor could overheat the infant in response to the falsereadings. However, using a strongly adherent tape tofasten the sensor can produce skin irritation or dam-age. Periodic checks of sensor placement are stronglyrecommended.

Inadequate control over the amount of oxygen deliv-ered in an incubator can cause hyperoxia or hypoxia(excessive or inadequate oxygen, respectively, in bodytissues). Excessively high concentrations of oxygen cancontribute to retrolental fibroplasia (i.e., formation offibrous tissue behind the lens, leading to detachmentof the retina and arrested growth of the eye) in prema-ture infants.

Incubators can produce excessive internal noise lev-els that might adversely affect infant hearing. Slam-ming a porthole, using a nebulizer, activating analarm, and tapping on the incubator hood furtherincrease the noise level to the infant; levels as high as90 to 100 dB have been reported. The 1991 Associationfor the Advancement of Medical Instrumentationstandard for incubators specifies that the maximumnoise level inside the infant compartment should notexceed 60 dB during normal use.

In a case reported to ECRI, an infant was able topush open the rear isolette door of a mobile infantincubator due to improper door gasket placement. Themattress in the isolette placed the infant at access-doorlevel, and the infant fell out of the incubator to thefloor.

Some older incubators still in use may have hoodthermometers or high-temperature thermostats (orboth) that are filled with mercury. Mercury contami-nation is possible if the thermometer or thermostatbreaks, which is most likely during routine cleaning ormaintenance. Several investigators have expressedconcern about possible adverse effects of an infant’sexposure to mercury vapor. It is not known to whatextent safe limits for chronic adult exposure apply tosick or premature infants who remain in incubators forextended periods of time. Some investigators recom-mend replacing all mercury-containing components ininfant incubators, and most hospitals have followedthis course of action.

ECRI has published an infant incubator checklist(see the citation from the Health Devices Inspectionand Preventive Maintenance System below) that canhelp detect many potential problems with incubatorequipment before they occur.

Purchase considerationsInfant incubators typically cost between $4,000 and

$14,000, depending on accessories purchased. Higher-priced incubators may include such features as amechanism that minimizes the temperature dropwhen the front-access panel is opened, a rail system tomount accessories, a humidification system, and anelectrical height adjustment capability.

Hospitals can purchase service contracts or serviceon a time-and-materials basis from the supplier. Serv-ice may also be available from a third-party organiza-tion. The decision to purchase a service contract shouldbe carefully considered. Purchasing a service contractensures that preventive maintenance will be per-formed at regular intervals, eliminating the possibilityof unexpected maintenance costs. Also, many suppli-ers do not extend system performance and uptimeguarantees beyond the length of the warranty unlessthe system is covered by a service contract.

ECRI recommends that, to maximize bargainingleverage, hospitals negotiate pricing for service con-tracts before the system is purchased.

A4363 O2 concentration variations can occur inOhmeda Medical Giraffe OmniBed infant incubator/radiant warming units, resulting in too much or toolittle O2 inside the infant compartment when the unitsare operated in incubator mode. The variations occuras a result of changes in the fan speed when the unitsare operated in incubator mode and free-flow O2 isadministered. Changes in the fan speed may occur atany time in response to the temperature of the bedheater or as the result of intentional user activation.The manufacturer states that it has not received anyreports of injuries arising from O2 concentration vari-ations. Injury is possible only if (1) free-flow O2 is beingadministered, (2) no servo-controlled O2 delivery sys-tem is being used, and (3) the O2 saturation of the

patient is not being monitored. The manufacturer in-itiated a safety alert by letter dated February 28, 2001.Verify that you have received the letter, prepaid re-sponse form, and operation and maintenance manualreplacement pages. About the chart specificationsThe following terms are used in the chart:

Backup thermostat: Backup device that deactivates theheater if the incubator’s internal air temperatureexceeds a limit when the primary controller fails.

Tubing ports: Number and type of openings in the incu-bator that allow therapeutic access to the infant.

O2 inlet ports/controllers: Number of openings in theincubator for tubing (oxygen delivery) and con-trol/monitoring of the oxygen delivered.

Internal noise, dB: The sound level inside the incuba-tor, measured in decibels. The values listed includeonly the noise generated by the incubator equip-ment itself.
