Facts about FASSeptember is a time when mostof us welcome the coming ofspring, but for thousands of ba-bies born each year, it is any-thing but a fresh new start.

International Fetal Alcohol Syn-drome (FAS) Day was on 9 Septem-ber. FAS is a polite, clinical termfor an infant that comes out of thewomb permanently damaged by al-cohol.Prof DenisViljoenof theFounda-

tion for Alcohol Related Research(FARR) says that FAS is by far themost common birth defect in SouthAfrica. “The incidence of FAS ishigher than all other birth defectscombined. This is particularlyshocking when you take into ac-count that FAS is 100% preventa-ble,” said Viljoen.Children with FAS have growth

deficiencies, irreversible mentalretardation and physical and cen-tral nervous systemabnormalities.The effects of FAS are permanentand irreversible.Viljoen disagrees with some doc-

tors’ advice that it is “OK to drinka little bit” when you are pregnant.“Ourmessage is that you shouldn’tdrink at all and we have good rea-son for that. I have studied over5 000 pregnant women over the last17 years, andwhatwe found is veryalarming.Noteveryonewhodrinksheavily has a FAS child, but youcan’t get aFASchildwithout drink-ing.“You can’t predict which woman

is likely tohaveachildwho is goingto be affected, based on her drink-ing habits. Obviously, the heavieryou drink, the more likely it is thatyour child will be affected, but itdoes not necessarily follow,” hesaid.“Other factors such as bodymass

and enzyme capability come intoplay. Many types of enzymes breakdown alcohol in the human body,

but you only inherit a specific sub-type from your parents, so somepeople have enzymes that breakdown alcohol very rapidly and oth-ers don’t.“Foranythreepregnantdifferent

women drinking the same amount,there will be three different out-comes; thesameis true for threedif-ferent pregnancies in one woman.We’ve had womenwho were excep-tionally light drinkers and still hadbabies showing signs of FAS disor-der.“Our advice to pregnant women

is stop drinking altogether, eathealthily, have all your check-upsduringpregnancyanddon’t smoke.You cannot think that all will bewell because you drank with yourfirst baby and he or she is alright.It does not work like that.“The bottom line is, FAS is 100%

preventablebynotdrinkingduringpregnancy.”FASoccursacrossall raceand in-

come groups, but is undoubtedly a

serious problem among the poorand unemployed in South Africa.DeAar, one of themost underdevel-oped rural places in the country,with an unemployment rate of upto 80%, has the grim record of thehighest prevalence rate of FAS inthe world, at 12,2% or 122 in 1 000babies born.Alcohol used by mothers and

damage to their offspring placeboth generations at higher risk ofabuse and other issues such as HIVinfection.The newly established Vital

Raising a child with FASWhile it is known that childrenborn with FAS have specific fa-cial charcteristsics, we often for-get about the devastatingphysival and mental effects onthe child.Helene* and her family have

been fostering 13-year-old Jays-on,* a FAS child, since he was 2years old. Jayson is the son offarm workers, and was takenaway from his parents owing totheir alcohol abuse and neglect.Helene’s experiences highlightwhat many tend to overlook –that babies with FAS grow up,and their problems don’t goaway.Helene’s family were the first

to ever takehimon trips fromthechildren’s home. He has no con-tactwithhisbiologicalparentsorany other family members. Shesays that when he first went toschool, he was teased: “This wasbecausehe looked different,withabigstomachandtinyontop,andhis head looked a little different.When he became angry aboutthat, he would grab them by thethroat. Luckily he was offered aplaceataschoolwhichaddressedhis problems, and helped him toovercome the anger due to hisabandonment aswell as FAS.Wetalk to him a lot and had him atanger management at Red Cross[War Memorial Children’s Hos-pital] after discussing the issuewith a social worker at the Fami-lyUnit.Weused toget letters eve-ry day from the school aboutproblems with his behaviour.“It is sometimes tough to han-

dle him, butwe find away,main-ly through talking with him andworking through issues, tellinghim of the consequences of whathe is doing and what is going tohappen. Eventually he realiseswhat he has to do. Children withFAS are not able to adapt quick-

ly.”This has been a very long proc-

ess, through which Helene hasbeen doing a lot of reading tolearnmore aboutFAS. “I learnedthat there is a chemical reactionin his brain that he cannot con-trol; if something happens, hecan change and react by gettingangry. When this happens, mostof the time he is not aware of it.You will blame him for some-thing,buthemaynotknoworun-derstand what he has done. Be-cause we are aware of this issuenow, we can handle it. Some-times that means stepping backuntil I am calm and he is calm,and then talking about it.“It was quite tough in the be-

ginning.Now it is going to anoth-er levelbecausehe isenteringad-olescence. I try to be ahead interms of what to expect, so I hadameetingwith the social workeraboutthat too,sothatIcanbepre-pared, since FAS is linked to ear-ly sexual development. We arevery open about what happens,what is wrong, and what is ac-ceptable in our environment. Iam always on the alert, though!“He is nowdoingwell at school

except in Maths, due to his shortconcentration span. We have tobe firmwith him about doing hishomework, but we don’t forcehim, because then he will justswitch off. Then I let the teacherknow, and he is made aware ofconsequences. There are no re-ports of fighting any more, andhe is no longer teased.”Helene’s message to other par-

ents and carers of FAS childrenis as follows: “They just needlove, just let them know that youcare for them. They also need astrong sense of security and alsoof boundaries, as well as struc-ture and discipline.”*Pseudonyms.
