PCR methods ampli fied a product in the lung of pigs No.2 and No.3 using either frozen or formalin -fi xed and paraffin -embedded lung tissue. DNA from Pig No. I samples did not ampli fy with any primer. This is the first time that molecular biology techniques (ill siru hybridi zation and PCR) have been applied to the study of porc ine pneumocystosis.

Introduction

Some authors class ify this organism as a protozoa n beca use of its morpholog ic similarities to protozoa and susceptibility to some antiprotozoan drugs. PC emerged as a lead ing cause of opportuni stic in fec tion and mortality in HIV-positi ve patients in the 1980s (Walzer, 1993).

Two major li fe stages have been fo und in the lungs of infec ted animals: trophozoi tes and cys ts (Ruffolo. 1994: Sukura. 1995). Trophozoites are irregular. 1-5 Jim in diameter. thin-walled and with a vari able number of fil opodia (tubular expansions of the ce ll membrane and cell wa ll ) (Bedrossian, 1989) . desc rib ed in foa ls (Ainsw o rth e t a l. , 1993) : goa ts (McConnell et aI. , 197 1) ; dogs and cats (Se ttnes and Hasse lager, 1984: Sukura et aI., 1996). There are several reports of PC pneumoni a or detec tion of thi s organism in pi gs (Kuce ra et aI. , 1968 : Se ibold and Munnell. 1977: Fujita et al., 1989; Bille- Hansen et al .. 1990: Kondo et aI. , 1993) .

[n thi s s tud y we co mpare th e ma c rosco pi c . histopatholog ic, e lectron microscopic and microbiologic features of a natural PC infection of three pi gs with those described in the li terature. [n additi on, thi s is the first report descri bing molecular bi ology techniques (i ll Silll hybridi za tion li nd PCR) for the study and di agnosis of porcine pneumocystosi s.

Materia[s and methods

Animals, histopathology and electron microscopy

Tiss ues were collec ted from one male (Pi g No. I) and two fe male (Pigs No.2 and 3) 8-week-old pigs (pigs No . I and No. 2 were from the same farm ), fixed in 10°1c neutral buffered formalin and processed routinely fo r hi stopathology. Six- micron lung sec tions were stained with HE, Gi emsa, PAS and Gomori' s meth enamin e sil ve r (G MS) stains. Formalin-fi xed lungs from pigs No. 2 and No . 3 were postfixed in l o/c OS04' processed and embedded in a mi xture of Epon-Araldite, stained with uranyl ace tate and lead c itrate and exa mined with a Philips 301 transmission e lectron microscope at 60 Kv.

In situ hybridization

The i ll Sil ll hybridi za ti o n tec hniqu e (I S H) to demonstra te Pnelllll oc),slis carillii has been described e ls ew he re (Mo nto ne , 1994). Bri e fl y , f ive -mi c ro n sec ti ons were deparafini zed , rehydrated and di ges ted with a solution (2.5 mg/ml ) of pepsin at 105°C for 3.0 minut es. The bi otin - labe led o li go nU Cleotid e probe. consisting of 22 base sequences (S'-TCTCTG AGGTA TGGCCGTAA CT-3') co mpl ementary to th e fir st 22 nucleic ac id sequences of PC 5S rR NA . was diluted to 200 ng/ml in a non-formamide based cocktail. The probe solu tion was applied to the slides and the ti ss ues were heated at 105°C for 2 min . to denature any secondary rR NA stru c tures. The ti ss ue ta rge t a nd th e pro be hybridized for 10 min at 40 °C. Streptav idin-perox idase was th e detec tion sys tem with di amin obenzidine as the chro moge n. Sec ti o ns we re co unte rsta ined with he matoxy lin . Nega ti ve co ntro ls in c luded sec ti ons co ntaining a va riety of bac te ri a, fun ga l or protozoa l organisms including Aspergillus j7av lIs and Toxoplasma gondii. Another negative control was substitution of the spec ific PC probe by an unrelated probe targe ting the 5S rRNA of Asp ergi llu s nu c le ic ac id s 1-2 2. Human lung naturall y in fec ted with PC was used as pos iti ve control.

Polymerase chain reaction

T he po ly me rase c ha in reac ti o n (PC R) was perfor med according to es tabl is hed protoco ls us in g f roze n or fo rmalin- fixed , paraff in- e mh edd ed lun g. (Wakefie ld et a l .. 1990 : Lee et aI. , 1993: Lu e t a l .. 1994) . Brie fl y. 50 Jim -thi ck paraffin sec ti o ns were deparaffini zed or thawed, homogeni zed, and centrifuged. Pe ll ets we re was hed with PBS-EDTA buffe r seve ral times. The pe ll et was resuspended and di ges ted with protei nase K diluted in diges tion buffer and incuba ted at 55 °C fo r 45 min. Th e mi xture was ex trac ted with phenol and chlorofo rm. The DNA present in the aq ueous phase was prec ipitated with eth anol whi ch was then removed by vacuum drying. The DNA was di sso lved in 50 pI o f Tri s-E DTA buffe r (PCR so luti o n). Th e o li go nu c leo tid e primers used for both pa raffin and frozen samples were pAZI02-E (5'-GATGGCTGTTTC CAAGCCCA-3') and pAZ I02- H (S'-GTGTACGTTGC AAAGTACTC-3') , that target the mitochondrial rR NA codin g reg ion in rat- and hum an-deri ved PC DNA. Amplification with these primers results in a frag ment of approx imate ly 350 base pairs (bp). [n addition, a nested PCR with primers amplifying the inte rnal transcr ibed spacer (ITS) was used. The nested PCR was perfo rmed using primers 1724 F (5' -AAGTTGGTCAAATTTGGT C-3 ' ) and [TS2R (5'-CTCGGACGAGGATCCTCGCC-3' ) for the first step and primers JTS IF (5'-CGTAGG TGAACCTG CGG AA GGATC-3 ' ) and [TS2 R I (5'­GTTCAGCGGGTG ATCCTGCCTG-3 ' ) for the second s tep (nes ted ) (Lu e t a I. , 1995). Ampli ficat io n of a diagnosti c fragment of 550 bp is specifically obtained with the ITS nested PCR fo r P. carinii.

PCR with pAZ primers was performed in a 100 pI mi xture containing template DNA, PCR buffer, 20 pmol of PCR primers . 0.2 mM of the fo ur deoxy nucleos ide triphosphates, and 2 U of Taql DNA polymerase. PCR was performed in two stages. The initial stage was 35 cyc les, with each cycle consisting of 1.5 min at 94 °C. 1.5 min at 55 °C, and 3 min at 72 0C. The final stage was extension at 72 °C for 10 min . The PCR products were elec trophoresed in a 2% agarose ge l and stained with ethidium bromide to determine the sizes of the ampli fied products. For nested [TS PCR the same conditions for th e pA Z primers we re used , but th e ann ea lin g temperature was 50 °C fo r the first PCR reac ti on and 65 °C fo r the second nes ted PCR reaction (Lee et aI. , 1993; Lu et al .. 1994). Rat lung experimentall y infec ted with PC was used as a positi ve control. Non-infected rat lung and liver from all three pigs were used as negati ve contro ls .

Bacteriological and virological examination

Lung, spleen, li ve r and small intestine were cultured for bac teri al pathogens . Sampl es of spl ee n, li ve r and lun g were tes ted by flu oresce nt antibody and viru s isolation fo r pseudorabies virus. and lung was tes ted by bo th me th ods fo r influ e nza viru s, a nd po rc in e

reproducti ve and respiratory syndrome (PRRS) virus.

Results

Gross lesions

All pigs were emaciated. Pig No. I had a rough hair coat with areas of hyperkeratos is on its dorsum and tail. Th e peri ca rdial sac and the epi cardium we re alm os t co mpl ete ly adh ered to one an oth er. T he lun gs we re diffusely congested with consolidation of the left middle lobe . Tracheobronchial and cervical lymph nodes were enlarged. Pig No. 2 had also a rough hair coat with areas of alopec ia and hyperkeratos is along the head and spine. An umbili ca l abscess was no ted . Th ere was mild fi brinous peritoniti s. The lungs d id not co ll apse , we re palpabl y firm and mottled red. The craniove ntral lobes we re conso lidated. Pi g NO. 3 lun g had crani o-ve ntral atelec tatic areas with lobular pattern .

Histopathology

An interstitial pneumonia associated with organisms co nsistent with PC was present in all pi gs examined although organi sms and the intensity of the les ions were much more ex tensive and severe in pigs No.2 and NO.3. The alveolar septa were conges ted and thickened with a mild to moderate number of lymphocytes. plasma ce lls . macrophages and fewer neutrophils. The alveolar lumen. and occas iona ll y the al veo lar du cts and res piratory bronchioles contained edema fluid. fibrin, and a foa my eos inophili c mate rial (F ig. I) th at wa s strong ly PAS pos iti ve and consisted of small round bodies (Fig . 2). Most of thcsc bodies had si ngle central basophilic dots, and were considered PC trophozoites . Some bodies had a thicker ce ll wa ll and multiple (up to 8) basophilic dots (F ig . 3) and we re co ns ide red PC cys ts co nt a inin g intracysti c bodies. A GMS stain stained the cys t wa ll in a ll pi gs (F ig. 4). Pigs No . 2 and No . 3 had a lso a hac teri al bronchopneumonia affec ting the crani al lobes with numerous neut rophils and fewe r macrophages in the small airways and alveo li . In addition. pig No.3 had peribronchi olar and peri vasc ul ar Iymphoplasmacy ti c aggregates. Nccropsies of 6 other I-month-old pigs from the fa rm where pigs No. I and No.2 where housed did not reveal PC but multipl e bac te rial pathogens we re isolated.

In situ hybridization

Lung from pig No . I was negati ve when tested with a probe for PC rRN A but lung from pigs No. 2 and No.3 gave an int ense g ranul a r bro wn reac ti o n (Fi g. 5 ) th at was loca li zed in the areas where PC orga ni sms we re o bse rve d by hi s topa th o logy and e lectro n mi c rosco py. Mos t o f th e reac ti o n a ppeare d to be re lated to troph ozo it es. T he re was no bac kg ro und staining. Pos iti ve and negati ve co nt ro ls perfo rmcd as ex pected.

Electron microscopy

Th e lun gs fr o m pi gs No . 2 and NO.3 we re examined . The al veolar surfaces were covered by single or multiple laye rs of irregular shaped bodies, 3-5 pm in le ng th. w ith a 20-25 nm ce ll wa ll (Fi g. 6) . Th e morph ology of these organi sms wa s consistent with trophozo ites of Pc. Variable numbers of fi lopodia , less than 100 nm in diameter, were ari sing from the surface of the tro phozo ites . Cross sec tion of these ex pansions revealed that they had a central hollow core surrounded by spikes (Fig. 7) . Tubul ar ex pansions from adjace nt trophi c fo rms were int ermingled or ve ry close to the surfa ce of al veo la r ce ll s but no direc t co ntact was o bse rve d . Ty pe I pn e um ocy tes had cy to pla s mi c projec tions that partiall y surrounded trophozo ites . Othcr s tru c tures. ide ntifi ed as PC cys ts, we re 3-5 flm in diameter. round to ovo id . and were intermingled wi th trophic forms (Fig. R) . The wall of the cysts was 80-90 nm in thickness. with a smooth surface and had three identi fiable laye rs: an outer dense laye r, a middle and thi c ke r e lec tro n- lu ce nt laye r , and an inn e r ce ll melnbrane. Most of the cysts had one or more (up to 6) intracys ti c bodies. Some cysts were collapsed. crescent­shaped, and empty. The ratio of trophozoites:cysts was usually higher than 100: I. Both trophozoites and cysts were seen in the interstitium .

Polymerase chain reaction

There werc no PCR-amplified products when PCR with pAZ primers and nested ITS PCR were perfo rmed on frozen or paraffin -embedded lung ti ssue of pi g No . I. Lung sampl es (paraffin-embedded and frozen) from pigs No.2 and 3 were amplified by both types of PCR. The molecular size of the ampli fied products for PCR with pAZ primers and for nested ITS PCR was approx imately 350 bp and 550 bp respec ti ve ly (Fig . 9) . Pos iti ve and nega ti ve controls performed as ex pected.

Microbiology and virology

Flu orescent antibody tes t and virus isolation were negati ve for pseudorabies, PRRS , and sw ine influenza in all pigs . Haemophilus parasuis was isolated from lung of pigs No . I and NO. 3 and StrepTOCOCCl/S suis type II was is o lated from lun g of pi g No . 2 . Actin omyces pyo!<encs and Pasteurella mulTOcida were isolated from the umbili cal abscess. Bordetella bronchiscptica was also isolated from lung of pig NO. 3.

Discussion

concurrent bacterial pneumonia (Travis. 1994). Swine pncumocystosis is more co mmon between 6 and II weeks of age (Kucera et al., 1968; Seibold and Munnell. 1977: Bille- Hansen et al.. 1990: Kondo et al.. 1993). The int e rstitial pneum o nia is initiall y multifocal but eventually becomes diffuse. Although microorgani sms capable of producing pneumonias were isolated from all pigs. the most likely cause of the pulmonary interstitial les ions was Pc. The lesions found in the cranial lobes of pig No . 2 were most likely produced by SlreplOCOCClI.I'

.I' ll;.\' type II and the cuffing pneulllonia in pig No .3 by Mycop/o.\"II/{/ sp. although there was no isolation of this on!an ism. Bacterial or viral infec tion s ha ve been reported in cases of swine PC pneumonia (Kucera et al .. 196R: Seibold and Munnell. 1977 ; Morin et al .. 1990: Kondoetal .. 1993) .

Incomplete descriptions of th e ultra structure of sw ine PC are avai lab le in the literature (Se ibold and Munnell. 1977: Kondo et al.. 1993) . PC from pigs No.2 and No.3 were morphologically indi stinguishable from other PC by electron microscopy (Bedrossian. 1989) . We detected two diffe re nt stages. the more numerous trophozoite. usually with filopodia. and the cyst stage. It has been hypothesized that filopodia are in vo lved in the reproduction of trophozo ites. transfer of nutrients and/or the attachment to alveolar cel ls (Bedross ian. 1989). None of these hypotheses has been proven convincingly.

We observed that trophozoites were in close contact with pneul1loc ytes with the epithe li a l cel ls "embracing" trophozo ites by means of cytoplasmic projections. This contact required the smooth portion of the trophozoite ce ll wall to undul ate and conform to the irregularities of the pneumocytes. but no filopodia were observed in the region of contact (Bedrossian, 1989: Pottratz and Martin. 1994). Close contact between epithelial alveolar cells and cysts was not seen. Receptors in the alveolar epithel ium and macrophages for vitronectin. fibronect in, mannose. surfactant protein 0 or the Fc fraction of immunoglobulin s in macrophages mediat e the attachment of PC (Pottratz and Martin , 1994: Limper. 199 5) . PC contains a major mannose-rich surface antigen complex termed glycoprotein A (Pottratz and Martin. 1994: Limper. 1995). It has been suggested that the susceptibility of HIV-infected patient s to PC is re lated to impairm ent of the al veo lar ma cropha ge mannose receptor by HIV (Koziel et al., 1993). PC not only attaches to l1lacrophages, but when this binding is through l1lannose (glycoprotein A) or immunoglobulins (Fc receptor in macrophages). it activates macrophages ancl phagocytosis of PC (Pottratz and Martin , 1994).

Rib oso mal RNA are abundant RNA sequenc es found within all eukaryotic and prokaryotic cel l types. These seq uences are conserved, and they are widely utili zed for the phylogenetic classification of infectious

organi sms (Olsen and Woese . 1993 ). Us ing non isotopic in situ hybridization (ISH ) we demonstrated the presence of rRN A spec ific for PC in the foamy alveolar ex udate. This technique proved to be simple and quick to perform (Montone , 1994). Other advantage of thi s technique was an intense and spec ifi c reac ti on in paraffin -e mbedded ti ss ues. The intensity of th e reacti on usin g paraffin embedded I ung sections of pig No. 2 was comparable to the human control. The sy nthesis of oligonucleotides for di fferent areas of the PC genome might be appropriate fo r retrospec ti ve epidemi olog ica l studi es and class if i­cation of PC isolates .

T he peR tec hniqu e proved to be a re liabl e and specifi c method to detec t diffe rent components of sw ine PC ge nome. Primers specifi c for PCR amplifi ed the genome of sw ine PC in pi gs No. 2 and 3. either using frozen or fo rm alin -fix ed and paraffin -e mbedded lung ti ssue. PCR is being used frequentl y in the di agnos is of hum an PC pneum oni a as we ll as for e pide mio log ic studies (Wake fi eld et al. . 1990: Lee et al. . 1993: Lu et aI., 1994). We used di fferent primers for mitochondri al rRN A, and for the ITS reg ions . Amplificati on of ITS

regions is bei ng used in human beings and laboratory animals for typing of PC strains (lu et al. . 1994) and has been conside red by some authors one of the best PCR methods to detect human PC (lu et al.. 1995). Due to the lack of other stud ies in sw ine we could not determinc whethe r thi s approach will be suitable in the porcine 'pecics. Although PCR has been described to detect PC in infec ted horses and dogs , the primers used did not include those for ITS regions (Peters et al. . 1994; Sukura eta l..1 996).

Samples from pig o. I did not hybridize with the spec ific probe for PC and did not amplify the PC specific DNA band by PCR usi ng the same conditions and primers as those for pigs No.2 and No.3. Further­more. sa mples from a ll pigs were processed in an identica l way. These results are puzzling and a definitive cxplanation is lacking. One possibility could be that due to the multifocal nature of the foa my exudate in pig No. I. sampling may have influenced the detection of PC when usin g frozen section s. Howeve r. both ISH and PCR techniques were also done using paraffin sec tions wit h organisms resembling PC. as demonstrated by HE and GMS stain s. although much less abundant than in thc ot her two pigs. Another possibility entertained was the existence of different strai ns of PC in swine. as it has bee n described in other spec ies (Gig liotti et al. , 1993). but pigs No. I and No.2 were housed in the same barn

whi ch makes it more lik e ly th at both anim a ls we re infec ted with the same strain of Pc. Furthermore. pAZ primers were designed on conserved regions of PC and amplify rat. ferret. human and other PC strain s which makes unlikely that PCR negati ve result in pig No. I was due to the presence of a different strain. Sensit ivi ty of both ISH and PCR may be an iss ue in this case. It has been show n that in some cases ISH using digoxigenin la be led probes is more se nsi ti ve than whe n biotin labe lled probes arc used (McQuaid et aI., 1995). In our case we used only biotin labe lled probes.

We conc lud e that in add ition to th e "c lassic" morphologica l methods to study and diagnose pneumo­cys to s is in th e porcine spec ies, molecular biology tec hniqu es ca n be s uccessfull y used in porcine PC infect ions. Future researc h usi ng molecular biology tec hniques will include epidemio logic studi es in the swi ne popUl ati on as well as the complete seq uencing of the amp li fied porcine PC products and comparison with those from other species.
