1. Introduction
A formal consultation on the proposed changes to the management structure across NTs, OOH and MDT co-ordinators was launched on the 3rd October 2018 and closed on the 2nd
November 2018.
The consultation document set out the proposal to create an integrated area management structure across the north and south localities. This structure proposed the incorporation of the OOH’s nursing service into the NT structure, replacing the existing MDT co-ordinator role with a new patient flow co-ordinator role and a new management structure of nursing, therapy and mental health area managers.
This consultation directly affects: All substantive Band 8a Managers within the current structure. All substantive Band 7 neighbourhood team managers. All Out of Hours Managers. All substantive Band 5 MDT Co-ordinators working within neighbourhood teams. All staff currently in seconded roles of NT Manager, Area Manager and OOH
Manager posts.
2. Consultation Engagement and Feedback
Following the two separate consultation launch meetings held with NT Managers and MDT Co-ordinators on 3rd October, there has been a series of opportunities for affected staff and others to respond to the proposals.
This has included: Q&As from the consultation launch were made available on the trust intranet. All consultation documentation including job descriptions has been made available
on the trust intranet. All staff were invited to meet with senior managers individually to discuss their
individual circumstances, with 25 members of staff holding 1:1s with a manager. All staff were invited to speak with their Staff Side representatives and HR staff. Several teams invited senior directorate management including John Martin, ADO
and Clinical Director Ben Underwood to visit the staff to discuss the proposed changes. Two staff questionnaires for NT staff and OOH staff were created in order for all staff to have the opportunity to comment on what work is like for them at the moment. Over 80 responses from NT staff and over 20 responses from OOH staff have been received to date.
A consultation email address was also set up to receive comments and representations from all staff members. Over 40 email responses have been received, from a cross section of NT and OOH teams and staff groups.
The following summary brings together the main areas of feedback raised from individual meetings with senior managers as well as the email responses received through the consultation period.

3. Summary on Feedback and Changes as a Result of the Consultation
The following points were highlighted by many of the comments and responses received:
The size of the job role for the Nursing Area Managers. The size of the job role for the Patient Flow Co-ordinators. The lack of resilience for the Business Support Co-ordinators. The lack of clarity regarding the management structure for OOH. The impact of the new NT management structure on Band 7 and Band 6 posts
across NT teams.
The directorate’s Divisional Management Team (DMT) met on 6 th November 2018 to review the responses and consider options for the way forward.
Key areas considered by the DMT which were used to inform the final outcome of the consultation included:

4. Main Areas of Feedback and Key Reasons for Changes
4.1 OOH Management
Feedback from large numbers of OOH staff noted a range of concerns about the proposed changes. These included a lack of clarity about line management and day to day responsibility for OOH teams, the alignment of OOH services who deliver pre-dominantly unplanned care with NTs delivering predominately planned care services, and ways of enhancing support and resilience for OOH teams. Feedback was also noted regarding the responsbility for management of OOH rotas, and clarification of the NT Duty Practitioner’s clinical responsibilties including triage.
As a result of the outcome of this consultation, the proposal to remove OOH managers and replace them with a NT Duty Practitioner has been withdrawn.
Going forward, the OOH Manager posts will remain. However, in order to offer greater support and resilience to out of hours teams across the directorate, OOH services will move to sit within the county wide Unplanned Care management structure.
The directorate is committed to strengthening the unplanned care offer to provide rapid community response and support admission avoidance across the county, and in 2019/20 will be considering the next phase of service transformation across unplanned care services.
Therefore effective from 7th January 2019 the OOH Managers for Cambridge, Hunts and North will receive line management from the JET Clinical and Operational Manager as OOH services move across into Unplanned Care.
4.2 Clarification regarding Advanced Clinical Nurse Practitioner role (ACNP)
The directorate remains committed to the ACNP role strengthening advanced nursing leadership, driving clinical care and quality across NTs. The ACNP model was introduced to strengthen clinical leadership through clinical supervision, supervisory role modelling, educating nursing teams, competency assessing, coaching band 6’s , leading audits, leading on quality improvement projects and driving nursing research forward.
We recognise that NTs have large numbers of new and relatively junior nurses and nurse associates in post, who require ongoing clinical leadership development to drive forward embedding a quality culture, focusing on quality improvement and delivering efficiencies in clinical productivity.
We can therefore clarify that ACNPs will not be operationally responsible for the day to day line management of Band 6 DNs and the NT nursing teams. This will sit with the Area Nursing Managers, with support from the Business Support Co-ordinators. However ACNPs will be expected to complete regular clinical supervision with nursing staff, as well as actively participate in the annual appraisals of Band 6 DNs within their NTs.
The directorate recognises that the proposed line management structure of Band 6 nursing staff being line managed by Band 8a posts is different from the Band 7 Advanced Therapy and Mental Health Practitioners who are currently responsible for line management and clinical supervision of Band 6 staff.

However, we feel that strong clinical leadership and focus on quality within therapy and mental health teams is better established across the directorate, and that going forward nursing within the NTs must maintain its current drive to strengthen quality and clinical leadership led by the ACNPs.
4.3 Number of ACNP roles
The directorate is currently carrying a number of ACNP vacancies across the 14 NTs.
In order to balance the requirement for day to day operational management of NT nursing teams whilst driving forward and embedding a culture of quality and safety, we have agreed to use 4 of the vacant Band 7 ACNP vacancies to fund 2 additional Band 8a Nursing Area Manager posts and 3 additional Patient Flow Co-ordinator posts.
The additional Patient Flow Co-ordiantor will support the clinically focused ACNPs to identify patients known to NTs to facilitate early discharge back to their homes and highlight complex cases requiring ACNP intervention.
In agreeing the revised allocation of 10 WTE ACNPs across 14 NTs, we have considered both the number of GP practices currently supported by each NT in addition to activity data related to numbers of unique patients receiving care by District Nursing each month. Use of this measure rather than current DN caseload size accounts for dormant patients who may remain inactive on NT caseloads.
The allocation of ACNPs across the directorate will therefore be:

4.4 Nursing Area Managers
Many of the responses to the consultation voiced concern regarding moving NT management and oversite away from sitting within each NT. One of the other key concerns was the number of direct reports for each Nursing Area Manager role.
Based on this feedback, further analysis of headcount of direct reports for Area Nursing Managers has been reviewed, as well as clarification regarding the line management of OOH services (see 4.1) and the role of ACNPs in line managing the Band 6 DN posts within each NT(see 4.2).
Some of the Area Nursing Manager posts will also be expected to line manage a locality Business Support Co-ordinator, and all Area Nursing Managers will manage at least one Patient Flow Co-ordinator. Given the Business Support and Patient Flow co-ordinator roles are both new to the directorate, it is felt that they will require significant development and support to ensure postholders work in a standardised, efficient and effective way which will be driven by the Area Nursing Managers.
Therefore, as a result of the outcome of the consultation, we have increased the number of Area Nursing Manager posts from 6 to 8 WTE. The final distribution of Area Nursing Managers is:

4.5 Line Management of NT Therapy and Mental Health Staff
In considering the number of direct reports for the Area Nursing Manager posts, the number of direct reports for the Area Therapy Managers and Area Mental Health Managers in the north and south localities were also compared to ensure equity across all the Band 8a roles.
To clarify queries received from the consultation, the line management, appraisals and clinical supervision of Band 7 Advanced Clinical OT and Physiotherapy Practitioners will be undertaken by Area Therapy Managers. Area Mental Health Managers will undertake the line management, appraisals and clinical supervision of Band 7 Advanced Clinical Mental Health Practitioners in addition to other non NT staff. The diagram in appendix one has been amended from the consultation paper to reflect this.
4.6 Development and Support for Band 7 and Band 6 Clinical Staff
A common theme from the feedback included the need for both Band 7 and Band 6 clinical staff including nurses, therapists and mental health staff to be supported in the development of their operational management and clinical leadership skills.
Some staff fed back that the current NT management structure had resulted in the Band 6 DNs being de-skilled in leading their nursing teams, and many comments reflected the need to provide robust training and development opportunities for both Band 7 and Band 6 staff in clinical leadership, caseload management and supervision and appraisals.
Part of implementing the new management structure from early 2019 will include the delivery of a series of workshops for these clinical staff delivering bespoke leadership training to support the development of these skills.
4.7 Integration of Discplines within NTs
A range of responses were received regarding the impact of the new discipline specific management structure on existing progress to integrate nursing, therapy and mental heath staff working together within NTs.
The directorate is absolutely committed to promoting virtual or horizontal integration through closer networking, sharing and joint working of different disciplines within NTs, as well with other directorate services in unplanned care and specialist services.
The new NT management structure in each locality will be made up of Band 8a senior managers representing nursing, mental health and therapies. It is the expectation that

this integrated area management team will work closely together to deliver the performance of NTs, sharing resources, providing support and resilience, sharing learning and promoting integration through networking, enhanced communication and support.
NT meetings will continue to be held as a collective group of multi-disciplinary staff, with the Area Nursing, Therapy and Mental Health Managers driving the agenda.
4.8 Number of Patient Flow Co-ordinators and Role
There was an overwhelming response regarding the number of patient flow co-ordinator posts proposed in the consultation, as well as concerns regarding:
the number of GP practices that each patient flow co-ordinator would be responsible for working with and impact on workload.
the grouping of Ely and St Ives NTs across one patient flow co-ordinator post. lack of clarity regarding the wide range of tasks the role is expected to
undertake. the change in banding from Band 5 for MDT co-ordinator to Band 4 for Patient
Flow Co-ordinator. Resilience and cover for sickness and annual leave.
Having considered the feedback carefully, the following response can be made:
The Patient Flow Co-ordinator role is different from the MDT Co-ordinator role.The job description for the new role was job matched by the Trust to a national Agenda for Change job profile of MDT Co-ordinator. This is a Band 4 nationally.
The Patient Flow Co-ordinator role will be line managed by the Area Nursing Managers in order to ensure that core tasks undertaken by post holders are standard across all NTs.
To re-clarify, the key duties of the patient flow co-ordinator role will be:
To review the urgent care dashboard daily, identify if patients known to NTs have been admitted, provide patient-level information in to hospitals, and liaise with hospital ward staff and NT staff to pull patients from hospital back in to community based NT care.
To co-ordinate weekly NT based multi-disciplinary team meetings to review patients on active nursing, therapy and mental health caseload patients plus other patients referred
To facilitate and attend monthly meetings with each GP practice to review patients identified by NT staff and GPs as beneficial for discussion.
To case review the top 20 ‘at risk’ patients for each GP practice each month and agree next steps related to community NT care. This Gold Standard Framework meeting needs to be considered part of the monthly GP meetings, and Patient Flow Co-ordinators will be expected to work with their Area Nursing Manager to agree the processes to best manage meeting with GPs each month.
Further analysis by NT of number of GP practices, population size and current number of active patients on DN and rehabilitation caseload has been undertaken in order to review the number of patient flow co-ordinator posts and the distribution across the NTs.
Therefore, as a result of the outcome of the consultation, we have increased the number of Patient Flow Co-ordinator posts from 7 to 10 WTE. This also takes into consideration

the reduction in the ACNP posts. The final distribution of Patient Flow Co-ordinator posts is:

Patient Flow Co-ordinators will be line managed by an Area Nursing Manager and they will be aligned to an ACNP to promote system wide working.
Given that there are number of posts supporting each locality, it is the expectation that cover and support in the event of sickness and annual leave will come from the pool of Patient Flow Co-ordinators within each area (Peterborough, EC&F, Cambridge and Huntington).
4.9 Resilience for Business Support Co-ordinator
Most of the responses to the consultation regarding this new post were largely positive.
However the large number of tasks this role is responsible for, the lack of resilience and support to cover sickness and annual leave, and the need for development and training were highlighted as concerns across staff feedback.
Some staff recognised that time would be released back to clinical staff by this role undertaking more of the administrative operational management tasks for NTs.
It has been agreed that professional supervision and cover for the Business Support Co-ordinators will come from the Deputy Administration Manager and the admin hubs. The line management and day to day management and work allocation of Business Support Co-ordinators will come from Area Nursing Managers.
Therefore the number of Business Support Co-ordinators remains as per the consultation as 4 Band 5 WTEs, with one in each of the 4 localities.
Given the breadth of the role, the directorate is committed to providing training for the Business Support Co-ordinators to ensure they are able to provide support to clinical teams. This will include for HealthRoster, Trac, finance systems, MI reporting, Datix risk management, HR processes, e-Expenses management, AL management, procurement systems and other individual requirements for development.
Part of implementing the new management structure from early 2019 will include the delivery of a series of workshops for these clinical staff delivering bespoke training to support the development of these skills.

4.10 Other areas where clarification was requested
4.10.1 Role of Deputy Locality ManagersThe Deputy Locality Managers will operationally manage the integrated area management teams in the north and south localities. These roles will be pivotal in developing and delivering the organisational development needs of NT staff, driving clinical and operational leadership and embedding a culture of quality improvement, empowerment and closer working within NTs.
4.10.2 Budgetary ManagementEach Area Nursing, Therapy and Mental Health Manager will be responsible for the NT budget as a whole and will need to work together across displines to decide how best to use the budget. Collective cost improvement savings will need to be considered as an integrated management team.
Budgets will therefore not be separated out for the Area Nursing and Therapy Manager posts. However the Area Mental Health Manager will also have budget responsibilities for the teams that sit outside of the NT structure.
Budget codes will be re-aligned by finance to support the new area management structure.
4.10.3 Job Title of Band 7 TherapistsThere is no change to the Band 7 therapists’ job title, which was incorrectly stated within the consultation paper as Clinical Lead. The correct job titles are Occupational Therapist and Physiotherapist Advanced Practitioner.
4.10.4 Serious InvestigationsIt is the expectation that all clinical staff at Band 7 and above will participate in the directorate’s serious investigation process.
4.10.5 Service Line ReportingEach integrated area management team of Area Nursing, Therapy and Mental Health Manager is expected to attend Service Line Reporting for their localities as a group, and may invite Band 7 staff within their respective NTs as a development opportunity as agreed.
4.10.6 GP EngagementThere is an expectation that GP engagement will continue at various levels within the new NT management structure.
Patient Flow Co-ordinators will communicate regularly with GP practices to support the patient flow of complex community patients across the system. Band 6 clinical staff will continue to work closely with their GP practices to deliver joined up patient care.
The Area Nursing, Therapy and Mental Health managers will facilitate strategic partnership working with pimary care in their respective localities working to the ambition of both NTs and the trust.
4.10.8 Cost SavingsIt was outlined within the consultation that one of the key drivers was to increase efficiency and reduce expenditure as part of the directorate’s contribution to achieve a £3m CIP savings this year.

5. Implementation
Current arrangements regarding reporting and managerial arrangements will remain in place until the implementation date of mid February 2019. However a transitional period may be applicable to ensure continuity of service delivery.
6. Recruitment
Preference forms/slotting in/normal recruitment.
The process for recruitment will remain as proposed in section 10 of the consultation document.
Wherever possible we will slot staff in to their first preference however we are aware that there may be some incidences where no suitable preference is available.
In the event where there are more staff stating a preference than there is posts, we may have to conduct interviews under restricted competition in order to make decisions regarding who goes where. In this case interviews will be conducted by senior managers within the directorate.
The preference form can be found attached to the Way Forward letter sent to all affected staff.

Job PurposeThis post is a key management role within the integrated area management team within a defined north or south locality.
* The Area Nursing Manager will work collectively with the Area Therapy Manager and the Area Mental Health Manager/ OPMH Clinical Manager to ensure the neighbourhood teams or relevant clinical teams within their locality deliver high quality community services for clients.
* The post holder will be expected to provide clear leadership and take management responsibility for the effective delivery of the relevant nursing staff within the neighbourhood teams.
* The post holder will be also responsible for the day-to-day operational functioning of the nursing staff in their neighbourhood teams and be accountable for their performance. With respect to this, the manager will receive regular constructive supervision from his/her direct line manager.
* The post holder will work closely with their nursing leadership team to deliver a quality led integrated service.
* The post holder will also work to develop and maintain excellent working relationships with their locality’s’ GPs and primary care teams ensuring regular meetings are held and attending engagement events.
Role of the DepartmentNeighbourhood teams aim to provide multi-disciplinary, seamless care closer to patients’ homes, reducing avoidable admission to hospital and facilitating hospital discharges.
Staffed by community nursing, therapy and mental health clinicians, neighbourhood teams provide integrated care for populations of 30,000- 50,000, based around GP registered lists.
The teams proactively care for those with long-term conditions, the dying, care home residents, and mental health service users’ conditions where care can be provided at home.
Main Tasks, Duties and Responsibilities1. Have overall management responsibility for their respective service including being
accountable for all aspects of performance for their services within the directorate’s Service Line Reporting (SLR) framework
2. Undertake the day-to-day team management for nursing teams providing support and lead on service development in collaboration with team members. This will involve a coordinated service delivery where policy and practice are regularly reviewed and evaluated, in-order to ensure evidence based effective practice
3. Ensure that the arrangements defined within the team’s Operational Policy for accepting, prioritising, dealing with referrals, and planning clients’ discharge are met
4. Ensure that teams are able to provide a responsive and effective service in line with the current service specification for Neighbourhood Teams

Job Purpose This post is a key management role within the integrated area management team
within a defined north or south locality.
The Area Therapy Manager will work collectively with the Area Nursing Manager and the Area Mental Health Manager/OPMH Clinical Manager to ensure the neighbourhood teams or relevant clinical teams within their locality deliver high quality community services for clients.
The post holder will be expected to provide clear leadership and take management responsibility for the effective delivery of the relevant therapy staff within the neighbourhood teams.

Staffed by community nursing, therapy and mental health clinicians, neighbourhood teams provide integrated care for populations of 30,000- 50,000, based around GP registered lists.
The teams proactively care for those with long-term conditions, the dying, care home residents, and mental health service users’ conditions where care can be provided at home.
Main Tasks, Duties and Responsibilities1. Have overall management responsibility for their respective service including being
accountable for all aspects of performance for their services within the directorate’s Service Line Reporting (SLR) framework

Job PurposeThe post is a fundamental role within the neighbourhood team structure. The role has the main responsibilities of providing full business support to neighbourhood teams within a locality.
The post holder will work closely with Nursing, Therapy and OPMH Mental Health Area Managers to support with business delivery for the Neighbourhood teams.
The post holder will also have delegated authority and responsibility to act independently and to handle and deliver local decisions and actions with regards to the business functions of neighbourhood teams within a defined locality.
Role of the DepartmentNeighbourhood teams aim to provide multi-disciplinary, seamless care closer to patients’ homes, reducing avoidable admission to hospital and facilitating hospital discharges.
Staffed by community nursing, therapy and mental health clinicians, neighbourhood teams provide integrated care for populations of 30,000- 50,000, based around GP registered lists.

Job PurposeThe Patient Flow Co-ordinator is a fundamental role within the neighbourhood team structure. It is responsible for the co-ordination and tracking of complex patients on neighbourhood team caseloads across health and social care teams such as acute hospitals, GP surgeries, social services, voluntary organisations and other CPFT services.
This role will co-ordinate clinical meetings, and liaise with teams, services, wards and GP practices to facilitate the provision of safe, timely, joined up care and services delivered by neighbourhood teams. Patient flow co-ordinators are key in supporting neighbourhood teams to prevent hospital admissions and ensure safe hospital discharges.

Close and regular liaison with patient’s GP practices to ensure that practices are up to date with each patient’s current health status, acting as a conduit for two way communication and updates regarding specific patients known to GP practices and community health services including neighbourhood teams, specialist services, urgent care services and community hospitals
5. Liaise with Community Intermediate Care Services and other community based services to support timely hospital discharges including ensuring any follow up appointments have been booked and transportation is arranged
6. To build and maintain relationships with GP practices, Community Therapy, Mental Health Teams and Community Specialist Services, Adult Social Care and third sector providers to ensure timely transfer of information between the teams.
7. To develop and sustain effective communication with wards at Addenbrookes, Hinchingbrooke, Peterborough and Queen Elizabeth Hospitals to feedback the outcomes of individual patient referrals including interventions carried out and details of any ongoing support that may be required including when active MDT patients are admitted into the hospital and ensuring that vital information regarding current

8. To deal in a professional, helpful and sensitive manner with service users, staff and other agencies by telephone or face to face, taking messages, advising patients about visits, referring other issues as appropriate and answering routine enquiries

This may including taking minutes at MDT meetings, and other patient focused meetings, photocopying, faxing, dealing with other forms as required to support the timely delivery of clinical services to patients.
10.To extract caseload information from various clinical systems, and undertake analysis of information for audit, performance management and capacity purposes, including data entry, retrievals, manipulation and reporting formatting & to feed back to team members.
11.To be responsible for maintaining a service directory and updating on a regular basis; sourcing referral criteria, understanding how services are accessed, ensuring information is available to all members of the wider teams
12.Allocation of urgent same day response to appropriate available Service. Allocation of visits to clinician’s rota’s based on triage and wait times. Rearrange appointments when required e.g. if urgent appointment is booked reschedule of other appointments to another day or clinician
13.Completion of the Delayed Transfers of Care (DToC) spreadsheet each week and monthly statistical provision for informatics as requested

Job PurposeTo work autonomously taking overall responsibility for nursing care, quality and clinical activity within the Neighbourhood Teams by providing advanced professional nursing leadership and guidance, clinical advice and management in relation to the key dimensions of health care quality: To educate, develop and clinically lead nursing staff within the NT to deliver
community nursing interventions and to support the care of patients with complex needs.

To provide the highest standard of clinical nursing care by using advanced assessment and diagnostic reasoning skills along with expert knowledge and critical understanding. Holistically assess patient’s needs and instigate and provide clinical treatments based on evidence based practice.
Be responsible for teaching and developing staff to enhance their practice and skills to manage capacity, caseloads and patient complexity.
Be advisory and supervisory role models; oversee community nursing caseloads and safer staffing.
To work with our local GP’s and Partner Agencies to provide care approach to a defined high risk patient group with complex long term conditions with the aim of preventing hospital admissions/ readmissions and facilitating timely discharge.
To take the lead in collaboration with other disciplines and agencies to facilitate inter-professional/ agency working with the aim of supporting people to maximise their health and well-being in the community.
Clinically lead and support the team to recognise, prioritise and respond appropriately to urgent and emergency situations including the assessment and management of risk.
Key Responsibilities
Clinical / Service Specific1. As an autonomous practitioner, working as an integral member of the community
nursing service the post holder will be expected to take a lead role in the advanced clinical assessment, diagnosis, management and treatment of patients who require case management in a community setting
2. To use advanced nursing, clinical, diagnostic and prescribing skills, to carry out a thorough holistic assessment, including a systematic physical examination, to develop a patient centred comprehensive care plan.
3. To initiate and provide advanced clinical/therapeutic care treatments, in partnership with other providers, based on best possible evidence that will improve health outcomes
4. To use advanced clinical skills and expert knowledge to proactively identify subtle changes in the patient’s condition as early as possible and to manage these in a manner that optimize well-being.
5. To be responsible for teaching and developing staff to enhance their clinical practice to manage capacity and patient complexity.
6. To investigate serious incidences and ensure learning from these incidences is embedded within the team.
7. To work closely with other members of the team, provider services, health and social care partners and GP to ensure that individuals are enabled to live at home with dignity and independence and empower them to reach and maintain their optimum function
8. To actively promote self-management which enables the patient to proactively manage their long term condition and know how to confidently manage or respond to any expected exacerbation or deterioration in their condition.
9. To work collaboratively with the patient and or family/career in the use of technology including tele-technology in the management of their condition.

20.Be leads for the consistent and equitable approach of sharing of clinical updates, supporting staff with any change in processes to ensure these are transferred into practice and enabling continuous professional development of self and others through the development of Professional Development Groups
21.Work in partnership and collaboratively with primary care, all disciplines within the neighbourhood teams and professional leads.
22.Be responsible for clinical supervision for band 6 nurses, embedding a reflective, continuous learning culture driving highly quality, safe, effective practice.
23.Ensuring the learning environment is conducive for students; provide mentoring and coaching to staff, Post Graduate Students and pre-registration nursing students.
