HISTORY OF C A S EIn t h e c a s e we h a v e t o r e c o r d , an u n m a r r i e d man, a g e d 2 3 ,e x p o s e d h i m s e l f t o i n f e c t i o n on March 1 3 t h , 1 9 3 1 . On t h e1 5 t h h e s u f f e r e d f r o m nausea, and r e p e a t e d l y p ut h i s f i n g e r s

down h i s t h r o a t - t o m a k e h i m s e l f v o m i t . Next d a y h e had ad i s c h a r g e from t h e u r e t h r a and f r o m b o t h e y e s . On t h e 1 7 t hh i s gums were s w o l l e n , a n d t h e f o l l o w i n g day h e n o t i c e d ap u r u l e n t d i s c h a r g e f r o m t h e m o u t h . Th e u s u a l t e s t sr e v e a l e d t h a t t h e i n f e ct i o n was l i m i t e d t o t h e a n t e r i o ru r e t h r a ; and t h e e p i d i d y m e s , s p e r m a t i c c o r d s , s e m i n a lv e s i c l e s , a n d p r o s t a t e were u n a f f e c t e d . Th e eyes showed ab i l a t e r a l p u r u l e n t o p h t h a l m i a ; t h e corneae were u n a f f e c t e d .Th e b u c c a l m u c o s a , a n t e r i o r h a l f o f t h e tongue, t h e gums,a n d t h e t o n s i l s were c o a t e d w i t h a g r e y i s h a d h e r e n t membrane.In p l a c e s t h e membrane h a d s l o u g h e d o f f , l e a v i n g b r i g h tr e d a r e a s w i t h numerous b l e e d i n g p o i n t s . D i a g n o s i sOn March 2 1 s t p l a t e s o f Thomson's medium were c o n -v e y e d i n a p o r t a b l e i n c u b a t o r from S t a f f o r d t o D u d l e y ,

a d i s t a n c e o f 2 2 m i l e s . Th e lamp o f t h e i n s t r u m e n t wasl i t b e f o r e l e a v i n g S t a f f o r d , s o t h a t t he medium was warmon a r r i v a l a t D u d l e y . T h i s i s an i m p o r t a n t p r e c a u t i o nwhen d e a l i n g w i t h a d e l i c a t e m i c r o - o r g a n i s m l i k e t h egonococcus. Swabs were t a k e n f r o m t h e p a t i e n t ' s m o u t ha n d were sown on s i x p l a t e s , w h i c h were i m m e d i a t e l yp u t i n t o t h e i n c u b a t o r and s e n t b a c k t o S t a f f o r d by r o a d .These d e t a i l s a r e mentioned t o s how t h a t d i s t a n c e i sno i m p e d i m e n t with p r o p e r e q u i p m e n t and p r e c a u t i o n s .After f o r t y - e i g h t h o u r s ' i n c u b a t i o n , f o u r o f t h e p l a t e ss h o we d an ab undan t g r o w t h o f p n e u m o c o c c i and s t a p h y -l o c o c c i ; t h e r e m a i n i n g two r e v e a l e d , i n t h e midst o f as i m i l a r g r o w t h , some c o l o n i e s h a v i n g t h e u s u a l appearanceso f g o n o c o c c i . These were s u b c u l t u r e d u n t i l t h e y becamehabituated t o a r t i f i c i a l c o n d i t i o n s an d were t h e n s u b m i t t e dt o t he us ual biochemical t e s t s . They gave t h e r e a c t i o n so f g o n o c o c c i , p r o d u c i n g a c i d i n g l u c o s e , but n o t i nmalt o s e o r s a c c h a r o s e . These m i c r o - o r g a n i s m s i s o l a t e df r o m t he p a t i e n t ' s t h r o a t w i l l be f o r c o n v e n i e n c e d e s i g n a t e da s " t h e Dudley s t r a i n . " They were not a g g l u t i n a t e dby t he p a t i e n t ' s serum which was drawn o f f w i t h i n t h ef i r s t week o f t he d i s e a s e ; a n d , u s i n g t h e m as an a n t i g e n ,t h e c o m p l e m e n t - f i x a t i o n t e s t on t h e p a t i e n t ' s serum wasn e g a t i v e , a s i t was with t h e s t o c k l a b o r a t o r y a n t i g e n .C o n s i d e r i n g a l l t h e c i r c u m s t a n c e s , t h e s e r e s u l t s canno tb e r e g a r d e d a s e x c e p t i o n a l .A rabbit was inoculated with a Stoke s t r a i n o f g o n o c o c c i

obtained some t i me ago f r o m Dr . M c E l l i g o t t ' s c l i n i c . - Th em i c r o - o r g a n i s m s were f i r s t k i l l e d by f o r m a l i n .
