HOLIDAYS

Integrity Management Services recognizes the following holidays:

Employee Level Holiday Pay Holidays Applicable

Team Members & Shift Managers Time and a half paid for up to eight (8) hours with any additional time paid at regular overtime compensation.

New Year’s Day Thanksgiving Christmas

Assistant Managers Time and a half paid for up to eight (8) hours with any additional time paid at regular overtime compensation.

New Year’s Day Thanksgiving Christmas

Managers, General Managers & Senior General Managers

Receives the day off, with regular pay or an alternate day within 30 days if unable to take actual holiday off.

New Year’s Day 4th of July Thanksgiving Christmas

District Managers, Directors of Operations, Vice Presidents, IMS Office Staff, Maintenance Staff

Receives the day off, with regular pay or an alternate day within 30 days if unable to take actual holiday off.

New Year’s Day Memorial Day 4th of July Labor Day Thanksgiving Christmas

PAID TIME OFF

Manager, General Managers & Senior Managers receive Paid Time Off (PTO) accrued from their hire or promotion date. PTO is accrued at the rate of one week (five days) for the first two years of service and two weeks (ten days) each year after that. Two week accrual begins on the first day of your third year of service.

Employee may only have a maximum of 190 Hours (4 weeks) PTO accrued and then will stop accruing PTO until some is used. Planned PTO must be scheduled through the District Manager at least two weeks in advance.

Hourly Employees eligible for PTO will accrue hourly, to the equivalent of their average work week over the year. Salaried employees will accrue based on their scheduled work week hours.

District Managers and Above: Same as above, but with a three week accrual that begins on the first day of your third year of service. Accrual is also capped at 190 hours.

SICK LEAVE: (CA Only- Per state Law)

All employees will receive three days (24 hours) of sick leave upon hire (or on 7/1 of each year for those hired prior to 7/1/15) An employee is entitled to use sick days beginning 90 days after the start of their employment. The minimum number of hours to be paid out is 2 hours and sick leave use is limited to twenty four (24) hours per year. Sick days are meant for unexpected illness or appointments for employee or their dependent. Sick days/hours should be requested as far in advance as possible.

ANNIVERSARY BONUS:

To thank our Shift & Assistant Managers for their service each year, we offer an Anniversary Bonus on the last paycheck of their Anniversary Month. This bonus is discretionary and paid only to those employees in good standing and planning to continue employment for the foreseeable future. The Shift Manager Anniversary Bonus is $100/Part-Time and $200/Full-Time

Assistant Manager Bonus: Same as above, but $400 if full time

BOOKS SCHOLARSHIP PROGRAM

Our franchise company is committed encouraging further education of our employees. As part of this, we

offer a book scholarship program which is open to all supervisors and above level employees working at

store locations. Each store has two (2) bi-annual scholarships available and recipients earn $.50/hr (up to

$600/semester) for their books. See program application for details. This bonus is discretionary & will only

be paid out if employee is in good standing.

EMPLOYEE REFERRAL PROGRAM

All employees are eligible to refer friends or family to work for any of our three brands and receive a

bonus of $100-$1000! Please see referral bonus program sheet for details. This bonus is discretionary &

will only be paid out if employee is in good standing.

BIRTHDAY AND ANNIVERSARY RECOGNITION:

All employees receive a free meal for their birthday & company anniversary. Birthday/Anniversary lists are sent monthly from the office. It is the employee’s responsibility to take this meal during their Birthday/Anniversary month.

BONUS PROGRAM:

PROFIT SHARING PLAN:

Integrity Management Services provides a profit sharing plan for employees at the General Manager level and above. Contributions are 100% funded by the company and are based on a percentage of the overall company profit each year. Each individual’s portion is based on several factors including salary, as well as profitability and overall performance of your store and the company in the previous fiscal year. The amount contributed to each individual is at the sole discretion of Integrity Management Services. Additional information can be found in the Company Handbook or by contacting the office. The plan is administered by an outside company which strictly adheres to IRS guidelines for such plans.

401 K :

A 401 K Plan has been adopted to provide you with the opportunity to save for retirement on a tax advantage basis. This program is for Managers and above. When employment is terminated, employee can elect to take out their monies which would incur taxes on next tax return filing or roll their monies over to a qualified 401K to preserve tax free status.

Managers, General Managers, Senior General Managers, Area Managers, District Managers and Director of Operations are eligible for our quarterly bonus program. Please see Annual Bonus Program Sheet for details.

INSURANCE: Health, Dental, and Vision Insurance

Integrity Management Services offers insurance for all Employees who work Full-Time (30 hours or more per week, on average). Please see Insurance pamphlet for current details. California Options: Medical, Dental and Vision Utah Options: Medical, Dental, Life and Disability

Upon hire, there is a 60-day waiting period, before coverage begins. If you elect insurance, it will become effective on the 1st day of the month following your 60-day waiting period (i.e., if you start work on June 26th, your insurance will become effective on Sept. 1st).

If you are promoted to full time from part time, your waiting period is 60 days from date of promotion. You do not have to take all 3 insurance options – i.e., you can elect to take medical, but not dental or vision, or any combination of the choices offered.

Employees who do not sign up when hired, or those who wish to make changes must wait until the Open Enrollment period. Watch for communication in October / November regarding open enrollment, or contact the office for further details.

Supplemental Insurance

Supplemental insurance is available to all full-time employees. This insurance provides a number of plans ranging from disability, life, accident, hospital, dental and more. Premiums are taken out through payroll deduction and are 100% employee funded. If you terminate your employment with our company you have the chance to keep this insurance on a personal plan. Check with the office if you are interested in hearing more about supplemental insurance.
