Blast in Annpara D Power Station

A massive explosion at the generator-turbine of the500 Megawatt unit (Unit 7) of Annpara D power stationon November 13, 2019.
Unattended hydrogen leak from the generator wasidentified prima facie as a major trigger.
It was not attended by shutting the plant andthere was anticipation that it can be fixedwithout shutting the unit.
Some workers who were unaware of the leakwere trying to fix some dirt blocks and loosenuts and bearings in the generator.
Non-(spark-proof) spanners andequipment could have triggered sparks.

OSHA inspection # 2272953
Two employees were assigned the job of tending a 100 gallon (water jacket)reactor kettle of methyl methacrylate in the mixing room.
Employee #1 used a metal wrench (visegrips) to pry open the cover of akettle. The wrench handle struck the angle iron support for the agitatormotor, producing a spark.
Employee #2 noticed the spark, which was immediately followed by amassive “fire ball”. Both employees were engulfed in the fireball.
The investigation states that non-sparking tools were not provided forthe employees. All three employees received first, and second degreeburns on their face, arms and abdomen.
Employee #2 also received some third-degree burns.

Employee #1 worked for a company that cleans paints andreplaces valves of Propane gas cylinders.
Before removing an old valve, Employee #1 would open the valveto let the residual gas leak out on the loading dock. Where therewere large quantity of cylinders.
Employee #1 had an accumulation of a gas and air mixture aroundhis work area. The vapors were ignited either by a spark fromthe metal screwdriver he was using to open a damaged valve.
There was a fire and explosion. The employee sustained 2nd and3rd degree burns on the lower half of his body and washospitalized.

Employee #1 and a coworker, both maintenancemechanics, were working in a 30 inch by 36 inchmanhole at a gas station.
Employee #1 was trying to change a fuel pump,while the coworker watched from outside themanhole.
Employee #1 was using an Allen wrench toloosen the bolts on the fuel pump lead when hecreated a spark that ignited the gas fumes in themanhole, causing an explosion.
Employee #1 suffered burns to his face, hands,arms and legs in the explosion and washospitalized.

Major Fire at Tank Wagon Siding

A major fire broke out in one of the tank wagon decantation railwaysiding of Oil Company during the first fortnight of January 2015.
While unloading was in progress, contractor workmen noticed that MSwas leaking from the flange joint connecting tank wagon unloadingheader and unloading hose of one of the MS tank wagon. The spilledMS was being collected in a bucket.
Without closing the bottom valve and master valve of the MS tankwagon, contract worker tried to tighten the bolt of the flange joint with aspanner to arrest the leakage.
During the process of tightening, the spanner slipped & fell downresulting in generation of spark. The MS vapors immediately caughtfire due to the spark and subsequently the MS tank wagon wasengulfed by the fire.

Fire incident at a CCR Unit

A major fire took place in CCR (Continuous Catalytic Reformer)unit at level transmitter (LT) of the 2nd stage suction Knock-OutDrum (KOD) of Net Gas Compressor.
Cause of Fire:Passing of valve(s) from KOD-03, which was at a pressure of15 Kg/cm2 (g) at the level troll, resulted in accumulation of H2into the level troll. In fact KOD-03 was isolated by closing asingle valve without any positive isolation.
Opening of the flange on the drain line resulted in release of H2and Naphtha from the level troll which caught fire either byauto-ignition or due to generation of spark whilehammering the line by not using (non-sparking tools) to de-choke the line.

In all the cases highlighted there was loss of assets and human lives or grievous injuries.
Post accident audit of all cases identified one common reason - Non-Sparking Hand Tools were not used.
