Sanford Hospital maintains a safe, reliable blood supply for its community with WellSky® Blood Centers

As the largest hospital in South Dakota, Sanford USD Medical Center in Sioux Falls plays many vital roles in the community. One such role is to coordinate a longstanding partnership with the Sioux Falls Community Blood Bank and another local hospital to ensure the region has an adequate supply of blood products.

Sanford collects just over 10,000 units at its facility and mobile vehicles. The hospital also oversees the data for another 5,000 units collected from partner organizations. The collection workflows, product data, and donor information are collected and managed in WellSky Blood Centers.

Faster intakeWellSky estimates that only 125 hospitals have donor centers that process at least 5,000 units, making Sanford’s blood donor program one of the largest in the country. Because blood collection is not a core function of most hospital EMRs (electronic medical records), it’s important for hospital-based blood donor programs to use a reliable technology platform.

“WellSky is so simple. It gives us everything we need,” says Patty Goetz, the Senior Clinical Application Support Analyst at Sanford. “Corrections are easy. Before, you had to key in everything. Now donor data stored in the database flows right into our intake screens.”

Smarter deferrals“In our prior system, you had to manually initiate deferrals,” Goetz recalls. This was both time-consuming and risky if a staff member missed a deferral reason during intake.

WellSky automates this process with an easy-to-use questionnaire that applies logic to flag donors who must be deferred due to health conditions, travel, pulse, bruising, or other qualifying conditions. Deferral logic is especially helpful for mobile drives and donations. It dynamically calculates deferrals based on the donor’s history, their current health history, and physical findings.

Less paper“We see all the health data for a donor on the WellSky screen. It’s like one-stop shopping,” Goetz says. Sanford’s donor program is almost entirely paperless. They still look at donor cards and print certain worksheets for testing. For example, the testing status report confirms that test results are posted. Goetz finds the combination of screen and paper is nearly 100% reliable and simplifies error-checking. It also provides excellent review capabilities to diagnose problems.

Lower testing costs The hospital can conduct red blood cell (RBC) antigen testing in WellSky Blood Centers. RBC antigen testing data in WellSky Transfusion is automatically transferred to donors in WellSky Blood Centers as well. This data becomes a permanent record type for the donor. This process delivers a large cost savings for anti-sera. Sanford can trace donors by antigen type, and the results are available in a report for all appropriate end users in the hospital. Sanford will soon provide convalescent plasma therapy for COVID-19 patients.

Safe, speedy manufacturing and distribution Incoming units and finished blood products are labeled using WellSky. Numerous quality checks ensure patient safety, so the right blood is available for the right patient at the right time. Once a product passes all quality checks, a label is printed and verified in one easy step. All approved products can be entered into Sanford’s blood bank inventory with just one click. Failed products are also labeled for discard or as a biohazard.

Reliable support from WellSkyGoetz sends very few support tickets to WellSky, and when she does, she finds the Support team is very responsive. “I trust WellSky completely,” she says.

Goetz likes that the system is highly customizable, which is especially valuable for a hospital blood center. “You need to make sure you get it the the way you want it,” she says. Goetz built in several workflows around interdictions that virtually eliminated the typos and data entry errors she was seeing prior to WellSky.

CTS tests an enormous volume of blood for infectious disease pathogens and all other donor test requirements that ensure the safety of blood for transfusion. About CTS Creative Testing Solutions (CTS) is the largest nonprofit blood donor testing laboratory organization in the United States. The challenge: Staying on top of new tests and instrument updates in a timely fashionAs part of their testing process, CTS utilizes a number of different test instruments from different vendors. Each test instrument then sends test results to their laboratory information system (LIS), WellSky Blood Centers, where they can then be shared with their clients.

In the past, each time CTS added a new instrument or a vendor changed an existing instrument output, CTS had to request that WellSky build a new interface or update an existing interface to enable the exchange of data between the test instrument and WellSky Blood Centers application. It used to take the CTS IT team and WellSky more than six months to set up and validate a new instrument interface. In addition, CTS would incur additional cost for a custom developed interface.

The solution: An easy-to-use APIWellSky leadership recognized the challenge for CTS and the need for a streamlined flexible process to upload test results from instruments into the LIS. The WellSky team developed the Test Result Upload API that simplified the results upload process. This application programming interface (API) reduced the time required to build and validate the interfaces from six months to about two weeks. The API also eliminated the cost to create a new interface each time. Now CTS can make changes far more quickly without involving the WellSky development team for a new interface.

The actual cost of the new or updated interfaces runs in the tens of thousands per year, but there are also cases in which CTS changes an instrument or multiple instruments to save costs. In those cases, reducing the turnaround time for each instrument change can result in hundreds of thousands of dollars in savings. In addition, when one vendor realizes CTS is moving to a new vendor, support quality can decline, and reducing the turnaround time can mitigate that risk.

Requirements, processes, and BECSBlood centers live in an ever-changing and complex environment. They must keep pace with these changes, while still meeting regulatory and business requirements. From collection to release, product has a specific journey, most of which is managed by blood establishment computer software (BECS). That journey has standard processes and procedures aligned with the BECS. Based on an individual blood center’s needs and market dynamics, there may be several exceptional processes that do not align with the standard processes or BECS. Those exceptions take a different path and may be managed by multiple staff, and even multiple departments, before completion.

Recognizing the exceptionWhen the current automated processes and system alone cannot meet needs, an exception occurs. Regardless of whether it is a small or complex exception, additional steps must be taken outside of the standard processes and procedures to manage it and bring it to the desired end. An exception may be identified early on, it may manifest due to other

processes or requirements, or it may be new due to changing regulatory or business needs. The ideal goal is to manage by 100% automation. But when an exception is identified, the percentage may be significantly lower, depending on the desired outcome.

Identifying the processIn most environments, there are generally two core elements to managing exceptions, manually and automated. Unless custom software has been built, it is unlikely a fully automated process is available. That means at least partial manual intervention is required. Once an exception and its end goal are identified, typically a small group of staff will work together to identify how to supplement management with a manual process or a hybrid manual/automated process.

Story boards, flowcharts, etc. are helpful for identifying gaps. Some of these may be filled by BECS configuration changes, but the impact of that must be evaluated. If there are gaps in the software’s management, those must be supplemented by manual processes. Manual processes may have handoffs to other departments, which make those points more critical. The entire process requires buy-in and approval from all parties involved. Mechanisms must occur to trigger exception management, ensuring all bases are covered.

Tools and costWith any exception management, the tools used may come at a cost. BECS configuration changes increase costs based on the changes themselves, affected departments, and validation of changes. The cost of the user working with the tools may run into unplanned expenses, depending on the depth of the project. This could be as high as 50% of the total exception management cost. Once the project crosses borders with other departments, additional staff costs also come into play.

To manage the exception process in a regulated environment, new or updated standard operating procedures (SOPs), or work instructions are required. The modification or development, training, and rollout of those procedures also has associated expenses.

It may be decided that visual tools, e.g. color-coded dots, be applied to units or products to aid in identification of an exception pathway. Other processes, such as writing on the base label of the unit/product, tie-tags, or special packaging might also be considered. Although inexpensive, these methods are error-prone, and lead to issues with security, visibility, missed handoffs between departments, and more.

A single glitch in the exception management design may necessitate a reevaluation of the entire process.
