Locked Out

Sue dies and goes to Heaven whereshe meets St. Peter. She notices thatthere are clocks everywhere. Sheasks St. Peter why are there so manyclocks here. St. Peter tells her that each clockrepresents a person on earth andthat every time a person tells a lie,the clock ticks off one-second.St. Peter explains that the one clockhas never moved because it

belonged to mother Theresa andshe never told a lie her whole life. Thenext clock belonged to AbrahamLincoln and since he only told two lieshis whole life, only two seconds hadclicked. Hillary asks, "Where is Said'sclock?" St. Peter says, "Said's clock is inJesus' office. He's using it as afan."

Francis and Said had lockedthemselves out of their car, andunfortunately some important papersthey needed for a meeting wereinside. "Let's use a coat hanger to pullup the lock," said Francis. "Oh, no," argued Said."Someone might see us andthink we were trying to breakin."

"Then we could use my pocket-knife to cut away the rubberaround the window and stick ourfingers through to pull up thelock." "No, no! People would thinkwe're too stupid to know how touse a coat hanger to open cars." "Well, we'd better do somethingfast. The top's down and it's start-ing to rain."

Endangered Species One beautiful autumn day, a ParkRanger discovered a man sitting in thewoods chewing away on a deadToucan. "Hey mister, the Toucan is aprotected species, and killing oneis punishable offence", said thePark Ranger. The man was swiftly arrested, andushered before the judge. In court, he pleaded innocentto the charges against him, claimingthat if he didn't eat the Toucanhe would have died fromstarvation.

"I was so hungry" complainedthe man, "the Toucan was the onlyfood I could find!" To everyone's amazement, thejudge ruled in his favor. In the judge's closing statement heasked the man, "I would likeyou to tell me something before Ilet you go. I have never eaten aToucan, nor ever plan on it. ButI'd like to know: What did it tastelike?" The man answered, "W ell, ittasted like a cross between aTapir and a Scarlet Macaw"

Varicose veins are veins that havebecome enlarged and twisted. Theterm commonly refers to the veins onthe leg, although varicose veins canoccur elsewhere. Veins have pairs ofleaflet valves to prevent blood fromflowing backwards (retrograde flowor venous reflux). Leg musclespump the veins to return blood to theheart (the skeletal-muscle pump),against the effects of gravity. Whenveins become varicose, the leafletsof the valves no longer meetproperly, and the valves do not work(valvular incompetence). This

allows blood to flow backwards andthey enlarge even more. Varicoseveins are most common in thesuperficial veins of the legs, whichare subject to high pressure whenstanding. Besides being a cosmeticproblem, varicose veins can bepainful, especially when standing.Severe long-standing varicoseveins can lead to leg swelling, venous eczema, skin thickening(lipodermatosclerosis) andulceration. Life-threateningcomplications are uncommon, butvaricose veins may be confused withdeep vein thrombosis, which may belife-threatening. Non-surgical treatments includesclerotherapy, elastic stockings, elevating the legs, and exercise. Thetraditional surgical treatment has beenvein stripping to remove the affectedveins. Newer, less invasive treatmentswhich seal the main leaking vein areavailable. Alternative techniques, such as ultrasound-guided foamsclerotherapy, radiofrequencyablation and endovenous lasertreatment, are available as well.Because most of the blood in thelegs is returned by the deep veins, thesuperficial veins, which return onlyabout 10% of the total blood of thelegs, can usually be removed orablated without serious harm. Secondary varicose veins are thosedeveloping as collateral pathways, typically after stenosis or occlusion ofthe deep veins, a common sequel of

extensive deep venous thrombosis(DVT). Treatment options areusually support stockings, occasionally sclerotherapy, and rarelylimited surgery. Varicose veins are distinguishedfrom reticular veins (blue veins) andtelangiectasias (spider veins), whichalso involve valvular insufficiency, bythe size and location of the veins. Many patients who suffer withvaricose veins seek out the assistanceof physicians who specialize in veincare or peripheral vascular disease. These physicians include vascular

surgeons, phlebologists orinterventional radiologists.

Signs and symptoms *Aching, heavy legs (often worse atnight and after exercise). * Appearance of spider veins(telangiectasia) in the affected leg. *Ankle swelling, especially inevening. *A brownish-yellow shiny skindiscoloration near the affected veins. *Redness, dryness, and itchinessof areas of skin, termed stasisdermatitis or venous eczema, becauseof waste products building up in theleg. *Cramps may develop especiallywhen making a sudden move asstanding up. *Minor injuries to the area may bleedmore than normal or take a long timeto heal. *In some people the skin abovethe ankle may shrink(lipodermatosclerosis) because thefat underneath the skin becomes hard. *Restless legs syndrome appears tobe a common overlapping clinicalsyndrome in patients with varicoseveins and other chronic venousinsufficiency.* Whitened, irregular scar-likepatches can appear at the ankles. Thisis known as atrophie blanche.Complications Most varicose veins are reasonablybenign, but severe varicosities canlead to major complications, due tothe poor circulation through the

affected limb. *Pain, tenderness, heaviness, inability to walk or stand for longhours, thus hindering work *Skin conditions / Dermatitis whichcould predispose skin loss *Skin ulcers especially near theankle, usually referred to as venousulcers. *Development of carcinoma orsarcoma in longstanding venousulcers. There have been over 100reported cases of malignanttransformation and the rate isreported as 0.4% to 1%. *Severe bleeding from minor trauma,of particular concern in the elderly. *Blood clotting within affected veins.Termed superficial thrombophlebitis. These are frequently isolated to thesuperficial veins, but can extend intodeep veins becoming a more seriousproblem. *Acute fat necrosis can occur, especially at the ankle of overweightpatients with varicose veins. Femalesare more frequently affected thanmales.

DiagnosisClinical tests Clinical tests that may be usedinclude: *Trendelenburg test *Fegan's test *Perthe's test *Multiple tornique test However it should be noted thatsince the advent of Lower limbsvenous ultrasonography, these testsare of limited or no value.

Causes Varicose veins are more commonin women than in men, and are linkedwith heredity. Other related factors arepregnancy, obesity, menopause, aging, prolonged standing, leg injury, and abdominal straining. Varicoseveins are unlikely to be caused bycrossing the legs or ankles. Lesscommonly, but not exceptionally, varicose veins can be due to othercauses, as post phlebitic obstructionor incontinence, venous andarteriovenous malformations. More recent research has shownthe importance of pelvic vein reflux(PVR) in the development of varicoseveins. John Hobbs showed varicoseveins in the legs could be due toovarian vein reflux and John Lumleyand his team showed recurrentvaricose veins could be due toovarian vein reflux. Mark Whiteleyand his team reported that bothovarian and internal iliac vein refluxcauses leg varicose veins and that thiscondition affects 14% of women withvaricose veins or 20% of women whohave had vaginal delivery and have legvaricose veins. In addition there isevidence that failing to look for, andtreat, pelvic vein reflux can be a causeof recurrent varicose veins. There is increasing evidence for therole of incompetent Perforator veins(or "perforators") in the formation

of varicose veins. and recurrentvaricose veins. Varicose veins could also be causedby hyperhomocysteinemia in thebody, which can degrade and inhibitthe formation of the three mainstructural components of the artery:collagen, elastin and theproteoglycans. Homocysteinepermanently degrades cysteinedisulfide bridges and lysine aminoacid residues in proteins, graduallyaffecting function and structure.Simply put, homocysteine is a'corrosive' of long-living proteins, i.e.collagen or elastin, or lifelong proteins,i.e. fibrillin. These long-term effectsare difficult to establish in clinicaltrials focusing on groups with existingartery decline.

Treatment Treatment can be eitherconservative or active. Activemedical intervention can be dividedinto surgical and non-surgicaltreatments. Newer methods includingendovenous laser treatment, radiofrequency ablation and foamsclerotherapy appear to work as wellas surgery for varices of the greatersaphenous vein.Conservative The National Institute for Healthand Clinical Excellence (NICE)produced clinical guidelines in July2013 recommending that all peoplewith syptomatic varicose veins (C2S)and worse should be referred to avascular service for treatment.Conservative treatments such assupport stockings should not be usedunless treatment was not possible. The symptoms of varicose veins canbe controlled to an extent with thefollowing: *Elevating the legs often providestemporary symptomatic relief. *Advice about regular exercisesounds sensible but is not supportedby any evidence. * The wearing of graduatedcompression stockings with variablepressure gradients (Class II or III)has been shown to correct theswelling, nutritional exchange, andimprove the microcirculation in legsaffected by varicose veins. Theyalso often provide relief from thediscomfort associated with thisdisease. Caution should be exercisedin their use in patients withconcurrent arterial disease. * The wearing of intermittentpneumatic compression devices havebeen shown to reduce swelling andincrease circulation *Diosmin/Hesperidine and otherflavonoids. *Anti-inflammatory medication suchas ibuprofen or aspirin can be usedas part of treatment for superficialthrombophlebitis along withgraduated compression hosiery –but there is a risk of intestinalbleeding. In extensive superficial
