The case began way back in February 2009 when Godwin Sutherland’s vacation house in the Mountain Pine Ridge area was burglarized and destroyed by fire. Police received information at around 9:00 pm on February 23, 2009 of a grey van heading from San Antonio Village to San Ignacio town loaded with suspected stolen goods. A team of policemen headed towards San Antonio Village. The police came upon the van on the road between San Antonio Village and Cristo Rey Village. The driver complied with the order to stop the van, exit the vehicle and to open the back door. The van was found packed with plastic chairs, a brown sofa, a folding table, floor tiles and other construction materials. The driver, identified as Rene Rivera, 26 at the time, told the police that he is a Belizean mechanic residing in Santa Familia Village, Cayo. Rivera was accompanied by his father in law Albert Woodye. Under caution Rivera told the police that he had gone with Woodye to get the items from a house in the Mountain Pine Ridge. He said that after loading the van and they were departing, Woodye told him to stop the vehicle. He said that Woodye exited the vehicle, walked back to the building after which he saw a huge flame coming from the building as Woodye returned to the van for the journey to Santa Familia. If the possibility of a contemptcharge was non-existent in termsof matters before a court, wewould have long commented onthe only possible outcome of thecriminal proceeding againstElvin Penner. What we find interesting in thismatter is at least two “firsts” thatit presents in the history of thenation. It is the first time that anyPrime Minister of Belize hasever publicly announced thebanning of an area representativefrom ever serving in the party heleads. It is also the very first time thatany serving Area Representativeis before a court of law on amatter relevant to the ministerialportfolio that individual held. If the government would havehad a larger majority of arearepresentatives, there would alsohave been the possibility that thiswould have been the first time anarea representative would havebeen successfully recalledtriggering a bye-election in theconstituency to which thatindividual was elected. A larger majority of arearepresentatives for any onepolitical party can either havepositive or negative implicationsdependent on the integrity ofthe leader of that party. On thepositive side a large majority ina team lead by an individual withthe moral fortitude to take actionagainst any and all forms ofcorruption will afford that leaderand his party the leverage torecall the individual who hasgone astray.

On the down side, a largemajority with a leader who,himself is engaged in officialcorruption, will only mean thepresence of a larger group ofindividuals raping the nation atthe expense of the masses. Sucha situation can easily result in theamassing of a large national debtwith nothing to show for it. It canresult in the corrupt giving awayof a nation’s key facilities tofriends and cronies for very littleor nothing at all. It can lead to theawarding of bloated contracts torelatives and friends resulting insubstandard work like forexample a bridge that washesaway even before a single personor vehicle passes over it. It canmean the washing away of amulti-million dollar bridge onthe Southern Highway with thefirst flood. It can mean a leakingcenter for the performing artswhich is less than ten years old.It can mean the giving away of12,000 acres of land (over40,000 house lots) to a singleindividual who has no intentionof using the land and insteadwalks into a bank gets a huge sumof money as mortgage for theland and allows the bank to takeownership of the land. A large majority in the Houseof Representatives with a corruptleader can result in the signingof secret agreements toaccommodate a multi-billionairefriend. It can result in thissame special individual notpaying taxes to the governmentor dividends to Belizeanshareholders of the lucrative

enterprise he owned. A large majority in the Houseof Representatives with a corruptleader can result in extremearrogance where one of them canpublicly tell the people that itdoes not matter how manymistakes he and his governmentmakes and that the more mistakesthey make, the harder theBelizean people will work to getthem out of trouble. A large majority with a corruptleader can result in the hurling ofinsults to the employable amongus as they do not create jobsbecause they publicly accuse usof being lazy thieves. They cancome forward and proudlyannounce that they don’t need usand that they can go to any of theiroffices in Montevideo, Canadaand Cayman, where they canmake a lot of money. A large majority with a corruptleadership can result in thecollection of an environmentaltax and after a couple yearsincreasing that tax by 100%without using the funds derivedtherefrom for its intendedpurpose while the people livingin the nation’s commercial

capital as well as those living inthe largest geographical districtin the land are subjected to thetoxic smoke emanating fromgarbage dump sites perpetuallyburning. The negative effects of a largermajority with a corrupt leader arereal and numerous, the nationmust therefore never ever againfall into that trap. The standard of living and themeaningful development of anation can be realized in anenvironment where officialcorruption is at its lowest level.The best combination for this tohappen is by way of the electionof a leader of a political partywith the moral fortitude of takeaction against all forms ofcorruption among the team heleads given a comfortablemajority in the house ofrepresentatives. It was a wise man who oncesaid that a nation who studies notits history, learns not frommistakes of the past andhighlights not, to the youngergeneration, the errors of the past,is doomed to a lifetime ofsuffering.

A gallstone (also calledcholelithiasis) is a crystallineconcretion formed within thegallbladder by accretion of bilecomponents. These calculi areformed in the gallbladder but maydistally pass into other parts ofthe biliary tract such as thecystic duct, common bile duct,pancreatic duct or the ampulla ofVater. Rarely, in cases of severeinflammation, gallstones mayerode through the gallbladderinto adherent bowel potentiallycausing an obstruction termedgallstone ileus. Presence of gallstones in thegallbladder may lead to acutecholecystitis, an inflammatorycondition characterized byretention of bile in thegallbladder and often secondaryinfection by intestinalmicroorganisms, predominantlyEscherichia coli, Klebsiella,Enterobacter and Bacteroides

Gallstonespecies. Presence of gallstonesin other parts of the biliary tractcan cause obstruction of the bileducts, which can lead to seriousconditions such as ascendingcholangitis or pancreatitis. Eitherof these two conditions can belife-threatening and are thereforeconsidered to be medicalemergencies.

Definitions Presence of stones in thegallbladder is referred to ascholelithiasis (from the Greekchol- (bile) + lith- (stone) +iasis- (process). If gallstonesmigrate into the ducts of thebiliary tract, the condition isreferred to as choledocholithi-asis, from the Greek chol- (bile)+ docho- (duct) + lith- (stone)+ iasis- (process).Choledocholithiasis isfrequently associated withobstruction of the biliary tree,which in turn can lead to acute

ascending cholangitis, from theGreek: chol- (bile) + ang-(vessel) + itis- (inflammation) ,a serious infection of the bileducts. Gallstones within theampulla of Vater can obstruct theexocrine system of the pancreas,which in turn can result inpancreatitis.

Characteristics andComposition

Gallbladder opened to shownumerous gallstones. The large,yellow calculus probablycomprises cholesterol, while thegreen-to-brown stones suggestbile pigments, such as biliverdinand stercobilin. Gallstones can vary in size andshape from as small as a grain ofsand to as large as a golf ball. Thegallbladder may contain a singlelarge stone or many smaller ones.Pseudoliths, sometimes referredto as sludge, are thick secretionsthat may be present withinthe gallbladder, either aloneor in conjunction with fullyformed gallstones. The clinicalpresentation is similar to that of

cholelithiasis. The compositionof gallstones is affected by age,diet and ethnicity. On the basis oftheir composition, gallstones canbe divided into the followingtypes:

Cholesterol Stones Cholesterol stones vary fromlight yellow to dark green orbrown and are oval, between 2 and3 cm long, each often havinga tiny, dark, central spot. To beclassified as such, they must beat least 80% cholesterol byweight (or 70%, according tothe Japanese- classificationsystem).

Pigment Stones Pigment stones are small anddark and comprise bilirubin andcalcium salts that are found inbile. They contain less than20% of cholesterol (or 30%,according to the Japanese-classification system).

Mixed Stones Mixed gallstones typicallycontain 20–80% cholesterol (or30–70%, according to theJapanese- classificationsystem). Other commonconstituents are calciumcarbonate, palmitate phosphate,bilirubin and other bile pigments.Because of their calciumcontent, they are oftenradiographically visible.

Tr eatmentMedical

Cholesterol gallstones cansometimes be dissolved by oralursodeoxycholic acid, but itmay be necessary for thepatient to take this medicationfor up to two years. Gallstonesmay recur, however, once thedrug is stopped. Obstructionof the common bile ductwith gallstones can sometimesbe relieved by endoscopicretrograde sphincterotomy(ERS) followingendoscopic retrogradecholangiopancreatography(ERCP). Gallstones can bebroken up using a procedurecalled extracorporeal shock wavelithotripsy (often simply called"lithotripsy") , which is amethod of concentratingultrasonic shock waves onto thestones to break them into tinypieces. They are then passedsafely in the feces. However, thisform of treatment is suitableonly when there is a smallnumber of gallstones. Treatment involves anoperation called a choledoch-olithotomy, which is the removal

The country’s half-century long conflict has takenthe lives of almost a quartermillion women, men, andchildren, and displaced nearly sixmillion more. The United Stateshas financed much of the conflictin recent years, investing $9billion since 2000—much of itto bolster Colombia’s securityforces. Yet peace may be near. On May16, the Colombian governmentand the Revolutionary ArmedForces of Colombia (FARC), thecountry’s largest guerrilla group,signed a preliminary accord onthe third of five negotiatingpoints in their ongoing peacetalks in Havana, Cuba: illicitdrugs. The agreement offers aviable plan for the FARC to endits involvement in the Colombiandrug trade, alternatives for thesmall-scale cultivators of cropsdestined for illicit drug markets,and meaningful policy reforms atthe national level for addressingissues of drug consumption andpublic health. Hope too lies with anannouncement that came earlierthe same day. Following nationaland international pressure—including an inter-parliamentaryletter signed by 245 representa-tives from the United States,United Kingdom, and Ireland—the FARC announced a unilateralceasefire. While the governmentmaintains that it will not endmilitary operations until anagreement is signed, and thoughthe FARC’s temporary ceasefireended on May 28, this act isencouraging because it bothsignificantly decreased violenceand will likely increaseconfidence at the negotiatingtable. In more than a decade ofits use in Colombia, aerialspraying has served only todisperse coca crops, destroy poorfarmers’ livelihoods, andengender local distrust forgovernment authorities, as theonly contact many communitieshave had with the state has beenthe occasional visit of a planeconducting aerial spraying. The agreement also addressesdrug consumption, an issuegenerally thought to be outsidethe purview of the peace talks.While details here are scant,linking this issue to the peacetalks will help continue regionaldebates on drug policy reform.Recognizing that drug policyshould be based on respect forhuman rights and public health isa valuable contribution. But a full agreement, ifeventually signed, will not be apanacea. Taking the FARC out ofthe cultivation and traffickingbusiness will not independentlysolve the drug issue or theassociated violence. As long asthere is worldwide demand—andparticularly U.S. demand—fordrugs, criminal organizations willfind a way to supply them.Furthermore, an accord willlikely leave a power vacuum in

rural regions of the country as theFARC demobilizes and cedesthose territories. There is agood chance that right-wingparamilitary successor groupsand criminal gangs will try to fillit. Establishing a positive statepresence and providing basicservices will be a majorchallenge, especially in regionswhere the armed forces havebeen the primary face of thestate. Because of these continuedchallenges, the United States hasan important role to play in theimplementation phase, both insupporting Colombia financiallyand in granting the Colombiangovernment political space toimplement the accords—evenwhen they contradict U.S. policypriorities. A State Departmentcommunique on the drug policyagreement, which highlightsthe continuation of forcederadication, raises questionsabout whether the UnitedStates will help or hinder theadvancement of the peaceprocess. Nearly two of every three aiddollars destined for Colombiagoes to the public securityforces. Will the U.S. governmentbe willing to shift aid to buildpeace rather than continue war?

Achieving durable reductions inpoppy and coca crop cultivationfor illicit drug production willrequireimplementing alternativelivelihoods and connectinglong-forgotten rural areas withthe national infrastructure. Afterdecades of waging a largelyineffective “war on drugs” inColombia, will the United Statesallow its longtime ally to breakwith the prohibition-focusedmodel and explore alternatives tothe current militarized approach?Some of the most revolutionaryagreements in the accord, such asall but ending aerial spraying,would challenge the existing U.S.approach. These questions, and the manymore that will be raised asthe talks progress, will likelydismay hardliners in the U.S.government who are not readyto shift drug control tactics. Butwith little progress to show afterdecades of violence and billionsof dollars spent, the Colombianand FARC negotiators have madean important step toward endingdecades of violence. The UnitedStates should stand ready tosupport Colombia, bothfinancially and politically, inthe coming months and years—and it should know when tostand down.
