I  have  read  and  understand  the  Potter  Fitness  Center  reservation  pol icy.  

The  reservation  may  be  changed  or  canceled  at  the  discretion  of  the  Potter  Fitness  Center  Director  at  any  time,  as  the  mission  requires.  A  staff  member  wil l  notify  the  requester  as  soon  as  possible,  i f  such  an  instance  should  occur.

The  using  unit/organization  is  responsible  for  setup,  tear  down,  and  cleaning  of  the  area  to  include  trash  removal  during  use  of  outdoor  faci l ities.  Non  compliance  wil l  result  in  disapproval  in  further  requests.

If  the  unit/origination  is  reserving  the  court  or  field  does  not  show  up  15  minutes  after  reserved  time,  the  court  or  field  is  returned  to  open  play  status.

Individuals  signing  this  form  are  held  responsible  for  any  damage  incurred  to  property,  faci l i ty  or  equipment  while  in  their  possession.

If  there  is  any  concerns  regarding  you  reservation,  report  i t  to  a  staff  member  immediately.

All  outdoor  reservations  wi l l  be  canceled  during  typhoon  conditions  TC-­‐2  or  higher,  indoor  reservation  may  continue  unti l  TC-­‐1.      

Units  wi l l  lose  their  reservations  i f  they  have  consistently  missed  their  scheduled  ongoing  reservation(s)  without  prior  confirmation  with  the  reservation  manager.
