Criteria For 2010/2011 Plan Year
Complete at least one of the following Wellness Programs (enroll before Jan 4, 2010):
1. EMPOWERED Coaching (13 Focus Areas) Completion criteria: have at least three contacts with your health coach before March 31, 2010.
2. Walk this WayCompletion criteria: pursue 360,000 steps by March 31, 2010.
3. Condition Management Coaching (7 Condition Areas) Completion criteria: complete at least three coaching calls before March 31, 2010.
During Next Open Enrollment: Health Assessment and Biometric Screening for both enrolled Employee and Spouse

4. Not having a physical exam in last 5 years: 16.2%

Notes:• All data was self-reported• Cholesterol and blood pressure data was inconclusive
- Biometric screenings next year will provide accurate picture of clinical risks

When running a report, you can utilize both Data Selection and Processing Options to control the data displayed on the report.

Review Data Selection by checking the Data Selection box and clicking ‘Submit’ from the Form Exit.

Data Selection will always reflect the original Data Selection assigned when the report version was created. If Data Selection is changed to run a report, the changes will not remain in Data Selection.

Clicking on ‘OK’ in Data Selection will take you to Processing Options. Processing Options do reflect criteria used the last time the report was run. Any changes made to Processing Options will remain until changed again.
