Field Accomplishments for the Month of August 2014

Narrative Report on Field (August 1-20, 2014)

Based on the August 2014 monthly work plan for field activities, the team has come up with the following accomplishments: 1. Continue for the Social Investigation

(SI) to area

Conduct key informants interview

The 10 Community Development

Facilitators (CEF) conducted the key

informants’ interview (KII) as part of the social

investigation (SI) to their assigned barangays.

The CEFs able to generate their August 2014

SI report with updated barangay profiles,

additional information and identified issues,

concern, action taken and possible

recommendation. The team utilized also for

the KII during the Kinhason festival celebration where close coordination was made with the

BLGU officials who participated during the activity starting August 8-12, 2014.

Conduct emersion/ ocular visits

As part of the final SI to the area, the

CEF and the Municipal Finance Analyst

(MFA)/ Community Finance Facilitators

conducted emersion/ ocular visits to both

the island and main barangays of the

municipality from August 5-7 and August

13-15, 2014. The team’s community

emersion was regularly monitored by the

AC and the DAC through the Field Activity

Notice Information Request (FANIR) and

Field Activity Report of Accomplishment

(FARA) forms.

Process/consolidate new

gathered data

Output of the team’s during the KII, ocular visits/emersion in the community were

process for the finalization of the barangay profile and the SI report for August 2014. Although

lack of computer equipment for the processing and consolidating of the new gathered

information, the CEFs able to gradually process the data from August 8-12, 2014 while the

MLGU and BLGU are busy on the Adlaw nan Dapa and Kinhason Festival and on August 15-

18, 2014. The CEFs have submitted their SI report on August 18, 2014 as the output of their

community emersion and social investigation.

2. Technical Investigation (TI)

and Financial Investigation

(FI)

Both the Deputy Area

Coordinator (DAC)/ Technical

Facilitators and the Municipal

Finance Analyst (MFA)/ Community

Finance Facilitator (CFF) conducted

the technical and financial

investigation to the municipality. The

DAC/TF Engr. Guil Lou dela Peña

and Engr. Angelie Morgado

submitted their Technical Investigation Report to the ACT and RPMO on August 15, 2014

while the MFA/CFF Keevin Jann Vercide and Janice Maghanoy submitted their Financial

Investigation Report on August 18, 2014 to the ACT.

3. Training Needs Analysis (TNA)

Fifteen (15) team members accomplished the TNA forms on August 6, 2014 and submitted

it via e-mail to [email protected] on the same day.

4. Tracking of the municipal consolidated status of local counterpart contribution

(LCC)

The MFA/CFF Janice Maghanoy and Keevin Jann Vercide closely coordinated with the

municipal finance committee for the submission of the local counterpart contribution (LCC).

They conducted consultation on August 18, 2014 with MSWDO, Municipal Accounting Office

(MAO), and Municipal Treasurer’s Office (MTO) with regard to Annex 7 & 8, VLCC and the

Certification for the Delivered LCC.

5. Other Area meetings/ activities

Attend to Flag Ceremonies

The team attended the flag ceremonies every Monday on August 4, 11 and 18

respectively. The team was updated with the municipal activities which were announced

during the flag ceremonies.

Participate to Clean-up Drive activity

As part of the environmental management initiative of the LGU, the team closely

coordinated with the MSWDO Nancybelle Trazares and MDRRMO for the attendance and

participation the clean-up drive to the coastal area on August 5, 2014. The team was guided

by Ms. Melissa Bou from the office of the Mayor.

for the Adlaw nan Dapa and opening for

3rd Kinhason Festival

Meantime, the team took part on the m

municipal opening for the Adlaw nan Dapa on

August 8, 2013 by participating on the parade in

response to the MLGU invitation. The MLGU also

provided the team with 15 pieces of printed t-shirts

souvenir during the said occasion.

Clean-up drive. The team actively participated the clean-up drive to the coastal area of barangay Poblacion 6. The activity spearheaded by the MLGU in times with the celebration of Disaster Awareness month, Adlaw nan Dapa and 3rd Feast.

Attend the State of the Municipality Address (SOMA)

On August 11, 2014, the team appeared during the State of the Municipality Address

(SOMA) of Mayor Yulie Y. Ruaya held at about 6:00 in the evening at the municipal open

stage. The mayor highlighted it support to the KC-NCDDP in her SOMA.

Join the 3rd Kinhason Festival

The team participated the celebration of the 3rd Kinhason Festival on August 12, 2014.

The festival was also the avenue for the team to meet with other stakeholders in the

municipality. The team utilized the festival for KII and scheduling of the ocular visits/

community emersion to the barangays.

6. Team meetings (weekly/ monthly)

Weekly meeting/ coaching session

The 4th ACT regular meeting on August 4, 2014 was a whole day meeting activity held

at the temporary office at the Alliance Building, Municipal Compound, Dapa, Surigao del Norte.

CEF Jerry Jay Galan served as the morning session minutes documenter while CEF

Jayvee Lao on the afternoon session with the following activity and agenda:

“Don’t worry!” learning-reflection session - a biblical reflection discussion with the

team taken from Mateo 6:25-34. It aims to identify the worrying condition/situation of

each team during the half month of stay in Dapa and how they learned to cope up

with the anxieties and to inspire the team to be more courageous and faithful amidst

difficulties in life.

Main agenda: 1) Presentation of AC, DAC, & FA updates on RMPO; 2) presentation

of August work plan; 3) review of the IPCRs; 4) tasking and agreements: submission

of needed reports on August 18 for SI and profiles, narrative reports on August 20,

minutes of the meeting on August 6 and 8, 2014.

Special agenda was the presentation about the Convergence by Mr. Kenneth John

B. Andohoyan. He also presented the office performance contract review (OPCR)

and the municipal transition plan (MTP).

On August 11, 2014, the team gathered at the Model Senor Citizen Building for the 5th

ACT regular meeting with MFA/CFF Keevin Jann Vercide as the minute’s documenter. The

following are the activity and agenda of the meeting:

Other matters

The team failed to have its weekly meeting on August 18, 2014 due to the urgent travel

of the AC and DAC for the RPMO meeting on the Accelerated CEAC on August 19-20, 2014

held at LJ Mega Convention Center, Butuan City.

Monthly planning and updates

For the month of August 2014, the team prepared their individual monthly work plan

for August 1 – 31 during its ACT brief meeting on August 1, 2014 at Butuan City. The

consolidated final work plan of the team was discussed and presented on August 4, 2014

during its 4th ACT regular meeting.

Submission of monthly work plan

The team finally presented the work plan for the month

of August 2014 during the 4th ACT regular meeting held on

August 4, 2014 at its temporary ACT field office in the Alliance

Building, Dapa, Surigao del Norte. The August 2014 was sent

via e-mail to RPMO on August 6, 2014 and copy furnished to

convergence, SLP and Pantawid.

7. Follow up enrollment form (SB Resolution)

On August 5, 2014, the AC talked with MSWDO

Nancybelle Trazares and SB Secretary Julius Navarro for

submission of lacking document for enrollment which is the

SB resolution for Capacity Building Implementation Support

(CBIS) local counterpart contribution (LCC). The SB

Team meets on its 5th ACT meeting on August 11, 2014 at the Model Senior Citizen Center, Dapa, Surigao del Norte. The team meet here since the Alliance Building was occupied accommodating visitors attending the festival activities.

Resolution No. 121 series of 2014 dated August 5, 2014 entitled “Resolution Expressing

Commitment and Support for the Local Counterpart Contribution for the KC-NCDDP Capability

Building and Implementation Support (CBIS) was approved by the municipal mayor Yulie

Ruaya on August 8, 2014 and e-mail copy to RMPO on the same day.

8. Report preparation (weekly & monthly accomplishment report)

Every members of the team submitted the weekly

accomplishment report (WAR) or the initial

DTR/Accomplishment report form every Friday of the month.

This WAR will be presented on every Monday ACT

meetings.

9. Report submission

SI report and TI report (narrative report)

The partial Social Investigation (SI) consolidated report for

July 2014 was submitted to RMPO on August 6, 2014. The

partial SI composed of the municipal background and the

context of barangay profile with some issues/concern and

possible solution identified.

Municipal Profile

Draft Municipal Profile (CDD form/template #1) was submitted via e-mail together with

the August 2014 draft SI report on August 21, 2014.

Barangay Profile

The CEFs have already submitted the Barangay

Profile of their assigned barangays on August 18, 2014 to

the ACT subject to the review of the AC and DAC prior to

submission to the RPMO.

Monthly accomplishment report (narrative

report)

The July 1-31, 2014 Dapa Team Consolidated

Monthly Accomplishment Report was submitted via e-mail to

RPMO on August 6, 2014 and copy furnished to

convergence, SLP and 4Ps of Dapa. An electronic file of the

said report was furnished to MSWDO Nancybelle Trazares

on August 5, 2014. The report contains the Narrative Report

on Field (July 14 - 28, 2014), the Cumulative Accomplishment Report Matrix for July 1-31,

2014, and the Identified Issues, Concern, Actions & Recommendations. The electronic copy

of the report was also shared to each member of the team to be informed of the consolidated

report and to inspire them of their significant contribution to the monthly team’s report.

This August 1-20, 2014 team monthly report will be submitted on August 25, 2014 via

e-mail also.

DTR/Accomplishment Report

On August 16, the team’s DTR/Accomplishment Report for August 1-15 was submitted

to RPMO. All electronic copies of reports were saved at the computer for future references.

Minutes of the meetings

The draft minutes of the August 4, 2014 ACT meeting were submitted to RPMO via e-

mail on August 6, 2014. Other previous minutes of the meeting with attendance sheets

were e-mailed also on August 8, 2014. Hard copies of all minutes of the meetings and

attendance sheets were keep at the office. The minutes of the 5th ACT regular meeting

was already submitted by Keevin Vercide to the ACT which will be forwarded soon to

RPMO via e-mail with the August 1-20, 2014 monthly report.

Attended Community Finance Facilitator’s Tactic Session (RFR, VLCC and LCC preparation is the main topic) at Beatriz Hall in Butuan City.

Submit it to Ms. Jonavie Bolanes at KC-NCDDP office in Butuan City the 14 staff DTRs/Accomplishment Reports with transmittal.

Check and monitor field staff: Harold Apat for continuation of Social Investigation and immersion at Barangay San Carlos. Visited the Daycare center & Elementary School at Brgy. San Carlos; Charlie: Continuation of Social Investigation and Social Immersion at Barangay Cabawa: visited the Daycare Center and puroks, observed the settlement of conflict or disputes headed by Punong barangay; Michael Jade Delarmente conducted Key Informant Interview, Informal Talk, & Ocular Observation in Barangay Dagohoy constituents for the continuation of Social Investigation; Jayvee Lao conducted continuation of social investigation at Brgy. San Miguel; Jerry Galan continued Social Investigation and barangay profiling for the finalization of report at Barangay Jubang; Keevin Vercide visited Brgy. Sta Felomina and Brgy. Consolacion with Peactche T. Cepeda for Social Investigation; Nelmar Gomez visited Barangay Corregidor to get the lacking data for barangay profiling at the same time having the immersion; Peactche Cepeda continued social investigation and gathered lacking data on Barangay Profile of Barangay Consolacion; Rohaida Hasaan continued of Social Investigation, Transect walk and Key Informant Interview (KII) at Brgy Bagakay; Violeta Namoc conducted 2nd Social Investigation at Barangay Buenavista and visited their Daycare center for validation.

23. Facilitate completion of enrollment requirement

August 14 Communicate the Mayor’s office and copy furnish the MSWDO on August 14 citing request for Certification or TOR of the hired Municipal Encoder who ensure that the data base in the LGU is in-placed and functional/operational. The Municipal Encoder should be ready for the upcoming training to be conducted soon; Certification of the utilization of trust fund account of supplemental feeding of the MLGU as temporary municipal account with separate book pending the opening of the municipal trust fund account for the KC-NCDDP; Lists and profiles of the MCT, MCAC/LPRAT based on the issued municipal Executive Order/s. The names of the MCT will be the basis for the invitation of the training that will be conducted soon.

24. Review and monitor the field staff

Monitor field staff and review FANIR of Jade who conducted Key Informant Interview, Informal Talk, & Ocular Observation in Barangay Montserrat: Keevin Vercide visited Brgy. Team representative suggested Mr. Andohoyan to conduct a brief orientation on the OPCR with KC-NCDDP Area Coordinating Team.

Inform the team prior to the visit for the team’s discussion on OPCR. Request if possible for another session with Mr. Andohoyan for the thorough discussion with the Area
