WAIVER AND RELEASE (must be signed by participate)I understand that my consent to these provisions is given in consideration of the acceptance of this registration and for being permitted to participate in this event. I am a voluntary participant in this event and in good physical condition. I hereby assume full and complete responsibility for any injury or accident which may occur during my participation in this event and I hereby release and hold harmless The Seiler Group, Merrill Lynch, Lower Makefield Township and The Bucks County Audubon Society and Delaware State Canal Park of any claims that I may have arising out of participation in this event, including personal injury or damage suffered by me or others, whether same be caused by negligence of any of the said parties agents or employees or otherwise. If I do not follow all the rules of this event, I understand that I may be removed for the competition. I give my full permission to any of the said parties to use any photographs, videotapes or other recordings of me that are made during the course of this event. I hereby grant permission for the child listed above to participate in Kids Dashes on April 20, 2002. Directions: From North: 95S towards PA. Take Exit 51A/Yardley. Taylorsville Rd becomes N. Main St. At 1 st light, make left onto Afton Ave. At end (stop sign), make right onto River Rd. Macclesfield Park is a ½ mile on right. (Park is approx. 7 minutes from exit) From South: 95N to Exit 51/New Hope. Keep right at fork onto Taylorsville Rd. Taylorsville Rd. becomes N. Main St. At 1 st light, make left onto Afton Ave. At end (stop sign), make right onto River Rd. Macclesfield Park is a ½ mile on right. Pre-registrations must be received by April 1, 2002 (photocopies accepted). Sign Waiver and Release required for each participant. I have enclosed a 100% Tax-Deductible Donation of $________ payable to “The Bucks County Audubon Society.”
