Single/Double Occupancy: $159 per room per night plus tax.

Includes $25.00 credited to MACA to defray meeting expenses.

The cut-off date for hotel reservations is friday, august 10, 2012.

Please note: all participants registering for hotel rooms, must also register with MACA.

meeting room is $159 per day. maca directors strongly encourage you to attend the program sessions Tuesday

online at www.maca.org. MACA will accept payment by check, made payable to MACA, mail or fax 314-

849-0988. Discover/Visa/MasterCard, or credit cards accepted by PayPal via online. The MACA early registration cut-off date is Friday, august 10, 2012. There is a late fee for registrations submitted after August 11.

The maca registration fee includes program speakers plus:

sPouses: All spouses are welcome to attend the speaker programs or golf. No organized activities are planned for spouses.

refund PoLicy: Any refund request must be made in writing and submitted to

eligible for special registration rates.

non-member regisTraTion raTes: Prospective members, professional

colleagues and guests are also welcome to attend the Annual Meeting at the

designated rates. If your company joins MACA and pays its 2012 membership dues prior to August 1, you and your spouse may attend at member rates.

STEP 3: GOLFThe golf tournament will be held on Wednesday, September 5. Advance registration, with payment, is required for golf. No registration at the meeting. Any refund request must be made in writing and submitted to the MACA office by Friday, August 17, 2012. No refunds after August 18, 2012.

golf: 2012’s tournament will be competitive and fun event for players of all abilities. you are welcome to put together your own team OR we’ll be happy to assign players to a team. your participation will make a significant donation to support the CropLife Ambassador Network program.

The 2012 MACA Tournament will be held on Wednesday, september 5 starting with lunch at 11 a.m. and the tournament is a shotgun start at 11:30 a.m. at the Dodge Riverside Golf Club, 2 Harrah’s Boulevard, Council Bluffs, IA 51501. The golf course is ten minutes from the Hilton Omaha.

Below are the basics for this year’s tournament: 1. 4-person teams. “Scramble” format. Tournament rules will be sent to all

golfers before the annual meeting. 2. One flight in the tournament. a. Based on each player’s handicap index or estimated

handicap based on reported scores, we’ll calculate a team handicap for the game. Players without a handicap index can report their last five 18 hole scores.

b. Players without a foursome will be assigned a team. 3. Prize payout to the top third of the teams.

The $90 early registration fee includes 18 holes of golf, cart, lunch and a donation to the CropLife Ambassador Network program. All golfers must register for the meeting to participate in the golf tournament. Final details will be emailed to golf registrants prior to the meeting. To reserve rental golf clubs at $20/set, please indicate your request on the 2012 Registration Form. It’s the individual golfer’s responsibility to pay for the rental clubs at the golf course.

Awards will be made immediately after golf at the Dodge Riverside club house. We should be finished by 5 p.m. and return in time for the President’s reception at the Hilton Omaha.

In order to keep the registration fee at $90, we’ll ask for you to be a MACA Annual Meeting Conference Sponsor – sponsorships range from $50 to $1,000 and include:

• Lunch for $15.00 per person. Estimate $900-$1,000.

Proceeds from the golf tournament will be made to CropLife Ambassador Network to continue telling the good news story of Agriculture to elementary school children and teachers.

ADDITIONAL INFORMATIONTransportation from Airport – Hilton Omaha Hotel Shuttle Service runs every half hour. It leaves the hotel on the top of the hour and at the half hour and picks up on the way back so it usually leaves the airport around the quarter of the hour. Pick up is on the Baggage Claim level at door number 4 at the middle island. Guests can call the hotel at 402-998-3400 or at the Hotel Board by the baggage pick up.
