Liver transplantation involves the process of surgically removing a disease-inflicted or damaged liver and replacing the same with a healthy one either from a living donor or a cadaver. The concept of liver transplantation is not something new and the same has been carried-out out across the world for over four decades now. During this period of time, millions of patients all over the globe after undergoing liver transplantation at acute and critical stages, today lead a healthy, safe and joyful life.

The primary reason for liver transplantation is Liver Cirrhosis which is most commonly due to alcohol that destroys healthy liver cells and replace them with scar tissue. Cirrhosis may also be caused by viruses such as hepatitis B and C, autoimmune liver diseases, build-up of fat in the liver, and hereditary liver diseases. It is also a common scenario that at times, the cause of liver cirrhosis is completely mysterious and unknown.

Another common reason for liver transplantation is biliary atresia in children where Bile ducts, tubes that carry bile out of the liver, are ruptured or damaged and the resulting obstruction cause cirrhosis. Other major causes and concerns for liver transplantation are certain liver cancers, benign liver tumors and hereditary diseases. Furthermore, sudden or rapid liver failure may sometimes affect children and adults.

The common causes for sudden liver failure include viral illnesses and drug reactions and even certain herbal/traditional medicines. A liver transplant can save life if undertaken at the right time.

If the patient care team recommends that you need a transplant you will be counseled regarding your options of going for a living donor transplant or cadaver liver transplant. If you are able to find a willing living donor in your family, your waiting time could be as short as one week as much of the time would be taken up for tests and legal formalities before transplant surgery. The testing makes sure the

liver is healthy, matches your blood type, and is the right size so as to best chance of working in your body. In case you do not have a living donor, your name will be placed on the Kerala Network for Organ Sharing (KNOS) waiting list with your consent. Your blood type, body size and how sick you are, plays a role in deciding your place on the list.

The duration of surgery may vary from 6 to 14 hours after which the post-surgery care requires hospitalisation for an average of 2 to 3 weeks to ensure proper working of the transplanted liver. Medicines and auxiliary support will also be provided during this

period to prevent rejection of new liver and also to prevent infections. Regular examination will be carried out for infections and rejection and routine diet will resume starting with clear liquids, then switch to solid food as the new liver gains vitality and functionality.
