Au cours de l'exacerbation aiguë des BPCO, la VNI
• PP Inspiratoire : – supporte, en partie le Wresp
la ventilation alvéolaire hématose ++
• PP Expiratoire : diminue le Wresp (WPEPi)

Mise en route de la VNI
• Choix d’un masque adapté
• Réglage du respirateur :– AI : 10 à 20 cm HCO
– PEP : 0 à 6 cm H2O
– FiO2 à adapter au besoin
– AI + PEP < 25 cm H2O
• Surveillance : clinique, monitorage (ECG, SpO2), GDS

Conclusion
• Dans les DE des BPCO, la ventilation non invasive :
– Principal progrès dans la prise en charge de cette pathologie depuis…quelques décades
– Nouveau standard de soins dans la prise en charge des EA des BPCO
– Entre les mains d'équipes bien entraînées la VNI pourrait être envisagée comme premier recours même en cas d'encéphalopathie respiratoire sévère.
