China Visa Instructions The following 4 pages show the China visa application. Please fill out the application IN CAPITAL LETTERS, and cannot be handwritten. Do not leave any answer blank, put “None” if the question does not apply to you. Please note the format that China uses for dates: YEAR/MONTH/DAY. 1.1 Name must match your passport. If you do not have a middle name in your passport, write, “NONE or leave blank.” 1.2 If you were born in China, Taiwan or Hong Kong, you must complete 1.2. 1.5 Please follow China’s format for dates, year/month/day. 1.6 Should match where your current passport is from. 1.7 If you were born in Taiwan, simply list “China.” If you were born in the US, type “NONE” here. If you were born elsewhere, list the country of birth. 1.8 Please be sure to list City, State and Country. 1.9 If you do not have a driver’s license, you can write “NONE.” If you live in San Francisco’s jurisdiction and do not have a driver’s license, please list your passport number here. 1.10 Type P passports are “Ordinary” passports. 1.13 This should match what is listed under “Authority” on your passport.

The following 4 pages show the China visa application. Please fill out the application IN CAPITAL LETTERS, and cannot be

handwritten. Do not leave any answer blank, put “None” if the question does not apply to you. Please note the format that China

uses for dates: YEAR/MONTH/DAY.

1.1 Name must match your passport. If you do not have a

middle name in your passport, write, “NONE or leave

blank.”

1.2 If you were born in China, Taiwan or Hong Kong, you

must complete 1.2.

1.5 Please follow China’s format for dates, year/month/day.

1.6 Should match where your current passport is from.

1.7 If you were born in Taiwan, simply list “China.” If you

were born in the US, type “NONE” here. If you were born

elsewhere, list the country of birth.

1.8 Please be sure to list City, State and Country.

1.9 If you do not have a driver’s license, you can write

“NONE.” If you live in San Francisco’s jurisdiction and do

12 months or write, “NONE” here.

QUESTIONS, YOU MUST INCLUDE A SIGNED

LETTER OF EXPLANATION. FOR 3.3, YOU MUST

INCLUDE A COURT DOCUMENT WHICH SHOWS

WHAT THE CRIME AND PUNISHMENT WERE, IF

APPLICABLE.

completed this application for someone else),

you must answer all questions, and sign 5.5

using a black or blue pen.
