Background Info■ Diabetes is the 7th most common cause of death in the U.S.

■ In 2012, 29.1 million Americans was diagnosed with diabetes– 1.25 million Americans adults and children had Type 1

■ Type 1– Autoimmune disease– Early childhood

■ Type 2– Causes: obesity, poor diet and genetics– Insulin resilience

History■ 1953 – Tablets for testing urine glucose

■ 1964 – Blood glucose testing strips using color code by the Ames Company

■ 1970 – First glucose meter by the Ames Company

■ 1976 – First insulin pumps were invented

■ 2016 – First FDA approved automated insulin system

Common Treatments■ Close monitoring of blood glucose levels

■ Insulin injections– 3-5 injections per day

■ Eating healthy

■ Exercise

MiniMed 670G■ Medtronic PLC

■ First automated insulin pump FDA approved– Automatically delivers basal insulin

■ Waterproof

■ Spring 2017

■ Cost: $6000-$9000

How It Works■ Attached to the skin with medical

adhesive

■ Checks glucose levels every 5 minutes

■ SmartGuard HCL

■ Basal vs. Bolus

■ Maintenance:– Recalibration every 12 hours– Change glucose sensor every week– Refill insulin reservoir every 3 days

Benefits■ Precise insulin delivery

■ Lower A1c levels

■ Reduced risk of hypoglycemia

■ More flexibility

■ Better blood glucose control– Reduces risk of hyperglycemia and hypoglycemia

Limitations■ 14 years old and older

■ Only automates basal insulin delivery

■ Diabetic ketoacidosis

■ Skin infections

Clinical Trial■ 124 patients with Type 1 (94 adults, 30

adolescents)

■ 2 weeks using an insulin pump that was not automated

■ 3 month period using only MiniMed 670G

■ Results:– Overall A1c was reduced by 0.5%– Average A1c of 7.4% to 6.9% (Normal A1c

range is 4-6%)– 40% decline in time spent in hypoglycemia

■ 80% have chose to continue using the device

Future■ Additional sensors:– Heart rate– Activity– Sleep patterns

■ Fully automated insulin pump for both basal and bolus
