Message from the President & CEO

2

The past year has been a pivotal time for the healthcare industry in our country. With the implementation of the Affordable Care Act, hospices have seen unprecedented amounts of change in how business is done, including the creation of the Hospice Quality Reporting Program, adjustments to the reimbursement rate formula, and penalties on hospitals who readmit patients with certain chronic conditions within 30 days. While the launch of the health insurance exchanges has caught the attention of the media, the Affordable Care Act’s effects have rippled through all aspects of healthcare, including palliative and hospice care. What does all this change mean for our community? It means that in order to fulfill our mission to affirm dignity and hope for those facing serious illness, we must elevate palliative and hospice care within the overall continuum of care. To do that, we must raise awareness and continue to educate our community about the value of our palliative and hospice services and programs.

Palliative and hospice care are proven models to provide expert care and support while also managing costs of care most efficiently during a serious illness and at end of life. Palliative care is appropriate at any stage of a serious illness, and can be provided together with curative and life prolonging treatment to relieve the symptoms, pain and stress of a serious illness—whatever the diagnosis. Additionally, patients and family caregivers can receive emotional and practical support as they navigate their way through the decision process. Once the decision is made to forgo curative treatment, hospice care becomes an invaluable option, and integral part of the continuum of care. This is the time for a radical shift in the way we think about the future of our healthcare system. Rather than palliative and hospice care being considered options when “nothing more can be done,” these specialized areas of care and support must be

better integrated throughout the care continuum—to enhance the overall quality of care and patient experience while adapting to the changing landscape of healthcare.

Our accomplishments this past year reflect this new reality. The Palliative Care Center Silicon Valley opened its doors last July, giving patients with advanced illness access to outpatient services that relieve pain and symptoms and provide emotional and practical support—allowing them to spend more meaningful time at home. Through our new Community Life program, we are reaching community members long before they are coping with the crisis of serious illness to educate them on advance care planning, life transitions, and much more.

In this annual report, we recognize and thank our generous partners, friends, families, and donors who make our compassionate caring possible. Now more than ever, we need your continued support to assure that our community-based, non-profit hospice and palliative care survives the turbulent seas of healthcare change. Celebrating 35 years of service to our community, Hospice of the Valley is a nonprofit organization that exists to support patients and their families during one of life’s most challenging—yet most meaningful—times. Hospice of the Valley provides serious illness and end-of-life care programs and services, including palliative and hospice care, grief counseling for adults, children and families, and emotional support, education and practical assistance.

We affirm dignity and hope for those facing serious illness by providing comfort through compassionate palliative, hospice and grief care. We strengthen our community through advocacy, education, research and volunteerism.

Palliative Care Center Silicon Valley – The first independent, community-based, licensed outpatient center of its kind in California, the Palliative Care Center Silicon Valley provides a place to turn for patients and families living with serious illness. A division of Hospice of the Valley, the Center offers expert medical care and emotional and spiritual support, including care coordination, to relieve the symptoms, pain, and stress of any serious illness. In addition, the Center offers the Transitions program as a community service, utilizing specially-trained volunteers to provide assistance to patients and families.

Center for Grief & Loss – The Center offers support to anyone in the community who has experienced the loss of a loved one or a significant life transition such as divorce, job loss, retirement, or relocation. Individual counseling and loss-specific support groups are facilitated by professionally trained therapists or supervised marriage and family therapist trainees/interns. Our services for children and adolescents include individual counseling, support groups for children ages 4 to 18, and a unique expressive arts program, offering young people the opportunity to creatively explore feelings and emotions in a safe and comfortable space.

Community Life – Hospice of the Valley’s Community Life program provides education and outreach on end-of-life care, life transitions, advance care planning, estate and financial planning, and more. The Community Life program relies on the generous support of our community to provide this educational service.

Volunteers – Caring and extensively-trained volunteers provide patients, caregivers, and family members emotional and practical assistance. Other volunteers perform community outreach, office support, fundraising, and more. Our volunteers are special individuals for whom we are most grateful.

Hospice – Whether it is spending time with loved ones, fulfilling dreams, or simply remaining comfortable at home for as long as possible, Hospice of the Valley’s team of physicians, nurses, social workers, spiritual counselors, hospice aides, and grief counselors guide patients and their families to meet their end-of-life goals. Services are covered by Medicare, Medi-Cal, and most private insurance plans, and equitable access to care is available for patients and families regardless of their ability to pay.
