Providence TrinityCare Hospice Foundation

With gratitude, we salute and remember our Foundation’s founder, Mildred Marx, whose past vision has propelled us into our future reality.

“Hospice showed me how to live, how to cope…I’ve seen a lot of death. I face my mortality without fear. Hospice helps people get the most out of life, not death. We want you to live until the very day you die. And, we want you to live comfortably and fully in control, surrounded by people who love and care for you and think only of your own good.”

Page 3: Trinity Care Hospice 2011 Annual Report

2010 was a year we won’t soon forget here at Providence TrinityCare Hospice Foundation. We return to you with the wonderful news that 2010 was our most successful fundraising year to date! This meant an incredible amount of stimulation for our foundation, spurring expansion in all of our programs. We made large strides towards the

continuum of care we are building. The impact of our progress in 2010 for our patients and their families can only partly be captured in numbers, and it has proved to be equally difficult to express our gratitude with words. Therefore we dedicate this report you: the people who made our year so successful.

First and foremost, we thank M. Douglas as we received from his trust our largest gift ever – $2.5 million donated toward a Center of Excellence. The Center of Excellence will be the beginning of many changes in the way people view their medical care – care based on quality of life. We also would like to thank Mr. Michael Hayde, executor of the Trust, and Mrs. Jacqueline M. Glass, our Community Ambassador, for making this gift possible. They recognized the important work we do, and understood that this calling deserves long-term investments to assure that we will not only provide care in the future, but be able to educate future generations of clinicians in palliative and hospice care.

We also want to thank our wonderful community! From children’s groups to community service organizations, people all over our area helped us to provide the best possible care. We watched as car-loads of hand-made blankets, baskets of food, miraculous amounts art supplies, and countless hours of love and care found their way into the lives, homes and hearts of the people we serve.

In this report, you will read about so many phenomenal events and happenings from 2010, such as our grant from the UniHealth Foundation to expand TrinityKids Care’s services to include pediatric in-home palliative care in Orange County, and the exciting news that we are now able to offer gift annuities. Thanks to Providence Health & Services, we have our own gift annuity license.

In the midst of all this activity, the Board members and staff at Providence TrinityCare Hospice Foundation had the best seat in the house. From our bird’s eye view, we witnessed you, our loyal supporters, find beautiful avenues to express and pursue the TrinityCare mission in your own very special way. This year could only be described by one word: phenomenal – highly extraordinary, known through senses rather than through thought or intuition. More than observed, beyond documented, 2010 was felt by our staff and the people we care for, thanks to you. With this 2010 annual report, please let us give you a peak at the phenomenal carnival of life this year has been.

2 Providence TrinityCare Hospice Foundation

When we consider this word we think of Jacqueline M. Glass, Providence TrinityCare Hospice Foundation’s community ambassador, who was instrumental in securing a $2.5 million gift from the estate of M.A. Douglas, making it the largest single gift in the history of our organi-zation. Mr. Douglas, the self-made son of impoverished migrant laborers, came to appreciate the compassionate work of Providence TrinityCare Hospice through his long friendship with

his business partner, the late Jerrold Glass and his wife Jacky, who were generous supporters of the organization. Mr. Douglas died in June 2008 and the gift was made by his friend, business partner and estate trustee Michael Hayde, the CEO of Western National Group in Orange County.

Providence TrinityCare Hospice has been asked by other organizations to share its unique approach to comprehensive palliative care, and this magnanimous gift will enable it to form a program with an appropriate facility to meet this educational need. The Douglas’s gift will be used to make TrinityCare’s renowned palliative care program available to even more patients by developing a center of excellence, encompassing both educational and clinical functions in hospice and palliative care. This center will ac-celerate the sharing of best practices and encourage the growth of palliative care in the US by training tomorrow’s clinicians.

“Douglas’s gift to us represents the fulfillment of the classic American dream,” said Providence TrinityCare Hospice Executive Director Terri Warren. “A young man from a poor migrant family, through his own hard work and dedication, went on to become so successful in his business that he was able to leave this incredible, compassionate and lasting legacy. Through this gift he will touch the lives of thousands of people, of all ages, at their time of greatest need. They will truly have been touched by an angel.”

2010 ANNUAL REPORT 5

Perhaps this is why “foundation” is the word that has surfaced to describe the structure of charitable organizations, like PTCHF. Whether it is feeding the homeless, searching for a cure, or as in our case, easing suffering and transforming lives, charitable foundations are created with the intent to lay ground-work for a better future for us all. The UniHealth Foundation is a non-profit, philanthropic organization whose mission is to support and facilitate activities that significantly improve the health and well-being of individuals and communities within its service area. In 2010, you might say that the UniHealth Foundation helped us fortify our own foundation.

PTCHF received a $150,000 grant from UniHealth Foundation which enables TrinityKids Care to expand its services by providing in-home palliative care to children in Orange County. We remain the only dedicated pediatric hospice program for children with life-limiting illnesses and their families in this area. We used this grant to expand services to coordinate care for chronically ill children. Children currently enrolled in Medi-Cal, with chronic, but not necessarily terminal illnesses, who are 20 years old or younger, can now receive pain and symptom management, or palliative care, at home, at the same time they are receiving curative treatment. This pilot program, called Partners for Children, is made possible by California’s Pediatric Palliative Waiver.

This program in Orange County will expand to Los Angeles County in the coming year, and with the immense additional support from UniHealth, TrinityKids Care will be able to lay a solid foundation for developing innovative new ways to ease suffering and transform lives.

found·a·tion UniHealth $150,000 grant Leads to Expansion of Pediatric Palliative CareThe word “Foundation” has a home for itself in topics ranging from religion to engineering. The term can morph into literal incarnations for subjects in architecture, then flex back into metaphorical meanings when we discuss ideas about things like politics or community. But however it’s used, the essence of “foundation” appropriately remains intact, always referring to the ever-important “support” or “underpinnings” of whatever subject it’s proxy to in the moment.

Torrance Four Rotary: garden Project

Gardens are special. They have the power to grant us a totally unexpected surprise should our eyes wander down the street, or even out our very own kitchen window. No matter how simple the plot, whether sprawling or stately, gardens instantly bring about notions of abundance, mystery, serenity, and an over-all sense of healing. Gardens are always changing, always new. This deep sense of possibility is truly powerful to those who are in the midst of loss or transition, and that’s why the Rotary Courtyard Garden adjacent to our in-patient hospice suites at Providence Little Company of Mary Transitional Care Center means so much to us here at PTCHF.

Opened in 2009, the suites were developed to meet the needs of patients who require a higher level of care to manage pain and symptoms. These three private hospice suites are specially outfitted and designed for the kind of care our patients deserve, without forcing them to compromise the privacy they desire for themselves and their families. But while the suites and care were excellent from the very beginning, the courtyard area adjacent to the suites at the time of completion was still in need of a little freshening up – “a facelift.”

Wonderfully, Torrance Mayor Frank Scotto and Greg Becker, president of the Lomita Torrance-Airport Rotary, had a vision for that tiny plot. Understanding what a garden could bring to our suites, our patients, and their families, they offered to redesign and landscape the space. Before we knew it, help from the entire community began to pour in. Over $45,000 was raised and

many hours of manpower were volunteered. Next Ken Nagao, vice president of Groundworks Landscaping, coordinated those recourses, overseeing the labor and planting required to implement the designs of Margery Morris, landscape architect, helped by Alex Mascola of the Lomita Torrance-Airport Rotary.

Right now, what has been described as a “garden of repose,” the Rotary Four Courtyard Garden is growing strong. PTCHF would like to thank all who made this possible. In times of grave illness and transition, the garden will be there to echo what all who work for and support PTCHF believe: moments of renewal and peace in this life can spring even in the end.

com·mu·ni·ty The term “community” has over ninety different

definitions. Rooted in “Communitas” (cum meaning

“with” or “together,” and munus meaning “gift”),

community’s amorphous quality comes from its ability

to spring up anywhere – even on the internet. However,

there is one component that must be present in order

to create a community – people.

Providence TrinityCare Hospice Foundation believes

the people who have offered up their time, effort and

financial support for PTCHF throughout 2010 truly

evoke the root meanings that breathe life into the term

“community.” After this past year, PTCHF understands

why “community” is so hard to define. The truth is,

when you witness a group of individuals coming together

with the intent to give, the effects are hard to put into

words. Thank you!

SANdPiPERS

Founded in 1931, Sandpipers is a women’s organization that aims to serve the needs of the South Bay community through philanthropic programs. During the holidays, these dynamic ladies have made it a tradition to arrive with car loads of toys, grocery certificates, laun-dry soap and other essentials, for our neediest of families. With our shared community and purpose, PTCHF’s relationship with the Sandpipers has al-ways been a natural one. We are especially grateful for our newest partner-ship with Sandpipers – the annual For Our Children Food and Wine Festival, presented by Honda and held on the Honda campus at the end of April.

LAS CANdALiSTAS: WALk ON THE WiLd SidE

Since 1969, the women of Las Candalistas have raised funds for non-profit children’s and environmental organizations through “Walk on the Wild Side,” a much-anticipated annual event here in the South Bay. This year, TrinityKids Care was honored again as a recipient for this year’s walk, themed, “Dancing on the Wings of Nature – A Butterfly Adventure.” Held on the traditional last Thursday and Saturday of April, the walk included a special lunch, opportunity shop for gourmet foods and crafts, and of course, time to enjoy the always-impressive decorations and flower arrangements. Our heartfelt thanks to the women of Las Candalistas for their gifts totaling over $100,000 these past ten years.

viSTAS FOR CHiLdREN

This distinguished philanthropic organization is comprised of a 100% volunteer group of women from the Greater South Bay area. Each year they bless both TrinityKids Care and The Gathering Place with their generous financial gifts. They were so moved by our tremendous needs this past year they chose to adopt one of our families receiving services from The Gathering Place. They brightened Christmas for a widowed mother and her five children by giving them not only Christ-mas presents, but also groceries and clothing for the entire family.

NATiONAL CHARiTy LEAgUE

National Charity League has created a unique and powerful way for mothers and daugh-ters to spend quality time together through community service, leadership development and cultural experiences. These women have been generous to PTCHF with their hearts and time for many years by providing grants, art kits for our Legacy Art Program, hygiene kits for our adult patients, and much more. This year, each of the girls made blankets and held an art supply and book drive for our children’s program, TrinityKids Care.

SLAMMERS: kidS HELPiNg kidS

The South Bay Slammers Baseball team had a fantastic 2010, winning many tournaments including the 2010 Triple Crown Orange County Classic Champions! Feeling super blessed for their success, the team of 11-year-olds wanted to give back by helping other children who may be going through a tough time. Much to PTCHF’s delight, the Slammers chose to get involved with TrinityKids Care. The team snagged one last “home run” for the year by providing an entire family with food, clothing, and toys for a mom, dad and two children during the holiday season.

New and old friends had the opportunity to enjoy a couple of intimate gatherings at a private estate in Bel Air. Feasting on fabulous morsels from Crustaceans in Beverly Hills, guests were entertained by singer/songwriters Esai Morales and Ted Silbert, and had the opportunity to listen to heartfelt stories from Dr. Glen Komatsu, chief medical officer and Gay Walker, clinical supervisor, TrinityKids Care. Tugging at your heartstrings, they shared their stories of caring for children at times when it is most needed. Touched by these stories, over $125,000 was raised for TrinityKids Care.

SONgS FOR A SUNdAy: JERALyN gLASS REFELd

Over 200 family, friends and supporters witnessed a very special concert at the James Armstrong Theatre in Torrance. Songs for a Sunday featured melodies of inspiration and hope by accomplished vocalist Jeralyn Glass Refeld and pianist Christoph Weinhart. Interspersed with the music of Strauss, Schumann, Bach, Gershwin and Sondheim were heartfelt stories of love and laughter from Providence TrinityCare Hospice staff members.

EMBASSy diNNER: A NigHT TO HONOR FRiENdS

With the roar of the Long Beach Grand Prix in the background, Providence TrinityCare Hospice Foundation celebrated with over 120 longtime friends and donors at the Embassy Dinner held at the Westin Long Beach Hotel. A special honor was bestowed upon the Simmons family – Jack and Wendy, Brendan, Katie and Sal – for their dedication to and heartfelt support of Providence TrinityCare Hospice and specifically TrinityKids Care. Wendy Simmons, along with her family, has been instrumental in spearheading numerous events and fundraisers for TrinityKids Care.

Tom Sullivan, actor, entertainer and Founda-tion trustee was the keynote speaker for the evening. With songs and amusing anecdotes, Sullivan entertained and motivated the audi-ence with his multimedia presentation, while stressing the importance of the mission of Providence TrinityCare Hospice.

THE gATHERiNg PLACE CELEBRATES 10 yEARS OF SERviCE ANd iS FEATUREd iN Southbay Magazine

Losing a love one can be the most profound challenges one can face in a lifetime. It’s complex, painful and difficult. The Gathering Place, a bereave-ment center in Redondo Beach has helped more than 10,000 children, teens and adults cope with their loss and life transitions. Located at Beach Cities Health District, over 200 friends volunteers and staff joined together last May to celebrate the bonds of friendship and healing made possible through The Gathering Place.

Then, in December, Providence TrinityCare Hospice was fortunate to partner with Southbay Magazine for an intimate holiday dinner at the home of John, Carol and Heather Shafer in Manhattan Beach. Chef Chris-tine Brown from Restaurant Christine generously donated her services and a four course gourmet meal, and interior designer and event planner, Rob Esterly exquisitely decorated the tables and Shafer home. After the guests oohed and aahed over the décor and food, Claire Towle, director of The Gathering Place was able to educate guests on The Gathering Place, the only facility to offer counseling and support groups to those in the commu-nity who have suffered the loss of a loved one from any cause.

cel·e·bra·tion “Celebration” is an important concept for Providence TrinityCare Hospice Foundation. PTCFH understands that a moment is all you need to transform a life, therefore we never underestimate the power a few hours of celebration among our friends and supporters can have for us personally. “Celebration” can mean “to rejoice in or have special festivities to make a happy day or event,” or “to observe or affirm.”

Throughout 2010, PTCHF hosted many events that we believe satisfied “celebration” to its fullest. Your presence was not only a gift to us and all of our friends, it was also an affirmation of your support for our mission to ease suffering and transform lives. Thank you for celebrating with us!

Dining for a Cause

For longtime Manhattan Beach residents and high school sweethearts John and Carol Shafer, a spirit of giving has guided them through 42 years of marriage. John, a successful dentist in Hermosa Beach, frequently travels on mission trips

to rural parts of Honduras, Vietnam and Kenya and oers dental care to children in cancer clinics. Carol, a celebrated jewelry designer and consultant, is a member of Sandpipers (26 years!) and Neptunian Woman’s Club of Manhattan Beach, serves on the board of directors for the LA5 Rotary Club, and sings at Hope Chapel, Hermosa Beach.

Together, they frequently host events at their beautiful neo-classic home, the holidays being a particularly busy time for the active duo. Southbay Magazine was fortunate to partner with Providence TrinityCare Hospice Foundation and help promote one of their most rewarding programs, e Gathering Place in Redondo Beach. John, Carol and daughter Heather graciously opened their doors for a festive event that brought together a handful of invited guests from the community in support of this wonderful cause. Chef Christine Brown of Restaurant Christine generously donated her services and a four-course menu to the delight of the attendees, who dined on tables exquisitely decorated for the season by interior designer and event planner Robert Esterley. Here’s a peek inside the intimate gathering…

By way of an intimate gathering at a private Manhattan Beach residence, Southbay Magazine teamed up with Providence TrinityCare Hospice Foundation to celebrate life overcoming loss as witnessed at The Gathering Place.

It’s been 20 years of Sunday by the Sea with $4.7 million raised – what better way to celebrate than to honor 20 years of chairs! We were thrilled to be able to gather together the pioneers of Sunday by the Sea to reminisce and celebrate its success. “What began as a party at my home, with planning around my kitchen table, has turned into a signature event in the community,” said Jacky Glass, event founder and current Community Ambassador for Providence TrinityCare Hospice. More than 800 guests sampled fabulous food and wine from over 60 restaurants, wineries and beverage purveyors. Entertainment, auctions and the opportunity to win a trip to Hawaii allowed us to raise over $200,000 to support PTCH programs and services.

A special thank you to Westin Long Beach Hotel and Red Car Brewery.

Not one, but two Light Up A Life trees were filled with stars to benefit Provi-dence TrinityCare Hospice Foundation. Throughout the month of December trees at the Westin Long Beach Hotel and Red Car Brewery in Old Torrance were filled with special stars purchased in memory of lost loved ones. Gifts from these special trees are used to provide the comfort of hospice and warmth of the holiday season to the lives of our neediest, terminally ill pa-tients and their families.

The Westin Long Beach Hotel hosted a lively tree lighting ceremony for 200 city dignitaries, hospice employees, family and friends. Gourmet hors d’oeuvres and drinks were enjoyed by all, and the Long Beach Jazzers en-tertained guests with jazz renditions of familiar Christmas carols. Gold stars adorned the Westin Long Beach tree, with proceeds benefiting all patients and their families served through Providence TrinityCare Hospice.

At the Red Car Brewery in Torrance, owners Laurie and Bob Brandt in-structed their employees and servers to distribute envelopes with a bright red TrinityKids Care star to each patron. Servers collected stars and donations throughout the holiday season to benefit our youngest patients at the end of life, through TrinityKids Care. At the special tree lighting ceremony, guests enjoyed happy hour drinks and appetizers and an additional 15% of their dinner bill was donated back to TrinityKids Care.

Charitable gift Annuities and Planned giving

“Intention” is performing an action with the specific purpose of affecting the end or goal that is aimed at.

Providence TrinityCare Hospice Foundation makes every effort to be transparent with our intentions. We are happy to be open with our supporters about how we utilize the support we receive because we know that the very act of signing a check, showing up to an event, or delivering toys or food, you are consciously contributing to affecting our goal of knowing, caring for, and easing the way for so many.

We also know that many people intend to do more to help Providence TrinityCare Hospice, but don’t know how to move forward. Some people may feel that they just can’t give more of their time and money, but wish they could.

If you wish to do more, we have two simple ways you can support our much needed end of life and bereavement programs for adults and children: charitable gift annuities and planned giving.

A charitable gift annuity is a simple contract between a donor and Providence TrinityCare Hospice Foundation. A set donation is made and the annuity pays the donor a fixed amount for the rest of their lives. Simple, but so very helpful.

A planned gift is designated in a will or trust. Bequests are common, simple and easy. They can be handled in a variety of ways – IRAs, Keoghs or 401Ks can be designated to support the programs and services of PTCH.

If you would like further information regarding gift annuities or planned giving, please contact Robin Steen, Director of Individual Giving at 310-257-3533. Thank you.

out·stand·ing Circle of Life – Celebrating innovation in Palliative and End-of-Life Care

“Outstanding” is an adjective implemented when we try to describe a person, place, or thing that is marked by quality and distinction in the midst of others of its kind.

Therefore, “outstanding” is the perfect word to describe our clinical staff. Between our innovation in palliative and end-of-life care programs, and the impressive intangible gifts our team members’ bring to the table, we can be positive that PTCH makes a difference in the lives of our patients.

Providence TrinityCare Hospice, in conjunction with Providence Little Company of Mary Medical Center - Torrance, has received the prestigious Citation of Honor from the Circle of Life Award committee. This committee, which includes national leaders in medicine, nursing, social work and health administration, annually identifies outstanding palliative and end-of-life programs across the country that excel in innovation. The committee specifically recognized PTCH for our pediatric care program (TrinityKids Care) and our focus on continuity of care across the settings from the hospital to home.

Employee giving Campaign Raises More than $30,000

Providence TrinityCare Hospice employees participated in a four week campaign to raise much needed funds for their very own programs and services. These generous employees took ownership of what they do and answered with their checkbooks. Over $30,000 was raised with 52% of our employees making a gift! What a powerful message to our community as to the belief our own staff has in the value of the work they provide to patients and families each and every day.

Many prizes were given to top participants including a Toshiba 32” Flat Screen TV to Maria Gomez, certified home health aide from the Torrance team. Thank you to our generous prize donors including Respiratory Therapy Home Care, TrinityCare Infusion Services, 24 Hour Home Care and Promoshop, and to our Community Ambassador Jacky Glass who hosted lunches for both the top individual donors and the After Hours Team.

Thank you to our fabulous employees who ease suffering and transform lives each and every day and night.

gen·e·rous A generous person is someone characterised by a noble or forbearing spirit and is liberal in giving.
