AACP Who We Are Founded in 1985, the American Academy of Cranio-facial Pain is the largest professional organization for those interested in the assessment, diagnosis and management of patients who suffer from craniofacial pain, temporomandibular disorders and sleep disor-ders in dentistry.

The goals of the AACP are: to elevate and enhance the professional standards of diagnosis and treatment in the field of Craniofacial Pain and TMD; to encourage

research and further study; to recognize for the benefit of the public those

Dr. Stoll is a board-certified physiatrist who is in private practice as President and Medical Director of Stoll Neurodiagnostics, PA, in Fort Worth, Texas.

Dr. Stoll graduated from the University of Kentucky in 1985 with BS in Psychology. He completed medical school at the Texas College of Osteopathic Medicine in 1990 after completion of a Predoctoral Fellow-

ship in the Department of Osteopathic Manipulative Medicine. He completed a one-year internship at the Osteopathic Medical Center of Texas in 1991 and completed his PhD in Physiology from the Univer-sity of North Texas in 1992. Dr. Stoll completed a residency in Physical Medicine and Rehabilitation at the University of Kentucky in Lexing-ton in June of 1995. In 2010, Dr. Stoll resigned from his 20 year career at the University of North Texas Health Science Center in Fort Worth (UNTHSC-FW) where he served as a Tenured Professor and Chairman in the Department of Osteopathic Manipulative Medicine and also as the Founding Executive Director of both the national Osteopathic Research Center and the Physical Medicine Institute at UNTHSC-FW.

Dr. Stoll is a Diplomat of the American Board of Physical Medicine and Rehabilitation, the American Osteopathic Board of Rehabilitation Medicine and the American Osteopathic Board of Neuromusculoskel-etal Medicine and Osteopathic Manipulative Medicine.

A general dentist and an Academy member, Anthony Sims will relate the integration of TMD and movement disorder treatment into the general practice and will bring scientific rationale for his recommended therapies. Included in the presentation will be an examination of the numerous neurological disorders that are associated with TMJD; a discussion of the

neurological pathways that are associated with TMJD at the second order neurons; a call to have, understand and properly read an MRI; and a look into the future of the neurological aspects of dentistry.

11:30 AM – 12:30 PM Interceptive Therapy as It Applies to Swallow and Habit Rehabilitation, Sleep, Breathing, and TMJ Dysfunction and Facial Development Joy Moeller, BS, RDH

Dr. Joy Moeller practices Orofacial Myofunctional Therapy and teaches courses in the subject internation-ally. She is currently teaching at UCLA in the Pedo/Ortho Department in Growth and Development and at NYU in the Sleep Medicine Department. With increased awareness of muscular causes and habits as being one of the causes of TMD, OSA, and orthodontic relapse,

the literature is showing that orofacial myofunctional therapy does help when it is monitored and organized in a systematic program that incorporates habit elimination, a sequenced exercise protocol and in some cases, Buteyko breathing exercises. Therefore, more dentists are seeking this treatment for their patients. This lecture will explore why and how orofacial myofunctional therapy is now becoming an accepted modality used in a multi-disciplinary team for many health care concerns. Participants can expect to learn how to recognize and treat posterior restricted lingual frenum and its connection to sleep disorders and TMD and to better understand oral habits that cause pain, including a successful treatment plan.

2:00 PM – 3:30 PM Understanding the Matrix Stabilization Paradigm of Chronic Disease Joseph McWherter, MD

Chronic disease affects one in four Americans, and by 2030 projections estimate that this number will increase to one in two. The underlying etiology of these diseases is chronic biologic inflammation resulting in a breakdown of the microenvironment or extracellular matrix (ECM) of the cell. Included in these chronic afflic-tions is temporomandibular joint disorder (TMJD), an

umbrella term which covers a chronic inflammatory disorder mani-fested by pain. Our clinical findings based on in-vivo animal models and human efficacy studies indicate that stabilization of the ECM of the temporomandibular joint by applying topical nutraceutical bio-modifi-ers results in resolution of TMJD symptoms. Dr. McWherter runs a two-office women’s clinic (which also treats men) in the Fort Worth metroplex area. He uses an integrated approach involving detoxifica-tion, the GI system, the cardiovascular system, the immune system, the neurological system and the hormonal system to treat his patients.

Dr. Payam Ataii lectures internationally on laser dentistry, marketing and clear aligner orthodontia. He will relate the integration of these facets with TMJD. With more than a decade of clinical experience, Dr. Ataii is a graduate of the Tufts Dental School, where he ranked first in his clinical class and also served a Harvard Externship. In 2010, he was recognized by The

Academy of Dental Learning as the Top Dental Speaker for the year. In his presentation, he will address identifying a patient’s Craniofacial pain and the use of lasers for a better smile and facial appearance; diagnosing and treating malocclusion and chairside techniques made easier; combining sleep medicine and ortho into a solo practice; clinical guidelines for identifying and stabilizing temporal mandibular disharmony; providing best care possible while running an economi-cally successful practice; and three surefire marketing tips for driving practice referrals.

8:00 AM – 9:30 AM Innovative ways of Treating Neurological Pain Michael R. Warner, DDS, PhD

Dr. Michael Warner is an oral and maxillofacial surgeon who earned his bachelor and master degrees in chemistry and his PhD in cell biology and anatomy. Temporomandibular joint pathology exhibits similar disease processes as seen in the somatic joints. Therapy has typically focused on the superior joint space. Arthroscopic observations demonstrate that the

inferior joint space is commonly involved and in some cases, exclu-sively involved. Common intra-articular disorders will be reviewed from an MRI/arthroscopic perspective such as closed lock. Joint pathology and its management will be reviewed using techniques ranging from home physiotherapy and medications to total joint replacement. Participants will become familiar with less common intra-articular, vascular and neural disorders that can mimic common TMD and will become familiar with using MRI to diagnose common intra-articular TMJ disorders.

9:30 AM – 10:30 AM Diagnosing and Treating TMD: Thinking Outside the Joint Jeff Johnson, DC

Dr. Jeff Johnson is a young, cutting –edge chiropractor who has worked extensively with a variety of ills, from special needs kids to adults with “intractable” pain to athletes with minor to major injuries and consequent need for rehabilitation. This includes TMJ disorders, which he sees as an accommodation to other maladies. He utilizes manipulative therapy, myofascial therapy,

cold laser therapy and cryosauna therapy. His was the 11th Cryosauna installed in the United States – there are now about 20. Cryosaunas gained notoriety when the Dallas Mavericks players utilized the therapy in their run to the NBA championship. Dr. Johnson’s presenta-tion is designed to show the importance of “thinking outside the joint” when diagnosing and treating TMD. The chiropractic approach to treat-ing musculoskeletal pain typically emphasizes a holistic approach. The assessment and subsequent treatment of the TMJ benefits from a similar approach. Takeaways include identifying musculoskeletal dysfunction in other areas of the body that can contribute to TMD; non-traditional treatments that work and why; and when and how to find competent alternative medicine practitioners to include on your treatment team.

11:00 AM – 12:00 NOON Connective Tissue, Faulty Occlusion and Implications for Body-wide Repercussions Judith DeLany, LMT

Judith DeLany is a an author of manuals and articles on pain management. She has co-authored two textbooks on clinical application of neuromuscular techniques. She lectures and trains internationally. During this presentation, she will discuss connective tissue, the single largest tissue component of the body, which gives rise to a fascial network that embraces and

enfolds all other soft tissues and organs. Fascia forms a totally con-nected network, from head to toe, so much so that a fallen arch

requires a repositioning of the head. Likewise, faulty occlusion can affect lumbar and sacroiliac mobility, as well as alter weight distribu-tion on the feet. When structural stresses are imposed, such as chronic strained sitting postures, fascia deforms, distorts, and dehydrates in an effort to adapt. Eventually trigger points develop, which can refer pain and a variety of paresthesia, limit range of motion, and weaken muscle strength. These adaptations may affect osseous and cartilaginous structures, with implications for body-wide repercussions. Her session will present the body as a tensegrity structure arising from a compre-hensive connective tissue network; discuss implications of everyday habits that have long-term impact on structural integrity; and consider the vast influences of the structures that lie below the clavicles on those that lie above them.

1:30 PM – 3:00 PM Pain – Sleep Interactions: Implications for Pain Management Donald Watenpaugh, PhD

Dr. Donald Watenpaugh is a physiologist who runs a sleep clinic. Before entering into an agreement 15 years ago with a physician to run the sleep clinic, Dr. Watenpaugh was a research physiologist and still has a bent toward research. This presentation will examine how pain reduces sleep quality and quantity, and how this in turn interferes with the somatotrophic and

analgesic properties of sleep. Such interactions create a vicious cycle that can delay healing and prolong pain. Conversely, protection of sleep from pain may help reduce wakeful discomfort by facilitating restorative functions of sleep. However, pitfalls in this process can circumvent the goals of improved sleep and reduced pain. The session will include discussions of how pain, and particularly craniofacial pain, impacts sleep quality; how the amount and quality of sleep affect sources and experience of pain; and implications of sleep – pain interactions for caregiver decision-making.

3:30 PM – 5:00 PM Dental Whole Body Connection: Unconventional Paradigm for Diagnosing and Treating Acute and Chronic Pain Patients Gerald H. Smith, DDS

Dr. Gerald Smith has been practicing dentistry for more than 40 years. He is a recognized international authority and pioneer in craniomandibular somatic disorders with a focus on resolving chronic pain. Dr. Smith’s presentation presents a comprehensive approach that defines uncommon causes for dental and somatic pain. An integrated system using occlusal

cranial, osteopathic, chiropractic, medical and quantum energy concepts provide the basis that 70% to 90% of all medical problems have a dental origin. The Dental Whole Body Connection is an intelligent evolution of over 42 years of clinical practice and provides a common sense approach to resolving pain. The session will include discussions of the true functionality of the occlusal cranial system; underlying etiologies of idiopathic and atypical facial pain; and an uncommon denominator present in most chronic pain patients.

COURSE SYNOPSISCraniofacial pain and TMJ disorders are often confusing and dif-ficult to accurately diagnose and treat. This lecture will provide the attendee with the background and knowledge to evaluate and differentially diagnose the most common craniofacial pain and TM disorders encountered in the dental practice. Included in the lecture will be a head and neck anatomy review, princi-ples of examination, imaging techniques, differential diagnosis and basic treatment protocols. When correctly diagnosed, all of these disorders tend to respond very well to early treatment. When ignored or misdiagnosed, these relatively simple prob-lems may quickly become chronic and complex pain disorders.

What attendees will learn• A review of craniofacial anatomy.

• Examination and imaging techniques used in the diagnosis of craniofacial pain and TM disorders.

• How to differentially diagnose and recommended treat-ment for cases of capsulitis, trismus and non-reducing disc displacement.

Program 2*: The Dental Hygienist’s Role in the Evaluation and Treatment of Sleep Disordered Breathing

COURSE SYNOPSISThe diagnosis and treatment of obstructive sleep apnea and snoring has exploded! Dental Hygienists are uniquely placed as primary care clinicians to be instrumental in the screening, referral and member of the treatment team for patients with sleep apnea and snoring problems. Oral appliance therapy has become a first line therapy for many patients, and is often the only viable hope for patients who have given up on CPAP. This lecture will give the attendee the background and tools they need to literally save the lives of patients with obstructive sleep apnea.

What attendee will learn• The basics of normal sleep and sleep disordered breathing.

• To screen their patients for possible sleep disorders.

• To work as part of the medical/dental team in the treatment of patients with obstructive sleep apnea and snoring.
