Mary Klockowski, RN Mrs. M. RN

Mrs. M, RN, was hired to replace a retiring school health provider. Having never been a school nurse prior to this appointment she has been learning on the job. When reviewing immunization records for students it was noticed that a number of students were out of compliance. She disclosed this situation when LHD staff made a routine visit (on 4/19/2018) to the school to drop off parent information fliers for the upcoming school year. LHD staff returned to the Afton school on 4/24/18 to offer assistance and review records that were out of compliance. Mrs. M, RN, attempted to contact parents and primary care providers of these students in the following week. On 4/30/18 LHD staff returned to Afton school to help school nurse contact parents and HCPs. LHD staff notified NYSDOH regional office to discuss the situation. On 5/1/18 LHD Staff returned to Afton school to aid in further chart review and answer questions from school nurse. On 6/11/18: phone call made to discuss upcoming school year and to schedule a visit in July (7/31/2018).

New staff not familiar with immunization requirements, lack parent follow through, school administration unfamiliar with requirements.

School to mail reminder fliers to parents in August and include LHD immunization clinic schedule in mailing. Nurse to follow up with phone calls within the first few days of the new school year to parents of students lacking vaccinations. Administration will be invited to attend a BOCES meeting where immunization requirements will be discussed.

Phone: School nurse unavailable for visit at the school. LHD contacted school nurse via phone to address questions and review immunization status of students school nurse had questions about. Used NYSIIS to compare to school records to assess if student meets school requirements. Reviewed ‘In Process’, ‘14 Grace period’, ‘Exclusion’, and ‘Exemptions’. Nurse states that she will be attending the School Nurse Orientation (NYSASN) in August.

PLAN: School nurse to call or email with any questions. LHD to visit school in Q3 prior to School Survey being due.

UPDATE (Q3) New Intervention: Educate LHD patients at each visit of recommended immunizations to bring that patient up to date as per ACIP recommendations.

UPDATE (Q3) Performance Measure:Track the number of vaccinations recommendations made by LHD staff, and the number of recommend vaccinations a patient takes within the 19-64 age group. For example: Patients generally schedule an appointment at the LHD requesting at least one specific vaccination. We will be tracking how many additional vaccinations the patient may need using NYSIIS/Vaccination Record and ACIP recommendations. In addition to tracking the number of vaccines our staff recommends, we will track how many of these vaccination are taken by the patient and how many are refused.

UPDATE (Q3) Baseline: LHD we determine the number of vaccinations the patients initially schedule their appointment for Baseline. (For example: Patient schedules appointment for Tdap vaccine)

UPDATE (Q3) Target:At least a 10% increase in the average number of vaccinations administered to patients aged 19-64 at the LHD. (Average determined by # vaccines requested at time of scheduling appointment vs. number of vaccinations actually administered).
