As with all matters related to Hospice of the North Country, it is a collaboration of employees, patients and their families, volunteers, Board members, and community members to make a meaningful impact and to nurture growth. It is important to never underestimate the power of a team working together—it is the driving force behind fulfilling our promises to those we serve.

This annual report highlights important developments and accomplishments that reflect our commitment to our core values: Growth, Care, Freedom, Integrity, Justice, Life, and Stewardship. These values are the foundation upon which we conduct ourselves; by striving to ensure we operate in a way that best serves our patients, their families, our employees, and our community.

In the spirit Mr. Penney’s quote, one of our core values, Growth, dictates that “we will evolve and improve in all aspects of our mission.” In 2015 we made a concerted effort to nurture progress within our organization and in the community. These initiatives included implementing a state-of-the-art EMR system, staff satisfaction surveys, improved organizational processes, and greater community involvement.

We encourage and are proud that our employees continually pursue training programs and certifications that will enhance the level of care we provide our hospice patients and their families as well as community members.

We will continue to move forward by fulfilling our Mission of helping people of the North Country die in spiritual, mental, and physical comfort. Our Mission is rooted in the belief that by honoring one’s way of life we are able to support and encourage them and their families to determine their end-of-life care.

We could not fulfill our Mission without the unwavering support of our Board of Directors, Medical Directors, employees, and volunteers. Thank you for your hard work and dedication.

As my role as the Board of Directors President has come to a close, I would like to express my gratitude to the talented, dedicated, and compassionate employees and volunteers who give of themselves so that our patients die with dignity and in peace. I have experienced great personal and professional growth as I worked along-side my fellow Board members—thank you for your stewardship, guidance, and dedication. I welcome Tim Boshart as the new Board President. During the past year, we had the privilege of caring for 332 patients and their families. As we worked to ensure our patients and families received exceptional service and care during their deeply personal and challenging journeys, we also focused on growth within our organization. It is due to the hard work and initiative of our employees that we were able to experience our greatest successes and significant growth. We took the final step in implementing a state-of-the-art medical record system that allows our clinicians to create complete and compliant documentation from the point of care that meets regulatory mandates. As a result, our patients receive the best possible care through consistent adherence to care plans. Our efforts to nurture progress included conducted a series of employee satisfaction surveys that resulted in an improved Staff Development Program design and the development of a training and operations improvement project. We are proud of our efforts to evolve and improve in all aspects of our mission, as our Core Value, Growth, calls us to do.
