Antimicrobial resistance threatens the effective treatment of bacterial infections and is a serious problem worldwide.
Multidrug-resistant bacteria are difficult to treat and the treatment outcome is worse than with susceptible bacteria. In Korea, the an-
tibiotic resistance rates of the major Gram-positive bacteria methicillin-resistant Staphylococcus aureus (MRSA), vancomycin-re-
sistant enterococcus (VRE), and penicillin-resistant Streptococcus pneumoniae (PRSP) are very high. Clinicians should know the
risk factors for developing multidrug-resistant bacterial infection, update the changing local epidemiology of resistant bacteria, and
choose appropriate antibiotics in clinical practice. The overuse and misuse of broad spectrum antibiotics should be avoided. This re-
view focuses on the epidemiology and risk factors of MRSA, VRE, and PRSP, the major multidrug-resistant Gram-positive

서 론
다제내성균에 의한 감염은 환자의 사망 위험을 높이고, 질
병의 경제적 부담을 증가시킨다[1-3]. 전 세계적으로 항생제 내
성균의 비율이 증가하면서 사용할 수 있는 항생제의 개수가
점점 줄어들고 있다[4]. 페니실린을 사용하고 10년이 지나지
않아 페니실린에 내성인 황색포도알균이 나타났고, 이 내성
균주를 치료하기 위해 메티실린이 개발되었으나 곧이어 메
티실린 내성 황색포도알균(methicillin-resistant Staphylococcus aureus, MRSA)이 출현하였다[5]. 현재 국내 병원에서 분리되
는 황색포도알균의 60-70%가 MRSA로 알려져 있다. 최근에는
vancomycin에 감수성이 있으나 최소억제농도(minimum inhibitory
concentration, MIC)가 상승한 MRSA 균주가 늘어나고 있고,
vancomycin-intermediate S. aureus와 vancomycin-resistant S. aureus도 출현하였다[6]. Vancomycin 사용량의 증가와 함께 유럽에서

VRE)이 보고된 이래 전 세계적으로 분리 빈도가 증가하였다.
국내에서도 1992년에 VRE가 처음 보고된 이후 현재는 종합
병원이나 요양병원에서도 흔하게 분리되고 있다. 지역사회 획
득 폐렴과 수막염의 흔한 원인균인 폐렴사슬알균(Streptococcus pneumoniae)의 페니실린 내성률은 전 세계적으로도 높은 수
준이다(Fig. 1). 본 종설에서는 MRSA와 VRE, penicillin-resistant
Streptococcus pneumoniae (PRSP) 감염의 위험인자와 임상양상,
치료에 대해 알아보려고 한다.
본 론
메티실린 내성 황색포도알균(MRSA)
내성기전과 위험인자
황색포도알균은 건강한 성인의 30%에서 피부나 점막에 집
락균으로 존재하기도 하지만 숙주의 면역상태에 따라 다양
한 형태의 침습 감염(혈액, 뇌척수액, 흉수, 관절천자액, 뼈와
같은 무균 검체에서 S. aureus가 분리될 때)을 일으킨다. 페니
실린 분해효소를 생성하는 페니실린 내성 황색포도알균을 겨
냥하여 반합성 페니실린인 methicillin, oxacillin, nafcillin이 개
발되었으나, 이후 페니실린이 부착하는 세포벽 단백질의 구조
변화를 일으켜 내성을 획득한 MRSA가 출현하였다. Methicillin
내성 검사는 oxacillin으로 하는데 oxacillin에 내성인 황색포
도알균을 MRSA라고 한다. MRSA는 최근 개발된 ceftaroline,
ceftobiprole 외의 다른 모든 beta-lactam 계열의 항생제에 내성
을 나타낸다. 현재 국내 대학병원에서 분리되는 황색포도알균
중 60% 이상이 MRSA인 것으로 알려져 있다[7-9]. Vancomycin
MIC가 2 µg/mL 이하일 경우 감수성, 4-8 µg/mL일 때는 중등
도 내성, 16 µg/mL 이상일 경우는 내성으로 분류한다. MRSA
균혈증은 사망률이 30% 정도에 이르는 감염질환이므로 MRSA
감염의 위험인자와 치료원칙을 알고 있는 것이 중요하다[10,11].
현재까지 알려진 MRSA 감염의 위험인자는 과거 항생제 사
용, 입원, 투석, 요양병원 거주의 병력, 과거 MRSA 집락이
확인되었거나 감염된 적이 있는 경우, 혈관내 카테터를 가지
고 있는 경우 등이다[12-16]. MRSA 감염의 위험인자를 갖고
있는 환자는 기저질환때문에 지역사회에서 발생하는 메티실
린 감수성 황색포도알균(methicillin-susceptible S. aureus) 감염환자에 비해 예후가 나쁠 것으로 예상되지만, 일부 연구에
서는 기저질환을 보정하더라도 methicillin 내성이 황색포도
알균 균혈증 사망에 독립적으로 영향을 미친다고 알려져 있
다[17,18].
지역사회관련 MRSA
1980년대 초 미국에서 MRSA 감염의 위험인자가 없는 건
강한 지역사회 거주자에게 MRSA 감염이 유행하면서 지역사
회관련 MRSA (community-associated MRSA, CA-MRSA) 감
염이 알려졌다[19,20]. 미국의 경우 CA-MRSA는 대부분 경미
한 피부연조직 감염을 일으키지만 5-10%에서는 치명적인 괴
사근막염, 폐렴, 패혈증의 침습 감염을 일으켰다[21-23]. 이후
연조직 감염으로 응급실에 내원하는 환자에서 MRSA가 가
장 흔한 원인균으로 알려지면서 미국진료지침에서는 지역사
회 획득 화농성 연조직 감염인 경우 CA-MRSA를 겨냥하여 경
험적 항생제를 선택하도록 추천하고 있다[24,25]. 국내에서
CA-MRSA는 외국에 비해 드물다고 알려졌으나 최근 보고에
따르면 늘고 있는 추세로 생각된다[26-28].
임상증후군
MRSA는 피부연조직 감염(농가진, 종기, 봉와직염, 상처
부위감염)부터 폐렴, 균혈증, 심내막염 등의 침습 감염까지
다양한 범위의 감염병을 일으킨다. 또한 의료기기(혈관내 카
테터, 인공관절, 인공판막, 내고정장치)와 관련한 이물감염
의 흔한 원인균이다.

피부연조직 감염
황색포도알균에 의한 피부 연조직감염은 농가진(impetigo),
종기(furuncle), 큰 종기(carbuncle)로 나타날 수 있다. 병원내
에서는 수술과 관련한 심부조직 감염이나 농양 혹은 말초
혈관 카테터와 관련하여 정맥염이나 카테터 주위의 연조직
감염으로도 나타날 수 있다.
카테터 관련 감염
황색포도알균은 카테터 관련 감염의 가장 흔한 원인균 중
하나이다. 카테터 입구의 감염, 카테터 터널 감염, 카테터 관
련 균혈증 그리고 중심정맥의 감염 혈전염, 심내막염까지 일
으킬 수 있다. 전체 MRSA 균혈증 가운데 17-30%는 감염된
카테터가 원인 병소로 알려졌다[29,30]. 중심정맥 카테터를
많이 사용하는 혈액투석실, 암 병동, 중환자실에서 특히 문
제가 된다. 혈액투석을 위해 인공혈관을 이용한 동정맥루를
갖고 있는 경우에도 감염 병소가 되어 균혈증이나 감염 혈
전을 일으킬 수 있다.
균혈증과 심내막염
황색포도알균에 의한 심내막염은 지난 20년 동안 증가하는
추세로 최근에는 심내막염을 일으키는 원인균 중 25-35%를
차지한다[31,32]. 황색포도알균 균혈증에서 심장초음파를 했
을 때 10%에서 많게는 25%까지 심내막염이 진단되지만 단순
균혈증과 심내막염을 임상적으로 구별하기가 어렵다[33-35].
심내막염은 치료기간이 길고 적절히 치료 받지 않으면 예후
가 나쁘므로[36,37], 전문가들은 모든 황색포도알균 균혈증
환자에게 심장초음파를 시행하여 심내막염이 있는지 확인하
는 것을 추천하고 있다[38]. 경식도 심초음파가 경흉부 심초
음파보다 심내막염 진단의 민감도가 높으나[33,39,40] 모든
황색포도알균 균혈증 환자에서 경식도 심초음파를 해야 하
는지에 대해서는 이견이 있다[40].
폐렴
의료관련 또는 병원획득폐렴의 원인균 중 20-30%가 MRSA
로 알려져 있다[41,42]. MRSA 폐렴은 치료실패의 가능성이
높다[41,43,44]. 공기감염 또는 흡인에 의해 폐렴이 생기는 것
뿐 아니라 우측 심내막염이나 균혈증이 있을 때 혈행 전파에
의해 폐에 여러 개의 전이병변이 생길 수 있다.
근골격계 감염
황색포도알균은 골관절 감염의 가장 흔한 원인균으로 혈
행전파, 주위 연조직 감염의 전파로 발생하거나 수술이나 상
처로 인해 뼈에 직접 균이 침투하여 발생할 수 있다. 성인의
경우 혈행전파, 척추 시술이나 수술과 관련하여 감염 척추염
이 잘 생긴다. 신경학적 이상이 있을 때, 항생제 투여에도 감
염이 조절되지 않거나 통증 등의 증상이 호전되지 않는 경
우에 수술을 고려한다.
중추신경계 감염
신경외과 수술 후 수술부위 감염(피부연조직 감염, 골수
염, 뇌농양 등)을 일으킬 수 있다. MRSA는 수막염의 흔한 원
인균은 아니나, 병원내에서 뇌실내 단락술이나 체외뇌실배
액과 관련하여 수막염을 일으킬 수 있다. 이때에는 항생제
치료와 함께 뇌실복강션트와 체외내실배액관을 제거하는 것
이 중요하다[45,46]. 드물지만 균혈증이나 심내막염 때 전이
감염으로 수막염이나 농양이 생길 수 있다.
치료
MRSA 감염의 기본적인 치료 원칙은 다음과 같다. 1) 자
세한 병력청취와 신체검진으로 감염 병소와 전이 감염을 확
인한다. 2) 농양이 있으면 배농하고 감염과 관련한 인공물이
있으면 제거한다. 3) 균혈증에서는 항생제 치료 시작 후 추
적혈액배양검사를 시행하여 균 음전을 확인한다. 4) 균혈증
환자의 경우 심내막염을 확인하기 위해 신체검진과 심장초
음파를 시행한다. 5) 합병증이 동반되지 않은 균혈증은 균
음전으로부터 최소 2주간 항생제 치료를 하고 합병증이 동
반된 경우에는 4-6주간 항생제를 투여한다[38,47].
임상증후군에 따른 치료
피부연조직 감염 중 농가진은 국소 연고로 치료할 수 있
고 종기나 큰 종기는 배농만으로도 치료할 수 있다. 병변의
범위가 넓거나 전신 증상과 징후가 동반된 경우, 면역억제제
를 복용 중인 경우, 기저질환이 있을 때, 고령이거나 배농하
기 어려운 위치(얼굴, 손, 생식기 주변)에 농양이 있을 때는 항
생제를 투여한다. 배농 후에 반응이 좋지 않을 때도 항생제를
투여한다. 카테터 관련 균혈증에서 카테터를 제거하지 않으
면 균혈증 기간이 길어지고, 혈행전파 때문에 전이 감염의 위
험이 증가하며, 치료 실패와 사망의 위험이 증가한다[48-50].
따라서 카테터관련 균혈증에서는 카테터를 제거하는 것이
원칙이다. MRSA에 의한 인공판막 심내막염은 vancomycin에
rifampin을 병합하여 6주간 투여하고, 초기 2주 동안은 gentamicin
을 병합한다. 또한 조기에 판막치환술이 필요한지 평가해야
한다. 골수염에서는 6-8주간 항생제 치료가 필요하고, 감염
성 관절염은 관절흡인이나 배농과 함께 3-4주 동안 항생제
치료를 한다. 인공관절 또는 척추내 고정장치와 같은 인공물
관련 감염이 있을 때는 바이오 필름 형성으로 항생제 투과

력이 떨어지기 때문에 감수성이 있을 경우 바이오 필름을
투과할 수 있는 항생제인 rifampin을 병용해서 사용하는 것
을 추천한다[51-53]. 각각의 임상증후군에 따른 치료는 표 1
에 정리하였다.
치료 약제
MRSA 감염의 일차 치료제는 glycopeptide이다. Glycopeptide
는 MRSA의 세포벽 합성을 억제하여 살균작용을 나타내는
항생제로 vancomycin과 teicoplanin이 있다. Vancomycin이 미
국을 포함하여 전세계적으로 많이 쓰이고 teicoplanin은 영국
과 유럽에서 많이 쓰인다[47,54]. Vancomycin은 신기능이 정
상인 경우 15-20 mg/kg을 8시간에서 12시간마다 정주한다. 신
장으로 배설되기 때문에 신기능에 따른 용량 조절이 필요하
다. 패혈증 상태이거나 수막염, 폐렴, 심내막염과 같은 중증
감염의 경우 25-30 mg/kg의 부하용량을 투여해 볼 수 있다
[55,56]. 황색포도알균 감염에서 vancomycin의 효과를 예측하
는 가장 좋은 약동력학적 지표는 area under the curve (AUC)/
MIC로 알려졌으나[57,58], 임상에서 모든 MRSA 감염 환자
에게 AUC/MIC를 측정하기는 현실적으로 어렵기 때문에 목
표 AUC/MIC 수치에 도달하는 최저농도(trough level)를 대체
지표로 사용한다. 항정 상태에서 측정한(3번째 또는 4번째
투여 한 시간 전 농도) vancomycin 최저농도가 15-20 µg/mL
에 도달하는 것을 목표로 한다. 단순 연조직 감염이고 신기
능이 정상일 때는 몸무게에 맞춘 표준 용량을 투여하면 되고
vancomycin 농도를 측정할 필요는 없다. Vancomycin의 주입
관련 부작용으로 red man syndrome이 3%에서 11%까지 발생
하는 것으로 알려져 있다[59,60]. Vancomycin 투여와 동시에
얼굴과 상체에 홍반 발진이 생기면서 심한 경우 혈관부종이
나 저혈압이 발생할 수도 있다. 주입관련 부작용은 아나필락

시스와 달리 처음 투여 때부터 발생할 수 있고, 발생 기전도
아나필락시스와 달라 약제 투여를 잠시 중지하고 대증 치료
후 증상이 호전되면 주입 속도를 늦추어 다시 투여해 볼 수
있다. Red man syndrome을 예방하기 위해 5% 포도당이나 생
리식염수에 vancomycin을 5 mg/mL 이하의 농도로 희석하여
1분에 15 mg 이하의 속도(1 g 투여하는 데 1시간 이상 주입)
로 투여한다. Vancomycin은 신독성의 부작용이 있는데, 용량
이 늘어날수록, 최저 농도가 높을수록, 신독성이 있는 다른
약물을 같이 사용하거나 혈관에 작용하는 약제를 같이 투여
할수록 신독성이 발생할 위험이 높아진다고 알려졌다[61-63].
Vancomycin 투여 기간 중 1-2% 환자에게 호중구 감소증이
생길 수 있고, 장기간 사용 시에는 12-3%까지 발생하는 것으
로 알려졌으나 약물을 중단한 후에는 대개 회복된다[64-66].
Vancomycin을 2주 이상 투여할 때는 백혈구 수치를 감시하
는 것이 필요하다.
MRSA 균혈증에서 균 음전이 되는 데 걸리는 기간의 중앙
값은 7-9일로[67,68] 적절한 항생제를 투여한 지 7일이 지났는
데도 균혈증이 지속될 때는 치료 실패를 의심하고 재평가를
해야 한다[47]. 환자의 전반적인 상태를 확인하고, vancomycin
농도, 균주의 감수성, 농양이나 이물 등 제거가 필요한 감염
병소가 있는지 다시 확인한다. MRSA 균주의 vancomycin MIC
가 2 µg/mL인 경우에는 vancomycin 농도가 적정 수준이고
가능한 감염 병소를 모두 제거하였음에도 환자가 중증으로
진행한다면 vancomycin 대신 다른 약으로 변경하는 것이 추
천된다. 미국감염학회 진료지침에서는 구제요법으로 고용량
daptomycin (10 mg/kg/day)을 우선 투여하도록 권고하고 있
으나, 국내에서는 희귀의약품센터를 통해 구입해야 하므로
현실적으로 사용하기 어렵다. 적절한 항생제 투여 후 7일 이
상 균혈증이 지속되더라도 환자가 임상적으로 호전추세를
보이고 MRSA 균주의 vancomycin MIC가 2 µg/mL 미만이라
면 vancomycin을 지속하면서 균이 음전되는 것을 기다려 볼
수 있다. 구제요법으로 quinupristin/dalfopristin도 사용해 볼
수 있으나 현재 국내에서 구할 수가 없다. Vancomycin 치료
실패 때 구제요법으로 vancomycin에 다른 약제를 추가하는
것과 linezolid를 기반으로 하는 치료로 변경하는 것을 비교
하였을 때 후자에서 30일 사망률이 낮다는 보고도 있으나
[69] 다른 연구에서는 linezolid와 glycopeptide 치료 성적의 차
이가 없었고[70], 좌측 심내막염에서 linezolid의 성적이 나쁜
것으로 알려져 있어 주의가 필요하다[6,69,71].
Teicoplanin은 vancomycin과 비교했을 때 정주뿐 아니라 근
주가 가능하고, 반감기가 길어 하루 한 번 투여할 수 있고 신
독성이 적으며 vancomycin의 주입관련 부작용인 red man syndrome
이 드물다는 장점이 있다[72,73]. 부하 용량(6 mg/kg/day, 400
mg/day)을 12시간마다 3회 투여 후 유지용량(6 mg/kg/day,
400 mg/day)으로 하루 한 번 투여한다. 부하용량은 신기능과
상관 없이 투여하고, 이후 신기능에 따라 유지용량을 조정한
다. 골수염이나 심내막염과 같은 중증감염에서는 12 mg/kg/
day의 고용량을 사용하도록 추천하고 있다[74]. 중증 감염이
나 신기능이 자주 변할 때 등 약물 농도 측정이 필요한 경우
검사하기가 용이하지 않고, MRSA 감염 치료에서 vancomycin
보다 우월하다는 임상자료는 없다[75-77]. 그리고 황색포도알
균에 의한 혈관내 감염과 심내막염에서는 vancomycin보다
성적이 좋지 않다는 보고가 있다[78]. 또한, 일반적으로 뇌척
수액과 유리체에 충분한 농도를 유지하기 어렵다고 알려져
있다[79]. 이러한 여러 이유로 임상에서는 MRSA 감염에서
vancomycin을 우선적으로 투여하고, vancomycin의 부작용 등
으로 투여가 어려운 경우 teicoplanin을 대체약제로 사용하는
경우가 많다.
Linezolid는 oxazolidinone 계열 항생제로 50S ribosome에 작
용하여 단백질 합성을 저해하여 MRSA에 대한 정균력을 가
진다. 피부연조직 감염과 폐렴 치료로 미국 식약청의 허가를
받았다. 경구 생체 이용률이 100%로 경구로 투여할 수 있고
신기능에 따른 용량 조절이 필요 없다는 장점이 있다. MRSA
폐렴은 vancomycin으로 치료를 하더라도 치사율이 50% 이
상으로 예후가 좋지 않아[80,81] 폐조직으로 침투가 잘 되는
linezolid와 vancomycin을 비교하는 많은 연구가 진행되었다.
일부 연구에서는 linezolid가 vancomycin보다 우월하다는 결과
를 보여주었으나[80] 대규모 메타분석에서는 두 약제가 안전
성이나 효과면에서 차이가 없다고 결론지었다[82,83]. MRSA
감염에서는 vancomycin에 부작용이 있거나 치료에 반응이 없
을 때 이차약으로 사용하는 경우가 많다. 경구 복용 때 설사,
오심, 구토 등의 소화기계 부작용이 생길 수 있고, 2주 이상
투여할 때 골수억제의 부작용이 나타나는데 보고에 따라
15-50%에서 혈소판 감소가 동반된다고 알려져 있다[84-86].
따라서 linezolid를 투여하는 중에는 주기적인 혈액검사가 필
요하다. 그 외에 세로토닌을 증가시키는 약물과 같이 복용할
때 열, 불안, 의식저하, 손떨림과 같은 세로토닌 증후군이 발
생할 수 있어 주의가 필요하고 이러한 증상이 나타났을 때
는 linezolid를 중단하여야 한다[87]. 장기간 사용할 때 시신
경염이 발생하였다는 보고도 있다[88,89].
Daptomycin은 cyclic lipopeptide 계열의 항생제로 MRSA를
포함한 다제내성 그람양성균에 효과가 있다. Glycopeptide 계

열의 항생제와 비슷한 범위의 항균력을 가지면서, glycopeptide
계열에 감수성이 저하된 균에도 효과가 있다. MRSA 감염에
서 vancomycin 치료 실패로 판단되는 경우 구제요법으로 우
선 고려되는 항생제이다. Daptomycin은 연부조직 감염에서
는 4 mg/kg로, 균혈증이나 우측 심내막염, 그리고 합병증이
동반된 연부조직 감염에서는 6 mg/kg로 하루 한 번 정주한
다. Vancomycin 치료 실패 때 구제요법으로 투여하는 경우 10
mg/kg로 사용한다. Daptomycin은 폐의 계면활성제에 의해 비
활성화되므로 폐렴에는 사용하면 안 된다. Daptomycin 사용
과 연관되어 근병증이 생길 수 있어 creatinine kinase 수치와
근육통이나 근위약감이 생기지 않는지 관찰해야 한다. 특히
스타틴을 복용하고 있거나 신기능이 감소된 환자에서는 더
주의가 필요하다.
Tigecycline은 glycylcycline 계열의 항생제로 광범위베타락
탐분해효소생성 그람음성균, MRSA, VRE 등 다제내성균에
항균력이 있다고 알려져 있다. 연부조직 감염과 복강내 감염
의 치료제로 미국 식약청의 승인을 받았다. 연부조직 감염이
나 복강내 감염에서 MRSA와 다른 균에 의해 복합감염이 있
을 때 tigecycline을 사용해 볼 수 있다. Tigecycline은 혈중 농
도가 낮고, 정균작용을 하는 항생제이며, tigecycline과 다른
항생제의 치료 결과를 비교한 메타분석에서 tigecycline 사용
군에서 사망률이 더 높았으므로 중증감염일 경우에는 사용
가능한 다른 약제로 치료하는 것이 추천된다[90].
반코마이신 내성 장알균(VRE)
내성기전과 위험인자
장알균은 정상 장내 세균총으로 병독성이 약하다고 알려
져 있으나 세팔로스포린 계열 항생제에 자연내성을 갖고 있
다. 최근에는 VRE가 증가하면서 이로 인한 감염병이 생기면
항생제 치료가 쉽지 않다. 임상에서 분리되는 장알균은 대부
분 Enterococcus faecalis와 Enterococcus faecium인데 E. faecalis는 대부분 페니실린 계열의 항생제인 ampicillin에 감
수성이 있으나 E. faecium은 페니실린 부착 단백질의 변화로
ampicillin에 내성을 보이는 경우가 많다. 미국에서 vancomycin
사용이 증가하면서 1980년대 후반부터 VRE가 출현하였고[91],
현재 의료기관관련감염을 일으키는 E. faecium 중 80% 정도
가 vancomycin에 내성으로 알려져 있다[92]. 국내자료에 따
르면 E. faecium의 vancomycin에 대한 내성률이 1998년은 11%
에서 2011년 23%로 증가하는 추세였다[93,94]. Vancomycin
은 장알균의 세포벽을 이루는 펩타이도글리칸의 전구체인
D-alanine-D-alanine에 부착하여 세포벽 합성을 방해하여 살
균작용을 나타내는데 VRE는 D-alanine-D-alanine을 다른 것
으로 치환시키거나 분해하여 vancomycin이 작용하지 못하게
한다. VRE는 한번 집락화되면 길게는 1년 이상 지속되며,
다른 사람에게 접촉으로 전파할 수 있다. VRE가 집락화되면,
vancomycin-susceptible enterococcus (VSE)가 집락화되었을 때
보다 감염병을 일으킬 위험이 더 높고, VRE에 감염되면 VSE
감염에 비해 입원기간이 길고 사망률이 더 높았다[95-97].
VRE 감염이 생기면 사용할 수 있는 항생제가 linezolid 정도
로 매우 제한적인 것이 가장 큰 문제이다. VRE 집락화의 위
험인자는 면역억제제 사용, 당뇨, 신부전 등의 중증질환, 입
원기간의 증가, 요양병원 거주, VRE 집락 환자 또는 감염환
자와 가까이 있는 것, 침습적 시술, 과거 항생제 사용 등이다
[98,99].
임상증후군
장알균은 복강내 감염이나 요로감염을 일으킨다. VRE가
집락화만 되어 있을 때는 치료할 필요가 없고 감염병을 일
으켰을 때 치료한다. VRE의 감염 병소는 VSE의 경우와 크
게 차이 나지 않으나 E. faecalis가 심내막염의 흔한 원인균
인 것과 달리 VRE에 의한 심내막염은 드물다[100]. 병원내
에서는 수술부위 감염, 카테터 관련 균혈증, 카테터 관련 요
로감염의 원인 병원체가 될 수 있다.
치료
VRE 감염 치료약제로 linezolid가 가장 많이 쓰이고 있으
나, 살균 작용이 없어 중증감염이나 혈관내 감염을 치료할
때는 감수성이 있는 다른 약제를 병합하여 사용하는 것을 고
려해야 한다. Beta-lactam 계열 항생제만으로는 장알균에 살
균력이 없으나 aminoglycoside를 병합하면 상승효과에 의해
살균효과가 있다. E. faecalis는 vancomycin 내성이라도 ampicillin
에는 대부분 감수성이므로, aminoglycoside에 감수성이라면
두 약제를 병합하여 사용한다. Linezolid는 이론적으로는 장
알균에 대해 정균작용만 있어 살균 작용이 필요한 심내막염
과 혈관내 감염에서 치료 효과가 떨어질 것 같으나, 심내막
염을 포함한 균혈증에서 치료 효과가 80% 정도로 보고되어
VRE 감염에서 가장 많이 쓰이고 있다[101]. Linezolid 투여
중 linezolid에 감수성이 저하되거나 부작용이 있어 사용하기
어려운 경우 daptomycin을 투여해 볼 수 있지만, 장알균에 대
한 MIC가 MRSA보다 높아서 MRSA 감염 치료에서보다 고
용량으로 사용한다. Quinupristin/dalfopristin는 E. faecium 감염
에서 사용할 수 있으나 현재 국내에서 구할 수 없다. Tigecycline
도 VRE에 효과가 있다고 알려져 있어 복합균에 의한 복강내

감염이나 연부조직 감염에서 사용할 수 있지만, MRSA 감염
치료 부분에서 언급한 것처럼 중증감염일 경우에는 사용 가
능한 다른 약제로 치료하는 것이 추천된다. 복강내 감염과
같은 복합균 감염에서는 VRE 외에 다른 균이 동정되지 않
더라도 장내 그람음성균에 항균력이 있는 항생제도 같이 사
용해야 한다. E. faecium과 E. faecails의 임상증후군에 따른
치료는 표 2와 표 3에 정리하였다.
페니실린 내성 폐렴사슬알균(PRSP)
내성기전과 위험인자
폐렴사슬알균은 성인 지역사회 획득 폐렴과 수막염의 가
장 흔한 원인균으로 인후에 집락화된 후 감염병을 일으킨다.

성인의 경우 나이가 증가함에 따라 침습 질환의 발생률과 사
망률이 증가하는데, 특히 65세 이상에서 발생률과 사망률이
높은 것으로 알려져 있다[102]. 1967년 호주에서 페니실린에
감수성이 저하된 균주가 보고된 이후 지난 20년간 다제내성
폐렴사슬알균이 전 세계적으로 늘어났고, vancomycin과 linezolid
를 제외한 다른 모든 계열의 항생제에 내성을 보이는 광범
위 내성 폐렴사슬알균도 보고되었다[103,104]. 페니실린 내
성은 폐렴사슬알균의 세포벽에 있는 페니실린 결합단백의
변화로 페니실린 부착력이 저하되어 발생한다. 폐렴사슬알
균의 페니실린 내성 기준은 2008년부터 중추신경계 감염과
비중추신경계 감염을 구별하여 적용하고 있는데(Table 4) 이
기준에 따르면 국내 중추신경계 감염에서 분리된 폐렴사슬
알균의 페니실린 내성은 80% 이상이지만 비중추신경계 감
염의 경우는 1% 미만이다[105,106]. PRSP 감염의 위험인자
는 과거 항생제 사용, 요양병원 거주, 최근 호흡기 감염, 면
역저하 등으로 알려져 있다[107-109]. 우리나라에서는 폐렴
사슬알균의 마크롤라이드 내성률도 70-80%로 높다[106,110].
퀴놀론 내성은 아직 심각한 문제는 되지 않으나, 증가하는
추세를 보이고 있어 지속적인 감시가 필요하다[105,106,111,
112]. 국내자료에 따르면 폐렴사슬알균의 levofloxacin 내성
률은 2.6-4.6% [106,113], moxifloxacin 내성률은 0-1.7%이다
[114,115].
임상증후군
폐렴사슬알균에 의한 감염병은 침습 질환과 비침습 질환
으로 나눌 수 있다. 혈액이나 뇌척수, 흉수 등 무균 검체에서
균이 동정되면 침습 질환으로 분류하고, 중이염, 부비동염,
균혈증이 동반되지 않은 폐렴은 비침습 질환으로 분류한다.
치료
폐렴사슬알균 감염은 폐렴, 부비동염 등 비중추신경계 감
염과 중추신경계 감염을 구분하여 치료하여야 한다(Table

5). 폐렴과 같은 비중추신경계 감염에서는 페니실린 내성률
이 낮아 beta-lactam 계열인 페니실린, aminopenicillin, 3세대
cephalosporin을 사용한다. 국내에서는 마크롤라이드 내성률
이 높아 지역사회 폐렴에서 마크롤라이드 단독 치료는 치료
실패의 위험이 높다. 수막염은 대표적인 침습 폐렴사슬알균
감염증으로 PRSP 수막염 환자가 페니실린으로 치료받을 경
우 사망률이 높아 적절한 항생제 선택이 중요하다[107]. 수막
염이 의심되는 경우 폐렴사슬알균을 겨냥한 경험적 항생제를
가능한 빨리 시작하여야 한다. 국내에서 수막염에서 분리된
폐렴사슬알균의 페니실린 내성률이 60-83%, 3세대 cephalosporin
비감수성(내성 또는 중등도 내성) 비율은 40-42%이므로[106,
116] 감수성 결과가 나오기 전까지 3세대 cephalosporin인
ceftriaxone이나 cefotaxime을 vancomycin과 병합 투여한다. 페
니실린에 감수성이 있는 것으로 판명되면 페니실린 단독 또
는 3세대 cephalosporin 단독으로 변경하고, 페니실린에 내성
인 경우 3세대 cephalosporin 감수성을 확인하고, 감수성이면
3세대 cephalosporin 단독으로 투여한다. 3세대 cephalosporin
에 내성인 경우라도 3세대 cephalosporin과 vancomycin을 병
합하여 사용하여야 한다. 폐렴사슬알균 수막염에서 스테로
이드 투여가 신경학적 후유증과 사망률 줄인다고 알려져 있
어[117,118], 세균성 수막염 환자에게 항생제 투여 전 또는 투
여와 동시에 dexamethasone을 투여하고 첫 4일간 유지한다.
수막염에서 스테로이드를 투여할 경우 vancomycin의 뇌척수
액 투과가 감소된다는 동물실험 결과가 있지만[119] 실제 사
람을 대상으로 한 연구에서는 vancomycin을 적정 용량으로
투여하였을 때 스테로이드를 같이 투여하더라도 vancomycin
의 뇌척수액 농도는 적절하였다[120]. 따라서 혈중 약물 농
도를 측정해 적절한 용법을 유지하면 스테로이드 투여 중에
도 뇌척수액 농도가 적절하게 유지될 것으로 생각된다.
Cephalosporin 계열 항생제 중에서는 ceftaroline과 ceftobiprole
이 PRSP에 효과가 가장 좋다. 3세대 cephalosporin 계열에서는
ceftriaxone, cefotaxime이, 4세대 cephalosporin 중에서는 cefepime,
cefpirome이 PRSP에 효과가 있다. 1세대, 2세대 cephalosporin
과 cephamycin (cefotetan, cefoxitin) 계열은 PRSP에 항균력이
없고, 3세대 cephalosporin 중 ceftazidime과 ceftizoxime은 효과
가 떨어진다. 3세대 경구 cephalosporin 중에서는 cefditoren이
PRSP에 효과가 있고, cefixime, ceftibuten은 항균력이 약하다
[121]. 국내에서 2세대 cephalosporin인 cefuroxime 내성률이
61.3%이므로, 미국진료지침과 달리 지역사회 획득 폐렴의 경
험적 치료로 권고하지 않는다[114,115].
Fluoroquinolone 중 moxifloxacin과 gatifloxacin이 levofloxacin
보다 폐렴사슬알균에 대한 효과가 낫지만, PRSP에 대한 임상
자료가 많지 않다. Fluoroquinolone 내성 균주에서 fluoroquinolone
치료 실패 사례가 있고, 침습 질환에서 levofloxacin 내성이
사망률과 관련이 있다고 알려져 있다[122]. 치료 전에는
fluoroquinolone에 감수성이더라도 치료 중 내성을 획득할 수
있어 이에 대한 주의가 필요하다[123,124].
Linezolid는 일반적으로 정균작용을 하는 항생제로 알려져
있으나, 폐렴사슬알균에 대해서는 살균력이 있고 PRSP에 대
한 항균력도 우수하다고 알려져 있다[125,126]. PRSP를 포함
한 지역사회 획득 폐렴의 치료제로 미국 식약청 허가를 받
았다. 수막염에 관한 연구에서는 linezolid와 cephalosporin의
병용요법이 vancomycin과 cephalosporin 병용 요법보다 효과
가 낮다는 보고가 있어, vancomycin을 투여할 수 없을 때
linezolid를 대체약으로 투여할 수 있을지에 대해서는 연구가
더 필요하다[127,128].
Carbapenem 계열 항생제인 imipenem, meropenem, ertape-
nem은 PRSP에 항균력이 있다. Imipenem은 경련의 부작용이
있고, ertapenem은 수막염 치료에 대한 임상 자료가 많지 않
다. Meropenem은 폐렴사슬알균 수막염 치료에서 3세대 cep-
halosporin과 비슷한 치료효과가 있어 페니실린이나 cephalo-
sporin을 투여하지 못하는 경우에 대체약으로 투여해 볼 수
있다[129-131].
결 론
MRSA, VRE, PRSP와 같은 다제내성균 감염증은 항생제
선택의 폭이 좁고 치료가 늦어지면 예후가 나쁘기 때문에, 위
험인자를 파악하여 의심될 때 적절한 항생제를 빨리 시작해
야 한다. 일단 다제내성균이 출현하면, 이후 전파를 막기 위
해 여러 가지 노력을 하더라도 다제내성균은 빠르게 전파되
어 이전의 상태로 되돌아 갈 수 없다. 또한 다제내성균이 늘
어나면서 광범위 항생제의 사용도 증가하므로 결국 광범위
항생제에도 내성을 갖는 내성균이 출현하게 되고 가까운 미
래에 다제내성균에 대해 더 이상 사용할 수 있는 항생제가
없는 상황이 생길 수도 있다. 다제내성균에 의한 감염을 줄
이는 가장 좋은 방법은 다제내성균이 처음부터 나타나지 않
도록 평소에 항생제를 올바르게 사용하는 것이다. 다제내성
균 감염이 의심되지 않거나 다제내성균이 분리되지 않은 환
자에게는 광범위 항생제를 사용하지 말아야 한다. 초기에 다
제내성균 감염이 의심되어 광범위 항생제를 시작하였더라도
이후 배양검사와 감수성 결과에 따라 가장 적절한 항균범위
