Early Support is a programme designed to deliver better co-ordinated service for parents/carers of disabled children aged between 0 and 5th birthday, as soon as the child
is identified as needing additional support.
Who is the Service for?Who is the Service for?Who is the Service for?Who is the Service for? A family whose child meets the following criteria:
• Aged 0-5th birthday
• Has multiple health needs or a disability
• Receives support from other services eg. Health, Education or Children’s Social Care
• Lives within the City of York

A referral can be made by any agency: Health, Education or Social Care. We also welcome referrals from families themselves. For further information and a referral form please contact the Early Support KeyWorker Co-ordinator as detailed on the back of this leaflet.
• Keep in regular contact, as well as responding to concerns
• Provide accurate and appropriate information and signpost to relevant services
• Listen to parents/carers views and ensure that other professionals are aware of them
• Provide an effective link between parents/carers and those working with their child
• Offer families Early Support information and materials
• Organises multi-agency Service Planning Meetings with the family at the centre

• Is the first point of contact, receiving referrals from parents/carers and professionals
• Co-ordinates and supports the Early Support KeyWorkers
• Is a member of the Child Development Centre Team at York Hospital
• Works closely with the City of York Children’s Centres
• Is a member of the Specialist Early Years Support Team
