Early Support is a programme designed to deliver better co-ordinated service for parents/carers of disabled children aged between 0 and 5 th birthday, as soon as the child is identified as needing additional support. A family whose child meets the following criteria: • Aged 0-5 th birthday • Has multiple health needs or a disability • Receives support from other services eg. KeyWorker do? KeyWorker do? KeyWorker do? A referral can be made by any agency: Health, Education or Social Care. We also welcome referrals from families themselves.
