Early Support is a programme designed to deliver better co-ordinated service for parents/carers of disabled children aged between 0 and 5 th birthday, as soon as the child is identified as needing additional support. A family whose child meets the following criteria: Aged 0-5 th birthday Has multiple health needs or a disability Receives support from other services eg. KeyWorker do? KeyWorker do? KeyWorker do? A referral can be made by any agency: Health, Education or Social Care. We also welcome referrals from families themselves. For further information and a referral form please contact the Early Support KeyWorker Co-ordinator as detailed on the back of this leaflet.
