From the President/CEO:

It’s all about the community

If I were to describe Winona Health during 2005 in a single word, it would be transformational. We couldn’t be more excited about the new building project and what that holds in store for transforming healthcare delivery for our community. Every day you can see how our campus is changing physically, and, if you have joined one of our tours, you even had a sneak peek at how our new facility will look. Each tour group has shown enthusiasm and delight in what the future holds for all of us.

While we await with great anticipation the opening of the new building and services in the spring, I want to tell you about the transformation that is occurring inside the walls of Winona Health. It’s a story of commitment and accountability, but more importantly it is work from the heart. The board of directors, the medical staff, and all staff and volunteers have a common vision – and it is all about you, the community!

Throughout the pages of this annual report you will read about how we are working to make a difference in the lives of those we serve and for the community as a whole. Winona Health has a very important role in the community – one we take very seriously. We have worked hard to demonstrate to ourselves – and now to you – the value of our local healthcare system with tangible results.

What will make a difference for Winona Health in the future? You. Our goal is to increase our market share. In other words, we want people in the community to seek healthcare services in Winona. We are unabashed about this goal. We are focused on listening to and learning from our patients, residents and their families about their experiences. We know we must address the issue of access to physicians, and we know you have high expectations. It is our intention to exceed them.

I suspect that those of you who experienced healthcare services in the past at Winona Health may not even recognize us if you came in today. It is my sincere hope that through reading this annual report and in talking with staff, physicians, and volunteers today, you will see Winona Health in a whole new way and will share in the excitement and pride we have for the future of healthcare in our community. More people are using Winona Health services

Winona Health had a banner year in 2005: The health system had an employee retention rate 10% higher than other facilities its size . . . four new physicians were recruited to the community . . . our building project remained on schedule and on budget . . . and our community nonprofit healthcare system realized gains from operations – despite an increasing need for charity care and growing expenses related to uncompensated care (care given to residents unable to pay their bills).

All of us on the Winona Health Board take our stewardship of the community’s healthcare system seriously. So we are pleased to report to you that Winona Health had a fiscally strong year, because 2005 saw an ever-growing number of you using Winona Health services . . .

Community Memorial Hospital’s Emergency Department cared for a record 17,949 residentsThe Dialysis Unit provided more than 6,500 treatments, up 12.68% from 2004The Family Birth Center had an 8.4% increase in nursery admissionsAdith Miller and Roger Metz Manors, for those needing memory care, were filled to capacity throughout the yearHospice Services’ average daily census quintupled from 4 in 2002 to 20.5 in 2005

I could continue enumerating Winona Health’s 2005 accomplishments, many of which are shared in the pages that follow. But what’s important to remember about our local nonprofit healthcare system is that any “profits” made are returned to the organization to strengthen and enhance its services. One very visible reminder of this investment in local healthcare is our $32 million building project. Spending that kind of money presented a big decision . . . one that required a good deal of thought. But it was also a decision we readily made because we knew the project would improve and enhance the quality of services we, our friends at Winona Clinic and other physicians serving this community deliver, in addition to making healthcare more convenient for area residents.

As we move toward the April 2006 completion of the building, we continue to collaborate with our local physician clinics to recruit primary care and specialty physicians to this area. The Winona Health Board is committed to making physician recruitment a top organizational priority, because it is a top community priority. Working together, we welcomed four new physicians to the community – an ophthalmologist, a pediatrician, an internist and a family practitioner. We know many of you will have the opportunity to meet them in the coming year as you address your family’s healthcare needs.

Providing exceptional healthcare services close to home is the heart of all we do as Winona Health Board members. We are proud to serve you and grateful for your support of Winona Health.

Cornerstone 1: A service environment where people can realize their full potential

Highlights from 2005 . . . Winona Health’s employee retention rate was 93.7%, compared to the national average of 83%.targeted efforts to ensure staff understood their work expectations resulted in a 21% increase in this item over the 2003 employee pride Survey (64.6% to 85.6%).a new performance appraisal tool and process was implemented, increasing timely completion of appraisals from 85.12% to 95.12%.Computer-based mandatory education was implemented with 98% completion by the due date.WH staff completed an institute for Clinical Systems improvement Culture of Quality survey, which showed:

88% of employees have a clear understanding of Winona Health’s mission, vision and values.81% feel encouraged to become involved in the improvement of patient care and service.72% agreed our quality goals and strategic plan are clear and well-communicated.71% agreed that quality improvement efforts are measured and communicated.

Workers’ compensation expenses decreased from about $396,000 in 2002 to $45,000 in 2005. in the same timeframe, lost days ratio decreased from 67 to 10 days (good), and the medical indemnity ratio improved from 1.38 to 7.70 (good).a cross-disciplinary team worked to implement a tobacco-free campus initiative, effective January 1, 2006.Winona Health created an annual Service excellence award program, recognizing five staff members for their exemplary customer service. this year’s honorees were lori Britton, Wendy Helmers, Barb Klink, Bev Speltz and Kris troska.

Building a Culture of Quality“An excellent organization is all about its people,” stated Susan Cornwell, chair of the Winona Health Board’s Human Resources Committee. “And Winona Health is fortunate to have such great people.”

Winona Health is committed to creating a culture of quality and a satisfying work environment. “To do so requires a shared vision for outstanding patient and resident care, allowing our people to do what they do best, and creating a culture of accountability where each employee takes responsibility for quality and excellence,” said Bill Gould, Chief People Resources Officer.

Staff, physicians and the Board of Directors work together to create that culture of quality. “Winona Health staff and our physicians provide excellent healthcare,” stated Dave Binius, chair of the Board’s Quality and Safety Committee. “Having attended many quality conferences, I have found that Winona Health is ahead of many organizations in providing the tools necessary and establishing the measurements to monitor quality healthcare.”

Those “tools” include membership in the Institute for Clinical Systems Improvement and adoption of the Malcolm Baldrige Quality Program. Both programs involve monitoring key performance indicators and sharing results throughout the organization. “Continually measuring our performance provides the mechanism we need to continually improve quality,” Binius said. “We have made, and continue to make, great progress on this front.”

Winona Health’s Environmental Services (EVS) department has made great strides in measuring and improving its performance. It recently earned 100% satisfaction scores for cleanliness at Community Memorial Hospital and Lake Winona Manor. “This is truly a team effort,” said John Matis, Director of Hospitality Services. “Everyone works together in order for us to achieve these outstanding results.”

Bonnie Mahutga, an EVS team member, said, “We have good employees who care about quality. I remind myself that, if my Mom or I were going to stay here, I’d like the rooms to be clean, so I clean accordingly. Our supervisors communicate regularly and check on us often. So we know our work is important.”

Matis, who also manages the Food and Nutrition staff, began improving quality by “listening to my staff. For example, Food and Nutrition staff told me they had to learn five or six jobs, which was difficult. So we moved to a culture of job ownership, where people own the job they do most often. ‘Swing staff’ fill in when necessary. Making these and other process changes improved team unity, and staff started seeing positive quality and satisfaction results,” Matis noted. “Then they began to receive recognition for their work.”

Most recently, the EVS staff earned the Environmental Services Account of the Year Award from Sodexho, the food and facilities management company that partners with Winona Health. “These rewards, along with being part of a successful team, go a long way toward building our culture of quality and excellence,” Gould concluded.

Bonnie Mahtuga is one of the environmental Services staff members responsible for high patient/resident satisfaction scores.

Highlights from 2005 . . . Community Memorial Hospital’s emergency Department cared for 17,949 area residents, an increase of 4.3%, while its Department of Behavioral Health’s admissions increased by 19% to 416; nursery admissions by 8.4% to 412; and Dialysis treatments by 12.68% to 6507.Community Memorial Hospital’s mortality rate was at 1.55%, compared to a national benchmark of 2.23%.Winona Health continued to receive staff suggestions for innovations and improvements, with more than 500 opportunity statements submitted and more than 350 implemented. the hospitalist program was initiated, ensuring a board-certified internal Medicine specialist is present in the hospital from 8 a.m. to 6 p.m. daily.Winona Health voluntarily participated in the Centers for Medicare and Medicaid Services project that defined effective care for three conditions. Compliance scores for all three conditions have improved since the project started:

acute Myocardial infarctions: 94.28 to 100% Congestive Heart Failure: 51.85 to 88.57%pneumonia: 63.38 to 89.66%

lake Winona Manor reduced its bed numbers to 160, opening up more single rooms and adding a second hospice room which has had a 47% occupancy rate. a feasibility study was completed for the october opening of weekend urgent Care Clinic.information technology (especially the electronic Medical Record) was used to improve patient safety and service quality by building in alerts and adding new features.Winona Health’s focus on patient- and family-centered care resulted in patient and resident inclusion on various teams, including the Breast Health team.

“To provide excellent care and service, we have to maintain our focus on our patients, our residents and their family members,” said Sara Gabrick, Winona Health’s Chief Nursing Officer. “We’ve worked in 2005 to sharpen our focus, adding a former patient to our Breast Health Team and implementing many staff-suggested innovations to improve patient, resident and family care and service.”

One hospital innovation implemented in 2005 was the start of a hospitalist program. “This program ensures that a board-certified Internal Medicine specialist is available at Community Memorial Hospital from 8 a.m. – 6 p.m. daily,” Gabrick explained.

“Internal Medicine physicians are experts in the care of adults,” said Charles Shepard, MD, an Internal Medicine doctor at Winona Clinic and Winona Health’s Medical Director. “The hospital-

based internists, known as hospitalists, care for most medical patients, assist in the care of surgical patients before and after surgery, and are available to respond quickly to changing patient conditions and to meet with patients and families.”

Robin Hoeg, RN, Winona Health’s Director of Inpatient Services, sees the benefits of having a hospitalist on a daily basis. “The

most beneficial aspect of the hospitalist program is having the physician available to patients and staff throughout the day. Nurses, other providers and family members can share their concerns with the hospitalist, and the hospitalist can share new assessments with the patient, family and clinical staff throughout the day.

“That’s a big plus for patients and their families,” Hoeg continued. “Hour-to-hour – and sometimes minute-to-minute – communication is occurring. Ultimately, that corresponds to improved patient care.”

Added Gabrick, “While we focus on patient and resident care at all Winona Health affiliates, we recognize that our patients’ and residents’ families are important members of the care team and are a key part of recovery. So our care and service initiatives, such as our hospitalist pro-gram, also take into account the needs of family members.”

Hospitalist programs have become increasingly popular throughout the country. “They have been shown to improve the care of inpatients,” Dr. Shepard said. “Because hospitalists are on-site throughout the day, delays in diagnosis and treatment are reduced, which leads to shorter hospital stays.”

CMH’s hospitalist program is staffed by internists from the Winona Clinic, with assistance from Mayo Clinic Internal Medicine fellows on the weekends.

One family’s experience with Winona Health

My mother-in-law, 79, was visiting us over Thanksgiving . . . She visited Community Memorial Hospital’s Emergency Department (CMH’s ED) twice in about a week – once with a gastro-intestinal problem that ended up being fairly minor (although not minor to her). The staff was very kind and professional. The second time she lost her balance and fell on the floor, suffering a concussion and moderate contusions to her face. Once again the staff was absolutely terrific. My mother-in-law, my family and I could not be more impressed and pleased.

This was not our first experience with your ED. Years ago, my then 7-year-old daughter fell off a swingset and had a laceration on the back of her head. She was terrified to go the ED . . . terrified of doctors . . . just terrified in general. The ED doctor was able to glue the laceration. But what impressed me is the sensitive way in which he treated my daughter -- not like another statistic coming through the ED, but like a terrified little girl that needed TLC. Thanks again for all CMH does for our community!

Highlights from 2005 . . . Winona Health’s building project remains on schedule and on budget. Completed in 2005 were the helipad and parking ramp, with the MRi and Surgery Suite set for occupancy in late fall 2005.Winona Health and Winona Clinic identified a strategic location for a Women’s Health Center on the third floor of the new Medical office Building.Four new physicians were recruited to the community:

Winona Health and Winona State university received a three-year $216,000 Minnesota Job Skills partnership grant to train Lean in Healthcare and leadership classes to 50 Winona Health employees who in turn will train all staff.the Winona Health auxiliary organized Follies, Holiday Happiness, Fantasy of trees and other events to increase awareness of Winona Health and support its services, donating $59,100 and more than 25,000 volunteer hours to Winona Health.

Cornerstone 3: Collaborative partnerships that serve and improve our community

Collaborating for women’s health“Winona Health is committed to meeting not only the current but also the emerging healthcare needs of the Winona regional community,” stated Loretta Bronson, Winona Health’s Chief Communication Officer. “To address these needs, we realize that we need to create collaborative partnerships with other community organizations.”

To help identify these needs, Winona Health staff uses a variety of listening and learning tools – from patient and resident satisfaction results to community surveys, focus groups and informal networking opportunities. “Using these tools, we recognized that more women wanted hometown access to specialized, personalized health services, so we began working to make that happen,” said Bronson.

Rachelle Schultz, President/CEO, was among the Winona Health staff who met with members of Winona Clinic’s Center for Women’s Health. She noted, “Research shows that women make 75% of all healthcare decisions, and they want a warmer, more user-friendly delivery system. Working jointly with the clinic, we identified space on the third floor of our new Medical Office Building for a women’s health center.”

Winona Clinic will operate this center, which will include services for women of all ages. “We’ll offer the full spectrum of women’s healthcare services in the center,” noted Troy Shelton, MD. “It will include services to address eating disorders and nutritional issues facing women, obstetrics care for pregnant mothers, gynecological care for menopausal women, bone and urological services for seniors – our services will address all their needs.”

In addition, Winona Health has created a Women’s Imaging Center within its Outpatient Diagnostic Center scheduled to open in April 2006. The center will feature digital mammography with computer-assisted detection (CAD) for breast masses, ultrasound for breast imaging and bone densitometry, according to Ann Baker, Director of Radiology Services. “It’s important that we have the best possible equipment for detecting breast cancer in the earliest stages,” Baker stated, “and our new imaging center will have just that.”

James Erwin, MD, the radiologist who examines mammograms done at Winona Health, explained, “Digital mammography is the wave of the future, and studies have shown that it results in improved detection of breast cancer in women with dense breasts. In addition, CAD studies show that it detects an additional 10-15% more cancers than without it. We’re committed to maintaining state-of-the-art technology and equipment to best serve women’s health needs,” he emphasized.

The Women’s Imaging Center will be a private area just for women, with three separate dressing and interview rooms, a private bathroom and its own waiting area stocked with coffee and refreshments. “Community women asked for greater privacy and more personal space during their mammograms, and we’re going to deliver on that,” Baker stressed.

Highlights from 2005 . . . Winona Health’s gain from operations was $2.54 million (4.1% operating margin) versus a budget of $1.69 million, the third consecutive year Winona Health outperformed budget despite:

Charity Care provided to patients increased by more than 300%, totaling $596,000expenses related to bad debt increased by 86% to $1,892,000employee health insurance costs increased by 24%

Winona Health worked with Winona County on mental health issues and received a stipend to help with 24-hour psychiatric call coverage.Winona Choice entered into a contract with Medica, introducing a third major health insurance option in Winona and enhancing rate competitiveness for employers and providers.Cost per adjusted Discharge, one efficiency measure, decreased from $6,020 to $5,801 after two years of increases.the Winona Health Foundation developed its mission, vision and values and its strategic plan.the Winona Health Foundation supported Winona Health:

Winona Health has experienced a very good year, and the financial results reflect that. Our balance sheet has over $100 million dollars of assets that are being deployed to provide excellent patient care in a fiscally responsible manner for the residents of the Winona area. That’s exciting!

From the Winona Health Foundation Chair:

Securing the future of community healthcare

Last year I shared with you that we recruited and installed a new Board of Directors to provide leadership and direction to the Winona Health Foundation. Our board has had quite a year!

I am excited by the mission and vision established by the Foundation board. The direction they establish will ensure that the highest quality healthcare will continue to be found right here in our backyard. It also assures Winona Health will continue to be community-owned, which is reflected in the Winona Health Foundation’s vision statement: To sustain Winona Health as an exceptional provider of locally owned, non-profit healthcare.

As we continue to strive toward that vision, the Foundation has adopted a mission: To gather, steward and distribute resources needed to support and advance the mission of Winona Health and its affiliates. The mission’s successful implementation rests on four cornerstones:

I have had the pleasure of guiding the Winona Health Foundation for the past several years. While proud of our many accomplishments, I believe the best is yet to come. In 2006, the Foundation will kick off a planned giving initiative that will secure Winona Health’s future. While I will be active in this initiative, I will be turning over the Foundation Board Chair’s position to Ken Mogren. An enthusiastic leader, Ken will continue to build on this year’s momentum, joined by Steve Blue who replaces Ken as vice-chair.

Following the sun . . .

On Monday, May 30, Memorial Day, we had friends over for a barbecue. Our son Scott was not quite himself . . . We were not particularly concerned, but did think his behavior unusual

. . . We both talked to Scott on Tuesday morning and he seemed fine. He left that morning to take a review course for a [university] final exam.

Later that day, we received a phone call from Scott. He was afraid and very confused. He was in Red Wing, Minnesota . . . had left his university that morning and driven eight hours. [He said] he was following the sun . . . [Scott was] later transferred to Winona Community [Memorial] Hospital. He had a psychotic episode and was diagnosed with Bi-Polar I Disorder.

Scott had been doing very well in college. He had taken out loans to finish his last two years of school . . . We did not see this coming . . . He had incredible care [at Community Memorial Hospital]. The doctors, nurses, social workers were amazing. They were professional, caring and supportive.

Yesterday Scott received the award determination from the Ben & Adith Miller Patient Care Fund. Words cannot express how grateful we are for the most caring and generous award determination on behalf of our son. Scott does not have mental health coverage on his insurance policy . . . The fund determination will enable Scott to move forward in his life . . .

We don’t know why Scott drove north to Minnesota. When he said he was following the sun, perhaps somehow he was guided to a very good place.

Religious services for all the senses

Recently the Winona Health Foundation used funds from the community to sponsor a new service for residents known as Godly Play.

This method of Christian education was originally intended to teach children about their religion. Winona Health chaplains Bill Flesch and Kathy Redig use Godly Play in special services with Lake Winona Manor (LWM) residents.

“Godly Play involves religious story-telling using visuals and wooden figurines that participants can pick up and handle,” Flesch explained. “It is especially effective for residents who have memory loss. Involving all the senses in the religious discussion helps trigger memories.”

Flesch enacted Advent Godly Play services for LWM residents in December. Betty Schmidt, a resident who taught Sunday school for 21 years, said, “It’s wonderful. I enjoy thinking about Christmases past when my husband and I would stay up all night decorating the tree and having everything ready for our children on Christmas morning.”

Making holidays bright

Winona Health Foundation’s Tree of Lights campaign makes the holidays brighter for families caring for loved ones with terminal illness. Last year, the campaign’s 17th, $50,789 was raised from 1,628 generous families and individuals.

Funds from the Tree of Lights allow families to receive Hospice services who may not otherwise be able to afford them. The donations also are used to purchase medical equipment needed by Hospice patients.

“The generosity from those who make donations to the Tree of Lights campaign have transformed the lives of Hospice patients and their families,” said Sheila Skeels, Hospice social worker.

Hospice provides end-of-life care. Staff and volunteers help patients and their families cope and adapt with their situations by providing physical, emotional, social and spiritual support.

Meeting diversity needs

Support from the Winona Health Foundation allowed Community Memorial Hospital’s Family Birth Center (FBC) to offer its first Spanish Prenatal Class in 2005. With the collaboration of Project FINE, Hispanic families were invited to attend the birthing class offered by FBC nurses. Project FINE provided transportation and an interpreter to assist with communication.

Twelve Hispanic families attended this two-day prenatal class and learned about pain control, prenatal care and the father’s role in delivery. Participants watched a Spanish birthing video and were given educational materials provided in Spanish.

“Having a baby can be an overwhelming experience for anyone,” said Tweety (Amparo) Oevering, RN and Staff Training Instructor for the FBC. “The goal of this class was to educate and inform our Hispanic community members that we are here, increase their awareness of the childbirth process and ensure they receive the best birthing experience.”

Patients benefit as Auxiliary puts the fun in fundraising

Winona Health’s Cardiac rehabilitation patients are among the many who benefit from the Winona Health Auxiliary. This year, proceeds from their Birthday Ball helped purchase a wireless telemetry system to allow cardiac rehab specialists to monitor a patient’s vital signs during exercise.

“It’s an extremely valuable assessment tool for patients who’ve recently had a heart attack or other surgery,” said Lynn Sprain, Winona Health’s cardiopulmonary rehabilitation coordinator.

In addition to the Birthday Ball, the Auxiliary presents the Follies, a variety show held every two to three years. Proceeds funded the new Patient Channel, a syndicated television service providing health-related programming to patients at Community Memorial Hospital and residents of Lake Winona, Adith Miller and Roger Metz manors.

“The Auxiliary has provided so much over the years. We appreciate the dedication and hard work of all our volunteers,” said Autumn Herber, volunteer services coordinator.
