What is DiabetesE?• Online whole system self assessment service for PCTs, Specialist Diabetes Providers and

Practices

• In 2009, 82% of PCTs undertook DiabetesE assessments

• Can be used to undertake a baseline assessment and drive continuous quality improvement

• All questions require Yes/No answers

• All questions evidence based (e.g. NICE guidance, Royal Colleges etc)

• All questions have electronic links to up to date evidence, policy and guidance

• Instant results on a single benchmarked (colour coded) dashboard

• Prioritised recommendations on all aspects of the diabetes service (up to 3 per module)

• Bespoke Action Plan containing top 5 priority recommendations from the entire assessment.


Changes to DiabetesE in 2010In response to feedback from PCTs and specialist services, from 26th July to 3rd September 2010 Innove will be piloting a new:

1.PCT questionnaire focusing on commissioning (3 modules):– Leadership, Policy and Strategy

– Productivity and Contracting

– Health Promotion and Prevention

2.Specialist provider questionnaire focusing on service provision and quality (4 compulsory modules (C) and 8 optional modules – the specialist provider selects the modules relevant to their service:

– Clinical Leadership (C) Staffing Levels (C) Staff Development (C) Patient Experience (C) – Children & Young People Adults with Diabetes Inpatients Prevention of Foot

Note: You do not have to complete all the modules during one login session. Individual modules can be completed at your own pace. You can choose to answer any number of questions in a module at any one time, however, you will only obtain a module score and benchmark colour on your results dashboard when you have submitted answers to all the questions in a module.

Each provider can select up to 8 optional modules (depending on the service they deliver) to create their service’s bespoke questionnaire.

For example, a specialist community provider would not select the Inpatient Care module, a specialist diabetes paediatric service would only select the Children and Young People’s module.

To access the questions for each module the user clicks the relevant module button e.g. ‘Adults with Diabetes’.

An example question page is shown on the next slide.

Each provider can select up to 8 optional modules (depending on the service they deliver) to create their service’s bespoke questionnaire.

Adults with Diabetes

If you do not enter and SUBMIT your answers and supporting evidence in this module within 2 HOURS you will be automatically timed out and your answers will not be saved.

You do not have to answer all the module questions during a login session. However, you will only obtain a module score when you have SUBMITTED answers to ALL the questions in a module.

1. Is there a published pathway, agreed with the PCT, for the management of adults with diabetes, covering early identification, initial management and continuing care?

2. Does the care pathway for the management of people with diabetes take into account the needs of hard to reach groups, for example those in secure estate, travellers, refugees, asylum seekers etc?

3. Does the care pathway for the management of people with diabetes include explicit guidance on referral from and discharge to primary care?

4. Does the care pathway for the management of people with diabetes include explicit guidance on coordinating care with other specialties eg kidney, foot, obstetric, ophthalmic, cardiovascular, care of the elderly teams?

5. Within one working week of a patient consultation, does the specialist diabetes team feed information back to practices e.g. changes to medication, investigation results etc?

This is an example of the ‘Adults with Diabetes’ questionnaire. All questions require a ‘Yes’ or ‘No’ answer. The user simply selects the appropriate check box.

Teams without Walls is an integrated model of care, where professionals from primary and secondary care work together in teams, across traditional health boundaries, to manage patients.

The Teams without Walls working party believes that an integrated model of care, where multi-professional teams work in a managed network across the interfaces and manage patients on a care pathway designed by local clinicians, is the best model for the future.

The ‘Action Plan’ is an interactive feature of DiabetesE. The user can enter details about the action the service intends to undertake to address each recommendation. It is essential to identify a lead person for every recommendation. The lead will be responsible for signing off the recommendation.

For each priority recommendation the user enters a summary of the action that is to be taken, a target date and the status of the action. A valid justifiable (auditable) reason must be given for postponing a recommendation. The completion date is filled in when the recommendation has been completed.

Once all the relevant fields have been filled in the user can save the ‘Action Plan’ to DiabetesE and save a PDF copy to their computer by clicking the ‘ Save and Print to PDF’ button.

The user can create a new action plan by clicking the ‘New Action Plan’ button. Any existing priority recommendations which still have a ‘No’ answer will appear in the new action plan.
