Application for

Nonimmigrant Visa. This

form is available online in

English with translation

into Spanish. Once filled

out, print the

"Confirmation Sheet"

with the barcode.

and CAS

4. Report to the CAS you

selected (Bogota or

Medellin) on the date and

time of your appointment,

for shooting and prints.
