We are seeking to recruit a Lead Colorectal Cancer Nurse Specialist This is an exciting opportunity for an experienced senior nurse You should be educated to nursing degree level. The post will be predominately Monday to Friday.You will have responsibility:

• To provide professional and clinical leadership in the speciality of Colorectal Nursing

• To ensure patients with suspected and confirmed Colorectal Cancer receive appropriate levels of support, education and advice while undergoing diagnosis and treatment

• To act as a clinical nurse specialist in the care of colorectal / stoma patients throughout patients journey from referral to discharge and on a continuing basis as appropriate

• To prevent re-admission to hospital by recognising colorectal/stoma/SACT related consequences and complications and working collaboratively with the AO/ Acute medical team to ensure an effective management plan with timeously communication with regional colleagues

• To provide direct psychological and informational support to patients with colorectal cancer and their families throughout the various stages of the pathway pre-diagnosis- follow up.

• To provide clear and consistent leadership to nursing staff working within the Colorectal Service.

This is an opportunity for an individual to apply for the post of either Physician Assistant or Advanced Nurse Practitioner with the Fife Neurorehabilitation Service based in the Sir George Sharp Unit at Cameron Hospital in Fife. This post is to cover a period of sabbatical leave. It would be suitable for someone to gain further experience as part of their career development or someone interested in gaining experience in the assessment and management of neurological conditions

You will join a highly motivated and skilled multidisciplinary team who provide integrated neurorehabilitation.The inpatient unit rehabilitation programme has been internationally accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF) since 2003 and seeks to provide a high standard of care and rehabilitation to people with a variety of neurological conditions with a particular emphasis on acquired brain injury (including stroke and TBI) and multiple sclerosis.

The Capacity & Flow Team is a dedicated team who provide highly skilled and professional support in directing patient flow across the acute division. Clinical site co-ordination supports patient flow to ensure timely bed availability, admission, discharge, transfer and appropriate placement of patients across Emergency and Planned Care Directorates, Scheduled and Unscheduled care and operates within the delivery of the 4 hour Emergency Access target.

This is an exciting and challenging role for an individual to make a significant contribution to the delivery of hospital site management. The clinical co-ordinator role is significant in the management of Acute beds and will work closely with primary care colleagues promoting patient flow to downstream hospitals as well as managing transfer to and from tertiary care.

You will be an experienced first level nurse with extensive post-registration experience with proven leadership experience and a focus on high standards of quality, patient centered care delivery.

This challenging post requires excellent interpersonal and decision making skills and you must be able to work autonomously and have refined and effective communication skills.

We are looking for an individual to join our Fife-wide team of Practice Education Facilitators (PEF) and Care Home Education Facilitator (CHEF). The Practice Education Facilitator role is crucial in influencing the quality of the clinical learning environment by providing support, educational input and development activities for mentors and nursing and midwifery students. The role aims to enhance the quality of the practice learning experience and positively contribute to the future nursing/midwifery workforce. This includes the development and maintenance of a positive learning environment within clinical settings, where both education and lifelong learning are seen as integral to clinical practice. This post requires an individual who can work as part of the existing team, as well as working autonomously and proactively. You will be required to be organised, flexible and have robust leadership and change management skills, for example in relation to performance review and support of mentor and students. You will have excellent interpersonal and communications skills working with a diverse range of staff and Higher Educational Institutions including representing Fife at national and regional groups.Seen as an experienced practitioner you will both lead and facilitate practice education developments (practice learning) to support staff to make the necessary changes in their practice as required by the profession and the service and in line with related standards. Working with local and national initiatives you will promote and support staff to develop using resources such as Flying Start and Effective Practitioner. You will be a registered nurse or midwife, have experience as a mentor as well as supporting development and education sessions.

The DMT MS service has been recently set up within Fife 18months ago. The service provides specialist advice, information, support and education for people with MS who would benefit from disease modifying treatments. Providing safe monitoring of these drugs. The service is a direct link to the rehabilitation service based at Cameron where a multi-disciplinary service is proved to people with MS. You will be an experienced first level registered nurse with extensive post registration experience. Evidence of ongoing professional development and be able to demonstrate relevant clinical experience. You should have proven experience and interest in MS (DMT) and multidisciplinary team management of long term conditions. An exciting opportunity has arisen in Victoria Hospital, Kirkcaldy for band 5 Registered Nurses to support, as part of the multidisciplinary team, the delivery of new models of care within 2 busy acute wards.

Ward 41 will open in early January 2018, initially until end of March 2018, caring for patients who require more complex discharge planning. The ward model will be supported by ‘Daily Dynamic Discharge’ methodology which has been developed by Scottish Government.

From early December 2017, ward 12 will increase bed numbers on a permanent basis from 16 to 23 beds and will continue to develop and embed the ‘Daily Dynamic Discharge’ methodology which it has recently introduced.

We require motivated Registered Nurses who are committed to multidisciplinary team working. As part of the multidisciplinary teams you will ensure the delivery of person-centred care and you will work closely with the Discharge Hub and other Health and Social Care colleagues to proactively manage the inpatient journey and discharge patients with appropriate support.

You should be able to work using your own initiative and have an active interest in mentoring students and junior staff. Venepuncture and cannulation skills are desirable but training can be provided. Good communication skills are essential.

Ward 41 will open in early January 2018, initially until end of March 2018, as a 16 bedded inpatient ward caring for patients who require more complex discharge planning. The ward model will be supported by ‘Daily Dynamic Discharge’ methodology which has been developed by Scottish Government. From early December 2017, ward 12 will increase bed numbers on a permanent basis from 16 to 23 beds and will continue to develop and embed the ‘Daily Dynamic Discharge’ methodology which it has recently introduced.

We require motivated Nursing Auxiliaries, committed to multidisciplinary team working. As part of the multidisciplinary teams you will ensure the delivery of person-centred care and you will work closely with the Discharge Hub and other Health and Social Care colleagues to support the inpatient journey and ensure patients are discharged with appropriate support. You should also have an active interest in developing further skills, e.g. Venepuncture, Cannulation and monitoring of patients observations, i.e. temperature, pulse and blood pressure.

Experience in the care of older patients is desirable but no essential as full training will be provided. You will have particular responsibility for:• Delivery of evidence based stop smoking interventions across Fife, ensuring that the work targets people/families living in deprived circumstances and specific groups in a range of settings for example communities, primary care, workplaces etc• Increasing engagement at community level within the most deprived communities and inequalities groups• Undertaking client assessments make appropriate pharmacotherapy decisions, ongoing client support and undertake 4 and 12 week follow-ups.

You will require to demonstrate effective engagement strategies and approaches with special interest and hard to reach groups that support referrals to services. You will have recent experience of delivering evidence based interventions. You will be required to demonstrate your understanding on Inequalities in Health and how this relates to tobacco smoking.

You should have a professional or academic qualification in community development, social care or Health or relevant experience in stop smoking support delivered on a one to one and group basis.

As an EM consultant group we believe strongly in providing a sustainable work/life balance. The job plan includes a PA for each on call. Clinical administration time has been factored in as per RCEM guidance. There is a 24 hour middle grade rota with no gaps.

We perform well in key areas such as sepsis, stroke thrombolysis and compliance with the 4 hour target. We have an excellent relationship and collaborate closely with all in-patient teams. There are also clinics for Adolescent Rheumatology and a monthly Young Adult Clinic. We have a weekly Virtual Clinic to discuss complex patients who may need biological therapies. There is a 6 monthly combined Ophthalmology / Rheumatology clinic. Day Unit facilities are at Queen Margaret Hospital, Dunfermline. There are close links with all Acute services within Victoria Hospital and Queen Margaret Hospital.

Experience in Rheumatological ultrasound is considered desirable.

The department of Trauma and Orthopedic Services in NHS Fife wishes to appoint a clinical development fellow (ST1/CT1 level). This is a 6 month, fixed term post, designed to allow the opportunity to build skills in the generality of surgery and with specific experience in Trauma and Orthopaedic Surgery. You will be allocated a mentor, who will offer you support, feedback and guidance during your first year in practice.

The Newly Qualified Practitioner Flying Start ‘Rolling Programme’ is offered throughout NHS Fife by the Practice & Professional Development Department (Nursing and Midwifery Practice Education Facilitators and the AHP Practice Education Lead). These 4 sessions offer you support and guidance about getting started and progression through Flying Start, as well as putting you in touch with other practitioners participating in the programme.

Muiredge surgery is a well-established practice situated on the Fife coast, 10 minutes’ drive from Kirkcaldy and approximately 30-35 minutes’ drive from Dundee and Edinburgh.

EXCITING SECONDMENT OPPORTUNITY

If you have well developed skills and experience in working with people who are experiencing homelessness, this could be an exciting career development opportunity for you to develop these skills within the community.

The NHS Fife Intervention Project is a highly innovative 18 month programme to test a new approach to providing services in NHS Scotland hospitals for those patients who are homeless. The project will be managed by Shelter Scotland and is supported by a multi-agency Steering Group led by Fife Council Housing Services and the Fife Health and Social Care Partnership. The key aims of this project are to:

improve working links between health and housing services provide homeless individuals with the right support to improve their health

outcomes reduce the re-admission rates among this cohort evaluate the preventative value of this approach in reducing pressures on

NHS Emergency Departments in Scotland.

The project will employ 2 staff and be run from Victoria NHS Hospital in Kirkcaldy, Fife. There will be close partnership working with hospital staff to engage patients who are identified as having insecure housing. Workers will follow a five-stage process to engage with the patient, undertake an immediate crisis response, work with them to identify key goals, put this into action focused on helping the person to develop skills and resilience, and finally support them to move on.

The project will focus on emergency admissions at VHK and Out of Hours. The target group will be individuals of any age and families who are homeless or at immediate risk of homelessness.  This project will be evaluated by Healthcare Improvement Scotland, who will work alongside the staff team in identifying outcomes and learning.

A key aim of this project is to share learning and build capacity with key stakeholders across Fife. Applications are thus being sought from staff in Fife Council Housing Services and NHS Fife who are interested in being seconded to staff the project. Key skills required include: needs assessment; experience of casework or care/support planning & delivery; experience of collaborative working. A significant programme of staff training and development will be a feature of this project.
