Don’t Miss Out!You have the opportunity to earn a wellness reward—and save a total of $260 in health insurance premiums in 2020! The Utz Wellness Rewards program is completely voluntary, you are not required to participate.

To receive the wellness reward, complete ALL of the following healthy actions and earn 300 points by September 30, 2019:

}} RealAge Test (100 points) }} Biometric screening (100 points) }} Preventive health exam (100 points)

Instructions on how to complete each step are included in this program guide.

Requirements for Associates enrolling in Medical Plan after January 1, 2019: For new associates or change of life enrollees, you will be required to complete the below healthy actions based on your medical plan enrollment date:

}} Prior to the start of the 2019 plan year to 3/30/19: Real Age test, Biometric Screening, Attest to Preventive Exam

}} From 4/1/19 to 5/31/19: Real Age Test and Biometric Screening

}} From 6/1/19 to 7/31/19: Real Age Test only

}} From 8/1/19 to 12/31/19: Automatically compliant

Required Healthy Actions

RealAge Test (provided by Sharecare)

The RealAge test is an interactive tool that assesses your current health status and gives recommendations on living a healthier life. Simply answer a few questions based on your current health status, diet, sleep, etc. and learn what your real age is.

You’ll take the RealAge test on the Sharecare website after you register for an account. If you prefer to take the test in Spanish, contact Highmark at 1-800-298-1967.

Register Today!You must register with ShareCare before taking the RealAge test.

How To Register with Sharecare

Go to mycare.sharecare.com

You can visit the website from a desktop or mobile device. If you are on a mobile device, you must access the site through a browser, not the Sharecare app.

During registration, you will be asked to provide:

}} Your first and last name

}} Your gender and date of birth

}} Your zip code

}} Your email address, along with a password

}} Your member ID number from your medical insurance card (enter numbers only; no spaces, no letters)

Once the information above is entered, click on ‘agree to terms and conditions’.

Access Sharecare 24/7!

After you register for an account online, you can download the ShareCare app from the Apple or Google Play app stores.

When prompted to register your app, use the email and password you used to create your Sharecare account to link the app to your newly created account.  It will automatically give you the opportunity to take the RealAge test, if you didn’t take the test when you registered, do so now!

Biometric Screening

Biometric screenings will be offered for free to all active, full-time team members. A covered biometric screening includes a blood test for cholesterol (total, LDL and HDL), triglycerides, glucose and additional measurements for blood pressure, height, weight, body mass index (BMI) and waist circumference.

You can complete your biometric screening in one of the following ways, depending on your work location:

}} Hanover area: If you are located near Hanover, schedule your biometric screening appointment at the Utz Health & Wellness Center. Any additional labs that your primary care physician orders during your annual exam should also be scheduled here in order to avoid extra charges. Call 717-969-8771.

}} Personal doctor: If you plan to complete your screening with your physician, you must have them fill out the 2019 Utz Physician Screening Form, which you can find on Sharecare or the Utz Intranet. Please submit it according to the instructions on the form. If you are unable to download the form, please contact Ginger Miller at 717-637-1759 ext. 1590.

}} On-site screening: On-site screenings will be available at select Utz manufacturing locations. Look for announcements in January on upcoming screening events.

}} Quest lab (option for team members outside of Hanover only!): You may visit a Quest Diagnostics Patient Service Center (PSC), which offers Blueprint for Wellness for biometrics. For instructions on mandatory registration and to find a lab near you, see the next page.

Quest Lab Registration

Go to My.QuestForHealth.com

}} If you’ve already established an account, use the Log In area to enter your username and password and select the green ‘Log In’ button.

}} If you’ve forgotten your login information, use the username link to retrieve your username or the password link to reset your password.

}} If you’ve never registered on the site to establish an account, use the Create Account area, enter the registration key - Utz2019, and select the ‘Register Now’ button and follow these steps:

}} Read through the Quest Diagnostics Terms and Conditions.

}} Scroll to the bottom and select the ‘Accept & Continue’ button.

}} Under Confirm Your Eligibility, enter your Unique ID and Date of Birth; then select the green ‘Continue’ button.

}} Your Unique ID is the first initial of your first name, your last name, and the last four digits of your Social Security number. For example John Smith SS # 123-45-6789 = user ID jsmith6789 (lower case).

}} Create a username and password to log in to your account; then select the green ‘Continue’ button.

}} The password must be at least eight characters long, include a number or special character, and include at least one uppercase and one lowercase letter.

}} Verify/complete all of the information under Enter Your Information and Mailing Address; then select the green ‘Save’ button.

}} Please note that an email address is required and will be used in a case where you need to retrieve your username or reset your password.

}} After registering, you will arrive at the dashboard.

Biometric Screening - Quest Lab Scheduling

If you have problems accessing the online scheduler or would like to make your appointment by phone, contact Quest at 1-855-623-9355.

Schedule an Appointment

Log in to your account at My.QuestForHealth.com

}} To schedule a screening at a Patient Service Center (PSC), in the Wellness Screening section, under Patient Service Center, select the green ‘Schedule a Screening’ button.

}} Select the Quest Diagnostics PSC location where you’d like to complete your screening; then select the ‘Continue’ button.

}} Note: to search for PSC locations in areas other than your home zip code, simply change the zip code.

}} The first available date will populate in the Date field; you can click the calendar icon to choose a different date.

}} Select the time you would like to complete your screening; then select the ‘Continue’ button.

}} Verify that all the appointment details shown on the screen are accurate; then select the ‘Confirm’ button.

}} Note: your appointment is not scheduled until you select the green ‘Confirm’ button.

}} Once your appointment has been scheduled you will arrive at the confirmation screen.

}} You can download your order requisition by selecting ‘Print Order Requisition’ on the left side of the screen or select the green ‘Back to Dashboard’ link to return to the dashboard.

}} On the dashboard, you will be able to see your scheduled appointment.

}} You can select the green ‘Download to Calendar’ link to add the appointment to your calendar.

}} If needed, you can also use the green buttons to cancel or reschedule your appointment.

}} Selecting the ‘Need more appointment information?’ link will take you back to the appointment. confirmation page.

Annual Preventive Health ExamRegular health exams can help detect potential problems early on, when they are easier—and less expensive—to treat. By getting regular preventive care, you boost your chances for a longer, healthier life.

You must complete an annual preventive health exam between October 1, 2018 and September 30, 2019 to qualify for the incentive.

You can complete your Annual Preventive Health Exam in one of the following ways, depending on your work location:

}} If you are near Hanover, you can schedule your exam at the Utz Health & Wellness Center in addition to your biometric screening (these do not have to be completed at same time).

}} If you are near the Golden Flake facility in Birmingham, Alabama you may schedule your exam during one of the Brookwood Baptist Mobile Clinic visits. For dates and to schedule, call Ashley Culliver at 205-458-7166.

}} Any team member can complete the exam at your primary care doctor’s office. If he or she submits the exam as “preventive,” the Utz Health Plan will cover the exam at 100 percent.

To receive the 100 points for completing your annual preventive exam, you MUST track your progress in the Sharecare app.

Sharecare App - Accessing Sharecare 24/7

After you register on the Sharecare website (see instructions on page 3) you can continue to access your wellness program and track your activities on the website or you can download the Sharecare app and access your program on your mobile device. Again, you must start by registering on the Sharecare website. Once you have registered, downloaded the app ( see instructions on page 3), and once you’re logged in, you can follow these steps to log your completion of each healthy action.

*Note that you can follow directions to track your activities through the website as well, but the look may be a slightly different.

From the App home page, tap Achieve.

Then tap in the Rewards area.

In the Rewards area, tap on Activities.

Select an Activity to track your participation.
