Our patients and their families experience

these seasons every day. As we help them navigate these new experiences, we too deal with the changing seasons as an

organization. Currently, we are preparing to hand off the South Carolina division to Agape Hospice. We have had the pleasure of serving South Carolina patients since 2016. Now we will be focusing our efforts on North Carolina care while Agape founder Scott Middleton and his capable team concentrate on caring for the patients in South Carolina. As a result of Agape’s intended expansion of the Smith-Phayer Hospice facility, more people in Spartanburg and it’s surrounding counties can be served. Hospice of the Carolina Foothills will ensure that proceeds from the sale will be designated for patients at the end of life in upstate South Carolina and Polk County. We will continue to care for patients in Polk County as well as operate the Thrift Barn. Our Columbus office remains open in Columbus Commons.

There are mergers and acquisitions in health care systems happening across the country. Just in the

counties we serve, hospitals and physician offices have come under new ownership, and with these changes, patients have different experiences. The HOCF sale of the South Carolina division to Agape will ensure we can focus our efforts on offering expanded services and care in Rutherford, Polk, McDowell and Henderson counties in North Carolina. We have spent the last few months reorganizing our processes, as well as determining how we offer palliative care. We are concentrating on seamless admissions and on breaking down as many barriers to care as we can. Our goal is to get to hospice patients within four hours of the referral. Very few hospices make that commitment to their communities. We also are striving to operate within the continual increased scrutiny and regulatory requirements. We

have no idea how the proposed new rules and Medicare Advantage plans will impact the way we provide care, but we do know that we are preparing for proposed changes by providing the highest quality care with the most value.

Just twelve years ago, the NC State Medical Facilities Plan indicated that Rutherford County had a need

for ten inpatient beds, so that is what we constructed. The plan today states there is only a need for five beds, because of the increased requirements to admit patients to inpatient facilities.

We began the palliative care program in 2004 based on needs expressed from our referral sources.

We have expanded the program over the years, and now we are restructuring it to better serve patients, while offering the service to as many people as possible. We are striving for accreditation in palliative care as we are accredited in hospice. We have added a part time social worker for patients at the highest accuity level. Social workers will also access spiritual care for patients when they need it. In order to be accredited, we will need to have 24-hour on call access for the highest accuity patients in the near future. Only the services of the palliative care nurse practitioner are billable, so this means that, although the service is much needed, it is by no means a break-even program. We currently have five full-time nurse practitioners serving approximately 250 patients.

Hospice of the Carolina Foothills is a nonprofit entity and depends on your memorial gifts, shopping at

the thrift stores, gifts from civic organizations, churches, estates, and grants to provide all of these programs and services not covered under Medicare, Medicaid or private insurance. So thank you for your support of Hospice of the Carolina Foothills in these ways.

We don’t know exactly where the future of health care will take us. What we do know is that we are

constantly evolving to ensure we are here for terminally ill patients and their families when they need it most. We will not rely on what worked in 1982 when we were founded, because health care and society changed. However, we will remain committed to why we were founded, and that is to take care of our communities.

Palliative care nurse practitioner visits are billed when appropriate under Medicare part B and com-mercial insurance. The program does not include access to the full team of hospice staff such as certi-fied nursing assistants, 24 hour vis-its if needed by a nurse, or the ser-vices of a volunteer. Medications, supplies and equipment are not provided under palliative care. So-cial work consultation and spiritual consultation may be provided under the program but are not billed. The care is provided regardless of the ability to pay. Christina said she was really go-ing to miss her hospice family when she was transferred to the palliative care program. “I’ve really gotten to know them and feel close to them,” she said. “But I know they will be there again if I need them and get worse.” Christina believes everyone should be open to hospice or pal-liative care. “I tell anyone I can that they should consider letting Hos-pice of the Carolina Foothills find out how they can help.”

For years, Dr. Gary Schafer, who is Christina’s doctor, has stated that many hospice patients he sees do get better because of the coordinat-ed care. He has said before that he is convinced patients live longer be-cause of the physical, emotional and spiritual needs are met. Although Christina remains on oxygen, she does not currently have a six month prognosis required to be a hospice patient. The palliative care program that is following her now provides some of the services hos-pice provides but not the full scope of care. Palliative care is for people who have life threatening illnesses, but who do not qualify for hospice care. Hospice care can be provided for Christina if she needs it again. Many of the 250 patients served by the Hospice of the Carolina Foot-hills Palliative Care program have stage 3 or 4 heart failure, stage 4 cancer, advanced dementia, oxygen dependent pulmonary disease, stage 4 or 5 kidney disease or advanced liver disease. The palliative care program provides the following ser-vices:• An initial consultation with a

member of the palliative care team

• Visits to where the patient lives by either nurse practitioners, nurses, or social workers

• Spiritual support accessed• Advance care planning (living

will, health care power of attor-ney, MOST forms)

• Medication reconciliation• Coordination with other spe-

This past Christmas season wasn’t very joyous for Christina Hardin and her family in Forest City. Christina was admitted to the Intensive Care Unit at Rutherford Regional Health System in severe respiratory distress and then transferred to the SECU Hospice of the Carolina Foothills Inpatient Unit on January 2. Chris-tina’s prognosis was grim when she entered the inpatient facility. To-day, however, she has progressed so much that she is a palliative care pa-tient rather than a hospice patient. “I have no doubts that Hospice saved me,” Christina said. “The whole team has been wonderful. They coordinated my medicines and got me back on my feet.” Chris-tina’s daughter-in-law Robin Hardin echoes these sentiments. “People don’t realize that Hospice is not a death sentence. Chris is a living

Still Making Unusual Art

HOCF Volunteer Wins Award

Many years ago, Paul Phillips, 88, of Tryon Estates and a current HOCF patient, had his coin collection stolen. He decided he wanted to collect something a little less conspicuous, so he started collecting match-book covers. Matchbook covers were created in 1889 and grew in popularity as a method of advertising. For more than 40 years, matchbook covers were the most popular form of advertising, so it was easy for Mr. Phil-lips to collect numerous, interesting covers. The retired furniture designer from Longview, North Carolina, grew his collection of matchbooks into the thousands. He decided to begin an additional hobby which he calls “Matchbook Cover Collage Art.” The picture to the left shows Mr. Phillips with just a few of the over 200 works of art he has created. He begins the process by locating a group of covers he finds interest-ing, he determines a theme, and then he finds a bowl, box, lamp, etc. to affix the matchbook covers. He me-ticulously cuts and glues and arranges the covers into a pattern and applies multiple layers of protective finish. Each piece Mr. Phillips creates is unique and covered entirely with pieces of matchbook covers. He numbers, signs and dates each one, but he never sells his pieces. He gifts them to family or displays them in the apart-ment where he currently lives. He has had a couple of

limited exhibits in art and folk galleries, but he prefers to enjoy them himself or with friends and family. Mr. Phil-lips has two projects currently in production, and he is thrilled that a grandson has decided to help him finish these projects and continue on with his art. Despite his health obstacles, Mr. Phillips continues to live life to the fullest.

On April 11 at the Volunteer Rutherford Awards Presentation at the Ruther-ford Senior Center, Hospice of the Carolina Foothills office volunteer Carolyn Goode was named the “Volunteer Rutherford Senior Volunteer of the Year.” The event is sponsored by the Rutherford County Commissioners, United Way and Volunteer Rutherford. Hospice of the Carolina Foothills nominated Carolyn for this distinction be-cause of her commitment to the operation of the organization’s office on Hudlow Road in Forest City. Since 2013, Carolyn has provided over 2,400 hours of her time working in the office. She scans, puts admission booklets together, and provides a variety of other important functions. “I don’t know what we would do without Carolyn,” Janelle Lodermeier, HOCF Volunteer Coordinator said. “She is really important to Hospice, and we appreciate everything she does.” In 2018, Carolyn gave 932 hours which almost the equivalent of a part-time job. Con-gratulations, and thanks Carolyn!

In honor of Welcome Home Vietnam Veterans Day, Hospice of the Carolina Foothills and the American Le-gion Post 48 in Chesnee hosted a veteran partnership panel discussion on March 29 in Chesnee. The purpose was to discuss the unique challenges faced by veterans at the end of life. Hospice of the Carolina Foothills staff and volun-teers have been collaborating with American Legions, veteran groups and veterans offices as part of the “We Honor Veterans” program. The program trains HOCF staff and volunteers to better care for hospice patients who have served their country. As part of the most recent effort to ensure needs are met, HOCF Vietnam veteran volunteers have been making beaded pins and presenting to veteran patients. HOCF Hosts Veteran Panel Discussion

There are So Many Ways to Help Volunteers are the key ingredient in making Hospice of the Carolina Foothills operate the way it does today. There are so many ways to help at HOCF. For ex-ample, the picture to the left shows RS Central ROTC students painting the arbor at the Carolina Event and Conference Center. Other students rebuilt flower beds at the inpatient unit. And in the picture at right, Pet Therapy volunteers John and Courtney Davis brought their standard poodle, Aslan, to visit Betty Gibbs in the Forest City SECU inpatient unit. Hospice has a place for you re-gardless of your time and talents. The list of volunteer opportunities is really endless. Some of the needs right now include but are not lim-ited to, people who enjoy providing companionship for patients, musi-cians who like to entertain at the in-patient unit or other facilities, meal time companions, eleventh hour, grounds, Resale Shop in our stores

in Landrum, Marion and Forest City, and pet therapy. Hospice train-ing is available at www.hocf.org so that you can complete the training around your busy schedule or participate in training quarterly in one of our offices. Day Camp Held in June Hospice of the Carolina Foothills held its first Kids Path® Day Camp at South Mountain Christian Camp on June 27. Hospice has held weekend camps since 1995, first under the name Camp Rainbows and then Camp Erin.™ After Camp Erin™ grant funding ran out due to a change with the Moyer Foundation and parents from previous camps stated they thought a day camp would be more effective, HOCF made the decision to operate a day camp this year. The camp is offered to children ages 6 to 17 who have lost a significant person in their lives. There were 24 children ages 6 to 13 who participated in this year’s camp. About 60% of the participants had no previous hospice experience. Those children lost loved ones due to a variety of circumstances. Trained volunteers and staff were on hand to ensure Kids Path® Day Camp participants learned how to appropriately deal with the grief associated with losing their important person. The camp is designed to provide therapeutic grief activities while enjoying a typical camping experience. Campers get to have fun playing games and getting to know each other. They also spend time making arts and crafts while learning about appropriate ways to express grief. The child at left made a memory box for her grandmother. Campers get to enjoy the wonderful South Mountain facilities including swimming and out-door time. This year’s camp featured a magician, as well as a musical therapy

activity. The children below left wrote letters to their loved ones sharing what they most enjoyed during their time together and what they miss most. They attached the letters to balloons and released them during a closing ceremony. The campers below right painted rocks to take home to remember their loved ones after camp. Hospice of the Carolina Foothills is one of the only hospice organizations offering the Kids Path® Day Camp. Kids Path® Day Camp is offered as the result of the generosity of individuals, churches and civic organizations in the community. There

is no cost to participants. Hospice of the Carolina Foot-hills also offers grief support groups within schools as part of the Kids Path® program. In addition to grief support, Kids Path® offers palliative and hospice care for children and teens. Festival of Trees 2019 Ornament Revealed

It’s never too early to think about Christmas! The 22nd Annual Festival of Trees will be held December 12 from 4 pm until 6 pm at the Carolina Event and Conference Center. All proceeds from this fundraising event ensure anyone can access hospice and palliative care regardless of the ability to pay. This year, the beautiful hand crafted ornament is an angel in memory of the angels who watch over us. Do-nors can purchase an ornament in memory or honor of a loved one. The loved one’s name will be added to a tag and attached to the gold ribbon. Average donation for each ornament has traditionally been $20, but anyone can participate in this event. Please give us a call at (828) 245-0095 to make sure you receive an order form.

Anyone Can Make a Hospice or Palliative Care Referral Did you know that anyone can make a referral to Hospice of the Carolina Foothills for hospice or pal-liative care? You can call (828) 245-0095 or (800) 218-CARE (2273) and ask how hospice or palliative care can help you or your loved one. With the potential patient’s permission, we can then call the primary care physician to determine if the patient is appropriate to receive ei-ther level of care, or if the physician will allow HOCF to consult with the patient to determine which level of care may be more appropriate. HOCF will set up an ap-pointment with the patient and family as soon as possi-ble. Hospice admissions are done within 24 hours, and palliative care admissions are done within a few days typically. The following are some times that it might be time to call HOCF:• When the person is given a limited life expectancy• Has frequent hospitalization and ER visits• Frequent MD contact• Frequent falls• More time in bed

• Unintentional weight loss• Weakness and fatigue are common• shortness of breath• Assistance needed with bathing, Etc. Hospice care is for people who are not seeking ag-gressive treatment and whose doctor has given them a six months or less prognosis according to the path of their disease. Many hospice patients live life to the full-est for much longer than that, but that is the regulatory criteria for hospice. A palliative care patient is either seeking aggres-sive treatment, the illness hasn’t yet progressed to a six month prognosis yet, or the serious illness has gotten better under hospice care and the patient is transferred to palliative care.
