From technology to geopolitics to global health issues, the world is different and ways of working have adapted to the changes. The HVO Board recently adopted a new stra- tegic framework, which will expand HVO’s efforts to utilize technology and better address new challenges in health care. HVO is very grateful that a generous dona- tion from Hansjörg Wyss, with the intent that HVO’s work will be expanded and magnified, will launch many of these new initiatives. Our mission, as always, is to “improve the availability and quality of health care through education, training, and professional devel- opment of the health workforce in re- source-scarce coun- tries.” This is an ex- citing time at HVO as we initiate pilot projects, develop new programming areas and new models for delivering our educational programs, and strengthen capacity to improve global health care. Expand HVO’s Global Reach Each year, more than 500 qualified health and medical professionals volunteer with HVO, providing mentoring, training, support, and opportunity to their col- leagues overseas. This is a model that has proven very effective and, with each person trained, there is a ripple effect on providers, hospitals, patients, and commu- nities. As a result, requests for HVO support continue to increase each year from around the globe. Over the next few years, HVO will expand its global reach by increasing the number of volunteers and by utilizing new and different tools for delivering edu- cational programming. In some project sites, volunteers have returned home and set up e-learning platforms, to continue training via teleconferences, Skype, etc. In Vietnam, a pilot project started in 2013 incorporates “blended learning” (a combi- nation of online delivery of educational content, as well as face-to-face interaction with teachers). This project is based on a 28-month internationally recognized cur- riculum to educate Vietnamese dentists in basic and clinical sciences in orthodontia. “This is an exciting time at HVO as we initiate pilot projects, develop new programming areas and new models for delivering our educational programs, and strengthen capacity to improve global health care.” continued on page 8

June Brady, MD, MPH, DTM&H,washonoredby theAmericanAcademyofPediatricsSectiononInternationalandChildHealthwiththeirHillman-Ol-ness Award at the national conference in October2014. Dr. Brady began her career in neonatologyand extended her outreach to international health.The Hillman-OlnessAward for “LifetimeAchieve-ments inGlobalHealth”honoredDr.Brady forher“outstanding contribution tomedical education andpediatrictrainingprograms.”Dr.Bradyhaslecturedfrequently in The Gambia, Kenya, Botswana, andUganda. For five years, she served as Chair of thePostgraduateTrainingProgram inPaediatricsat theUniversityofZimbabwe.There,sheandherhusband,Dr.GeorgeHyde, apediatric surgeon,partneredonthedevelopmentofapediatricsurgeryprogram.De-scribedasa“consummateclinicianandteacher”,“aninspiring rolemodel” and someonewho“embodiesthe spirit of international health” through herworkandthroughthepersonalrelationshipswithlocalstaff

The American Society of Anesthesiologists hon-oredLena Dohlman, MD, MPHwiththeNicholasM.Greene,MDAwardforOutstandingHumanitari-anContributionattheirOctober2014annualconfer-ence.Dr.Dohlmanhas focusedmuchof her careeroneducation,intheUSandinternationally.Shehasprovidedover27yearsof service toHVO,andhasserved in almost every capacity open toHVO vol-unteers.Herninevolunteer assignmentshave takenhertoMalawi,St.Lucia,SouthAfricaandVietnam.From2009-2012, she served as project director fortheSouthAfricaAnesthesiaProject, and shewas amemberoftheHVOAnesthesiaSteeringCommitteefrom 1997-2012, serving as committee chair from1997-2008. In 2007,Dr. Dohlman receivedHVO’sGolden Apple Award for her work designing anddeliveringprimarytraumacoursesaspartofHVO’sanesthesiaprojectinVietnam.Sheinitiatedandpro-motedastrongpartnershipbetweenHVOandtheSo-cietyforEducationinAnesthesia(SEA), tosupport

TheAmericanCollegeofDentistspresentedArthur Ingram Hazlewood, DDS, MPH with theWilliamJohnGiesAwardat theirOctober2014conference.TheawardisthehighesthonortheCollegebestowsuponFellowsandDr.Hazlewoodwasrecognized“forhistrulyexceptionalleadershipandaccomplishmentsindentistry,dentalpublichealth,education,human-itarianism,caretotheunderserved,dentaladvocacy,andglobaloutreach.”Dr.Hazlewoodhasservedonnumerousdentaltaskforcesandhasbeenaprominentmemberof thedentalcommunity inNewYorkcityand state.Hehas provided consulting and advisoryservices inCanada,Barbados,Peru, andChina andhasworkedwiththeUNDevelopmentProgramandNIH.AnHVOmember since1998,Dr.HazlewoodhasservedinGuyana,conductedsiteassessmentsinGhanaandHaiti,andservedon theHVODentistrySteeringCommittee1998-2001. Janna Beling, PhD, PT was recently presentedwith the Medal of Honor by the Vietnamese gov-ernment’sMinistryofLaborandSocialAffairs.Theministry,whichoverseesrehabilitativecareandveter-

Established in 2013, theawardhonors“anindividualfortheirexemplarylo-cal,national,orinternationalvolunteerandoutreacheffortsinwomen’scancercare,research,ortraining.”Dr.Creasmanwashonoredforhisleadershipinteach-ing gynecologic oncology to residents inHondurasandAfrica,andforhisserviceonHVO’sOncologySteeringCommittee.In2013,Dr.CreasmanreceivedHVO’sGoldenAppleAwardforhiscommitmenttoeducation. Linus Chuang, MD, MPH, MSwashonoredwiththeHarryLong,MDAwardbytheSocietyofGyne-cologicOncologyat theirannualmeetingonWom-en’sCancer.Theawardhonorscontributionstomul-tidisciplinary mentorship, collegiality, and teachingin the field of gynecologic oncology. Dr. Chuang’scommitments to educationhave takenhim toHVOprojectsinHonduras,Vietnam,andParaguayandheservesontheOncologySteeringCommittee.HewashonoredwithHVO’s2012GoldenAppleAward. TheAmericanDentalAssociation has announcedthatFrancis G. Serio, DMD, MS,willreceivetheirhighesthonor,theHumanitarianAward,attheannu-almeeting inNovember.The award honors dentistmembers “who have distinguished themselves byoutstanding, unselfish leadership and at least a 10-year commitment to their fellow human beings inthe field of dentistry” in underserved populations.Dr.Serioisbeinghonoredforoverthreedecadesofservice.AninitialtriptotheDominicanRepublictoprovidedentalcareinaremoteregionledtothede-velopmentoftheDominicanDentalMissionProject,towhichhehasdevotedhimselfsince1982.Dr.Se-riohasledstudentsandotherdentistseachsummerfor33years toprovidedental care servicesaswellasdentalandmedicalsuppliesforthelocalhospitaland some60,000 residents in a very rural area.Dr.Serio’scommitmenttoimprovingglobaloralhealthiswellknowntoHVO.HehasvolunteeredinHaiti,Guyana,andJamaica;hehasconductedsiteassess-ments,servedontheOralHealthSteeringCommitteefornineyears,andhasservedasProjectDirectorforHaiti. The orthopaedic community recently lost a gi-antwhenDr.EugeneBleckpassedaway.Heisbestknownastheauthorofthesemi-nalOrthopaedic Management in Cerebral Palsy andwasaworldauthorityonthesubject.AdevoutJesuitandNavyveteran,hetookhis training at Duke UniversityasafellowoftheNationalFoun-dationforInfantileParalysis.HewentontoestablishthepediatricorthopaedicprogramatStanfordandwasafoundingmemberofthePediatricOrtho-paedicSocietyofNorthAmerica. Dr.Bleck traveled extensively teachingorthopae-dics.HevisitedclinicsandtaughtinSoutheastAsia,Europe, and South America and was an honorarymember of the IndonesianOrthopaedicSociety.Heleft a legacy through his work and advocacy for apragmatic, child-centered approach to cerebral pal-sywhich has affected the care of disabled childrenaroundtheworld. Inhis lateryears,Dr.Bleckremainedinvolvedinthe Stanford Orthopaedic Surgery Department asProfessor Emeritus and self-styled court jester. Heremainedgenerousinsharinghisideas,whichwereoften fascinatingly contrarian and always imbuedwithcompassionandgoodsense.His residentsandcolleaguesrememberhimasamanofremarkablein-telligence,grace,andwit.

HVO and Bhutan to Partner on Residency TrainingIn January, HVO signed a Memorandum of Un-

derstandingwiththenewlyestablishedUniversityofMedicalSciencesofBhutan.Overthenextfiveyears,HVOhas agreed to assistwith training residents inthefieldsofophthalmology,generalmedicine,gen-eral surgery, pediatrics, obstetrics/gynecology, andanesthesiology.Inaddition,trainingwillbeprovidedfornursesinthenew2-yearBSc.Programfornurseanesthesia.The intention is for those trained to be-comestaffand/or facultyofeachrespectivedepart-ment.HVOhasbeenworkingwithBhutansincethelate

1980’swhenDr.RobertSteinexploredtheneedfororthopaedic training in a countrywhere therewerenoorthopaedicphysicians.Hiscommitmentledtotheestablishment of an HVO orthopaedic project. Thesuccess of that project led to otherHVO speciality

Recognition for Belize Nurse Anesthesia StudentsSuzanneBrown,CRNA, recentlysent thisupdate

ontheBelizenurseanesthesiaproject.Belize,acountryofabout350,000permanentres-

idents, has four regional hospitals and one referralhospital.However,therearenoon-goingeducation-alprogramstotrainnurseanesthetists.TheMinistryofHealthrequestedassistancefromHVOand,since2000,HVOhastrainedtwocohorts.Clinicalsitesforstudentsareratherlimited,soithasbeenratherchal-lengingattimestoworkwith sixteen students infouroperatingrooms.On the 11th of Janu-

ary, a Recognition andAwardsdinnerwasheldfor the Belize NurseAnesthesia students.Seven students wererecognized for theircompletion of the ma-jorportionsoftheNurseAnesthesiaprogram.Allseven of the studentshave completed theiracademic requirementsand successfully taken the certification exam. Thestudents have additional clinical case requirementsthatwillbecompletedinthenextfewmonths.Theirprogram recognized the clinical constraints and be-ganwithastratifiedcaseloadrequirement.Studentshadabasicnumberofcasesrequiredforgraduation,and an additional number of cases required before

theycouldbeginworkingaloneasgraduates.Therewerethreestudentswhowereofferedsome

additional assistancewith academics and those stu-dents are anticipated to complete their program inJune. Theywill then take the certification exam inJune. It is anticipated that the program will finishwith ten students completing the NurseAnesthesiaProgram. The University of Belize will be award-ingall thestudentsapostbaccalaureatedegree thatwillallowthemtobelicensedasCertifiedRegisteredNursesinBelize.Originallythereweresixteenstudentsadmittedto

the program. The University selected the students,andHVObecameinvolvedshortlyaftertheselectionprocess.Notalloftheselectedstudentshadabach-elor’s degree upon admission, and provisions weremadebytheUniversityforallstudentstoobtaintheirbachelor’sdegreeduringthispostbaccalaureatepro-gram.LeciaBevans,CRNA, and a graduate of the first

HVOprograminBelize,joinedtheprograminSep-tember of 2013 as theUniversity of Belize faculty

member. The programwas coordinated bySuzanne Brown, Kate(Durkan)HammondandRichard Henker. KateHammond was able toutilize this project asa part of her capstoneproject, and was ableto bring some fundingfrom the AANA Foun-dation for an electronictracking/evaluation pro-gram (Typhon) thatwasextremely valuable incoordinationofaninter-nationalprogram.

ConsistentcommunicationutilizingSkype,e-mailandeveninternetteachingallwereveryvaluableinthecompletionofthisprogram.Numerousvolunteersdonatedtheirtimeandtalentstotheeducationofthisgroupof studentswhowillutilize their skills to in-creasethelevelofanesthesiacareinBelize.

Theprojectincludescomputer-basedinstruction,su-pervisedpatientcare,on-linementoringbyorthodon-tists,andin-personinstruction.Oncologyvolunteershave set up long-distance monthly tumor boards.HVO is lookingatways toexpandon this,usingaglobalpanelofexpertswhoconsultandcollaborate.Suchinnovativeprojectswillnotreplacetheface-to-face training opportunities butwill be used to aug-mentvolunteerserviceandstrengthentheexchange.

Expand HVO’s In-country Outreach HVOalso is expanding its outreachwithin coun-tries. In many resource-scarce countries, resourcesand infrastructure tend tobeconcentrated inoneortwomajor cities.As a result,most ofHVO’s proj-ectshavebeen locatedat large teaching institutionsintheseurbancenters.Inrecentyears,HVOhasde-veloped partnerships withsmaller institutes locatedaway from the larger cities.These partnerships havestrengthened educationalresourcesandprovidedpro-fessional development op-portunities toareas inneed.HVO’sprojectsthataddresstheneedsofpopulationsfarfrom the capital cities notonlyimproveaccesstoqual-ity health care, but developthe triageand referralcapa-bilitiesoftheseinstitutes.

Expand HVO’s Programming Areas To address the changing needs of global healthcare, HVO intends to expand its programming inseveral different areas: trauma care and rehabilita-tion,women’shealth,andhospitaladministrationandmanagement.Theincidenceoftraumahasincreasedsignificantlyoverthelastdecade,asaresultofroadtrafficaccidents,civilunrest,andwar.Over5millionpeopledieeachyearfrominjuriesand90%ofthosedeaths occur in low- andmiddle-income countries.HVOcurrentlyhasprojectsin20countriesaddress-

ingtraumacarebutmanylackarehabilitationcom-ponent.Given the increasingneed for rehabilitationservicesasaresultofroadtrafficaccidents,violenceandchronicdiseases(suchasdiabetes,heartdiseaseandcancer),HVOisexploringnewmodelsforcom-prehensive programs that incorporate both surgicalandrehabilitationcomponents. Expanded efforts inwomen’s health carewill fo-cusontwomajorareas- trainingincervicalcancerscreening and treatment, and safe anesthesia in ob-stetric care.Cervical cancer is the thirdmost com-moncancerglobally,with80%ofcasesoccurringinresource-scarcecountries.Inthosecountries,itistheleading cause of death fromcancer amongwomen,resultinginsome190,000deathseachyear.Trainingin cervical cancer screening, treatment, and patienteducationcan lead toearlydetectionandtreatment,

with a greater potential forcure. Eachyear,hundredsof thousands ofwomen dieduringpregnancyandchild-birth, and the majority ofdeathsoccurinlow-resourcesettings. Most of thosedeathscouldhavebeenpre-vented if skilled providerswere present and availableto initiate emergency ob-stetric care. HVO projectsinbothEthiopiaandBelizehave demonstrated decreas-

es in the death ratewith training in safe anesthesiacare.HVOplanstoimproveandexpanditstrainingforanesthesiacareprovidersinresource-scarcecoun-tries. Anewareaoffocuswillbehospitaladministrationandmanagement.TherecentEbolacrisishighlightedthefragilestateofthehealthsectorinfrastructureinLiberiaandSierraLeone.Investinginthetrainingofhospital personnel to strengthen administrative andmanagement capacity is a universal need. Reflections from the FieldVOLUNTEERS REPORT ON THEIR ASSIGNMENTS

Dr. Dean Laochamroonvorapongse, MD, MPH ∙ Anesthesia ∙ VietnamIalsogaveaPALSlectureto70+nursesbecauseIwastoldthattheywereinterestedinpediatricresuscitation.TherestofthetimeIspenttherewasencouraging,supervising,andperformingultra-soundguidedperipheralnerveblocks.Theyareveryskilledtechnicallyandjustneededourreas-suranceandaffirmationthattheywereperformingtheseblockscorrectly.Attheendoftwoweeks,Isawthemtakingtheinitiativetousetheultrasoundindependentlyandhopethistrendcontinues.

Brett Fink, MD ∙ Orthopaedics ∙ NicaraguaIexpectedtoteachmuchofwhatIknowaboutfootandankleprocedurestothesurgeonsthere,butIfoundmytraumaknowledgetobemuchmorehelpful.Ilearnedmostlyhowagroupofextremelyintelligentandresourcefulsurgeonstherecanmakedo,andprovidereasonablecareatanegligiblecost.Itwasatrulyremarkableexperience.

Stephanie Windler, PT, DPT, NCS ∙ Physical Therapy ∙ MalawiMy fondest teachingmemory at the rehab hospitalwaswith a patient namedKingsley.Kings-ley is a22yearoldmanwithaT11complete spinalcordinjury.Dayafterday,Inoticedhimcometothegym.Hewouldstartwithhisupperbodyexerciserou-tine,performsittingbalanceexercisesandstandintheparallelbarswithhislegsplints.Inspeakingwiththestudentwhowastreatinghim,Irealizedthatthether-apistsandstudentswerenotaddressinghiswheelchairmobilityskills.Inoneweek,wetaughthimhowtodoawheelie,navigatecurbsandramps,outdoorterrainandcompleteafloortransfer.Kingsleywassohappyandthankfultohavelearnedthesenewtechniquesandfeltpreparedtogetbackintothecommunity.Thestudentsweresohappytohavetaughthimtheseskills.
