It’s hard to believe that it is yet again time for back to school for the kids. With schools being the hotbed of germs that they are, I have been thinking a lot lately about the word immunotherapy. This may or may not be a new thought; my guess is it’s so deep in your chiropractic brain you didn’t think to let it out! Immunotherapy is a treatment designed to assist the body to heal itself. I guess you can refer to eating more greens, vegetables, fruit and less fast food as a more natural form of immunotherapy. Allergy shots are referred to as immunotherapy. Weakened TB – yes, Tuberculosis – can be used to treat bladder cancer as a form of chemotherapy referred to as immunotherapy. In this therapy, the bladder recognizes the toxin TB, and the WBCs attack, full speed ahead and fights the cancer. I think what we do every day when treating and touching patients can also be referred to as immunotherapy. You know why, and it’s a nice extra benefit that we provide as a bonus with the adjustment. Chiropractic helps the body that was designed to heal itself to do just that: heal itself. I would refer to this as another natural form of immunotherapy. I think our patients need to know that we affect this system with every chiropractic treatment we give! I am not sure if or how much we need to touch the thoracic region, but many people like their back touched and rubbed and maybe this is why. There’s so much we don’t know and then again, so much that we do! Reminder note … no matter how often someone gets chiropractic manipulative therapy, it is still the most effective, most natural, non-invasive, liver- and kidney-friendly pain relieving treatment avail- able for most of the world’s population! The spinal manipulation treatment protocol that isubiquitous and somewhat unique amongst thechiropractic profession mandates that carefulevaluation of spinal stability be performed. Themedian atlanto-axial joint is a target site for manyof the etiologies of spinal instability and, for thatreason, is the topic of discussion.

The median atlanto-axial joint is a complex regionof anatomy that needs to be assessed during theroutine evaluation of cervical spine images. Thisjoint consists of the odontoid process (dens), theposterior aspect of the anterior arch of the atlas,associated ligaments and synovial tissue. It is thisanatomy that predisposes this region to instability.The transverse ligament tightly holds the odontoidprocess against the posterior portion of the

anterior arch thereby establishing a potential siteof weakness in cases of trauma. Interposedbetween the anterior surface of the odontoid andthe anterior arch of the atlas is a synovialmembrane and interposed between the posteriorsurface of the odontoid process and thetransverse ligament is another synovialmembrane. The synovial anatomy creates thepotential for erosion of the osseous structuresand/or ligaments, and therefore instability of thejoint, in cases of inflammatory arthropathies orother causes of soft tissue inflammation.

However, despite the atlanto-axial joint complexity,the determination of stability is relatively simple.The space between the anterior surface of theodontoid process and the posterior portion of theanterior arch of the atlas is called the anterioratlanto-dens interval or ADI. the cervical spine, taken at a 72 inch focal filmdistance, a space that exceeds 3.5mm in the adultor 5.0 mm in a child in either the neutral positionor in flexion indicates instability. Instability isalmost always associated with fracture of theodontoid whereas isolated instability withoutfracture indicates an abnormal transverse ligamentrelated to trauma, inflammation or congenitalanomaly. This includes rheumatoid arthritis,ankylosing spondylitis, tonsillitis and pharyngitisthat leads to ligament laxity due to hyperemia. Asa matter of fact many of the inflammatoryarthropathies are capable of causing instability.Within the congenital category, Down syndrome isoften associated with an unstable atlanto-axialjoint.

So, in practical terms, what does this mean? Priorto manipulation of the cervical spine, a patient witha history of cervical spine trauma, known orsuspected inflammatory arthropathy, infection or

certain congenital anomalies, should be screenedwith a lateral view of the cervical spine in theneutral position and in flexion to evaluate the ADI.Routine radiographic examination without flexionviews may not be sufficient for evaluating the ADIprior to manipulative therapy. An important caveat;do not perform the lateral view in flexion until theneutral lateral is screened.

Keep an eye on the ADI if you want to improvepatient care, minimize your liability, promote asuccessful practice and enhance your professionalstanding in the health care system.

I wanted to take just a little bit of space in the newsletter

this month to talk about an organization that I just found

out about at the Orlando National Convention. It is called

The Patriot Project. Ret. Staff Sgt. Shilo Harris was at the

convention and was representing them. Standard Process

was again one of the supporters and vendors at the FCA

convention and was promoting and donating to this great

organization. They will be donating up to $5,000. At the

Standard Process booth, they had a stationary bike with an

A little be about The Patriot Project…it was founded by

Timothy P. Novelli, D.C. The Patriot Project is a grass roots

movement to provide chiropractic care to ALL ACTIVE

military, their families, wounded warriors and all gold star

dependents at no charge.

donations, but chiropractors all over the county to help with

this mission. Their goal is “enlist” 6,000 chiropractors by
