My team are currently working with the Council’s Adult Social Care
Service to look at how people first ask for a social care
assessment
Earlier this year a new service went live – the Adult Early
Intervention Service & my team are leading a 6 month review of the
service
Plus, we are looking at creating a new online tool for people to
use…
…and, this is where we would like your help!

We are working on a new
Initial Information Form
The form will be used to capture key
information from people looking for care &
support for themselves or someone they care for
The Council will then use this to contact the
individual for a full assessment – if the needs
of the individuals mean that they are eligible for
an assessment
It is not replacing the current telephone system
Instead, it is designed to offer another method
for people to ask for an assessment
