Temple Claim Form

Claim Form #1-6

#1: Fill in today's date#2: All homeowners who are on the title of the home. #3: Fill in your address.#4: Check single family home.#5: Leave blank if you live at the home you are filing a claim for. #6: Provide one telephone number.

Claim Form #7-10

#7: You need to write: edge checking, thickness swell, swelling around nail heads, waxing, bleeding, and buckling.

#8: Write down the year your home was built. #9: You need to check yes and put down that your home was painted at

least 5-6 years ago.

Claim Form #11

If you have horizontals boards ( which is called lap and usually located on the front of your home) you need to fill #11 out. If you do not have horizontal boards go to the next question which is #12.

#11: A: Measure from the seam at the top to the bottom edge of the board and write the distance

in letter B. C: Write 6 inches. D: Write any wall that has the shortest

distance from the bottom of the sidingto the ground

Claim Form #12

If you have vertical boards (which is called panel and usually located on the sides of your home and the back) you need to fill out #12 if not then go ahead and move to question #12

#12: A: Measure the nails at the bottom of the board and put distance on letter B. C: Do the same as you did for letter A but find a place in the middle of the board. F: Put 6 inches.

Claim Form #13

Here you need to measure each side of your home. Panel is located on the two sides and back of your home and are usually 4 x 9.

Claim Form #14

You need to provide 10 pictures of your house. They need to be 4x6 when you print them off. If you need help with taking the pictures we can send out a contractor to take them for you for 48.

Claim Form #15

You need to provide a 16inch by 16inch sample cut to give proof that you have Temple Inland. If you do not want to cut the sample yourself you can call our office and we can set an appointment to have a contractor come out and do it for you for $10.
