Replaceable parts, such as springs are guaranteed for one year.2. This guarantee is void if instruments are altered or not maintained or repaired properly or if they are not used for their intended surgical purpose.3. Any UNUSED and UNOPENED instrument may be returned in new, packaged and unused condition for full credit within 90 days of order date.If an opened, unused instrument is found to be defective, it must be be accompanied by our certificate of sterilization prior to returning it to VOI or its subsidiaries. All used instruments must be sterilized prior to shipping and credit will only be issued if product is deemed defective.

Returns will only be granted and authorized by your VOI customer account specialist by calling 904-436-6540.The issue of an RMA number should not be interpreted as a final credit to your customer account. Just like VOI, Integra-Miltex reserves the right to evaluate incoming returns prior to issuing any customer credit.Product must be returned UNOPENED, UNUSED and in its original packaging with no additional labels or materials added to the package. Integ-ra-Miltex has a 25% restocking charge for product returned 90 to 120 days after receipt of shipment. If a product has been opened but found defective, or in need of repair or evaluation, a Certificate of Sterilization MUST ACCOMPANY your return. Used or not, this ensures the instrument you are returning has not been in a surgical situation or in contact with blood or other biological contami-nants that could pose a danger to yourself or VOI.Restrictions:

• NO returns will be accepted beginning 120 days after receipt of shipment. • Non-Sterile products which have been opened, or returned out of the original packaging or are returned after the expiration date, will not be

credited.• Discontinued products, private labeled products, custom made products, custom etched products and neutralized products are not returnable.
