WHAT IS THE UNICEF SUPPLY COMMUNITY?The UNICEF supply function has staff working in 158 Country and Regional Offices worldwide, as well as the Supply HQ in Copenhagen, Denmark - which also oversees a supply section in New York and warehouse hubs in Dubai, Panama and Shanghai. Supply staff manage forecasting, contracting, technical specification, quality assur-ance, logistics and supply reporting functions within UNICEF.

Supply Community staff work with their programme colleagues, govern-ment counterparts and United Nations (UN) and non-governmental organization (NGO) partners to address the supply needs of children. This large network of experts share a common interest in the success of the UNICEF supply chain. With new links being forged with the private sector and universities, the network continues to grow, giving UNICEF access to the wide range of expertise needed to fulfil our commitments to children - in what is an increasingly complex global supply environment.

UNICEF provides a range of essential supplies for children, including antiretrovirals, bednets, vaccines, therapeutic food, school materials and water equipment, to name just a few. As each type is unique, it requires a different approach to procurement, including forecasting, shipping mode, documentation, customs processing, handling, storage conditions, type of transport and distribution methodology. Upstream activities (assessment, procurement, shipping) need to be planned seamlessly into the downstream activities (customs, storage, distribution). Integrated solu-tions, where different product types use the same

transport, infrastructure and services, require close coordination - considering different geo-graphical supply sources, production lead times, shipping schedules and arrival times required for programme implementation. These individual supply chains, where possible, need to be consoli-dated to avoid duplication and ensure the most effective and efficient use of limited resources.

Getting all this right, down to the last mile and to every child in need, is the mission of the UNICEF Supply Community.

essential commodities. Over 12 million children went back to school in 2007 with UNICEF educational supplies. In addition, over $11 million worth of in-kind assistance (including

for instance a major donation of Vitamin A from the Canadian Government) was managed, as well as $64 million worth of international freight. Some $671 mil-lion worth of procurement services were provided to governments and other partners, leveraging further supplies to children in need.

Due to our programmatic focus, UNICEF is expert in several areas that are key to implementing the MDGs. As examples, vaccine security, the safe delivery of HIV/AIDS-related products including antiretroviral medicines, back-to-school educational and recreational supplies, the introduction of long-lasting bednets, the transition to the use of artemisinn-based combination therapies (ACTs) for treatment of malaria - all these demand that UNICEF maintains a cutting-edge expertise in procurement for children.

The delivery of strategic supplies directly supports the MDGs and is the cornerstone of the work of the UNICEF Supply Community. Essential supplies cut across all the UNICEF focus areas: young child survival and develop-ment, basic education and gender equality, HIV/AIDS and child protection – they save children’s lives, help them grow and protect their rights.

SUMMARY The Millennium Development Goals (MDGs) can only be met at the beneficiary level. The timely provision to children and mothers of quality products at afford-able prices is a vital component of the drive to meet the Goals. The needs are identified on the ground by UNICEF Country Offices worldwide, coordinated with governments and procured locally, regionally or interna-tionally - with international quality standards and best prices ensured. The goods are delivered via UNICEF Country Offices for handover to gov-ernments and other partners. Finally, the in-country logistics and delivery of the goods all the way to the end user, the child or the mother, are strengthened and monitored and information is fed back into the procurement cycle.

In 2007, UNICEF procured over $1.4 billion worth of supplies (an increase of 15 per cent over 2006) of which nearly $1.1 billion was international procure-ment. The Organization supplied vaccines reaching 55 per cent of the world’s children. UNICEF also procured antiretroviral medicines for 56 countries, and remained one of the world’s largest buyers of mosquito nets and oral rehydration salts. UNICEF assisted 62 countries to respond to emergencies through the urgent provision of

YOUNG CHILD SURVIVAL AND DEVELOPMENT Child survival supplies constitute the bulk of our trans-actions. Supply professionals in HQ and field locations collaborate in the procurement and delivery of strategic products to reduce child mortality and support early childhood development.

ImmunizationTo achieve MDG 4 (of reducing under 5 mortality by two thirds by 2015) immunization activities must be scaled up, particularly in sub-Saharan Africa and South East Asia. UNICEF plays a critical role in vaccine supply in these regions.

The WHO/UNICEF Global Immunization Vision and Strategy (GIVS) seeks to help achieve MDG 4 in three key ways: by boosting routine coverage, by scaling up campaigns (including multiple inter-ventions) and by introducing new vaccines. Record high levels of vaccines were procured in 2007 (3.2 billion doses with a value of $617 million), supporting all three GIVS focus areas.

Routine coverage in support of global polio eradication through the supply of Oral Polio Vaccine (OPV) continues to be high. In 2007, the specially developed monovalent OPV type 1 led to a 90 per cent decrease of the global type 1 virus cases - including interrupting wild polio virus transmission for the second time

in one of the most challenging supply chain environments of the world - Somalia.

NEEDS DEFINITIONUNICEF Pakistan and Supply Division worked closely with the Measles Partnership and the Government of Pakistan in defining needs and forecasting/procurement for five phases of the country’s planned national mass campaign against measles. UNICEF was subsequently commended for securing and delivering 64 million doses of vaccine in 2007, despite a range of country-specific constraints.


In 2007, it became clear that the 2010 goal of reduc-ing the number of measles deaths in Africa by 90 per cent of 2000 figures had already been met – four years ahead of schedule. Using UNICEF procured vaccines, the Measles Partnership supported campaigns in 25 countries in Africa and 26 in Asia, delivering 234 mil-lion doses of measles-containing vaccines bundled with safe injection devices.

The procurement of Global Alliance for Vaccines and Immunization (GAVI)-funded supplies rose by 76 per cent to over $230 million, delivered to 60 countries. The use of combination vaccines against diphtheria, pertussis, tetanus, hepatitis B and haemophilus influenza (DTP-HepB/Hib) has increased and UNICEF now supplies to 37 countries. These vaccines were considered ‘new’ a few years ago, but are now becoming integrated in the Expanded Programme on Immunization (EPI) of the poorest countries. In 2007, UNICEF was assigned by GAVI as the

procurement agency for pneumococcal and rotavirus vaccines - the next generation of new vaccines to be included in EPI.

Only a handful of manufacturers supply essential vaccines, and vaccine security – the sustained and uninterrupted supply of affordable quality vaccines - remains a challenge.

UNICEF supports safe vaccine delivery through the procurement of $42 million worth of auto-disable syringes and $27 million of cold chain equipment – and through following up on all deliveries using Vaccine Arrival Reports. 2007 also saw the introduction of new electronic time temperature monitoring devices on vaccine shipments.

With a global target of 90 per cent coverage, even with increasing quantities of vaccine available at the national level, challenges remain to ensure vaccines reach children in even the most remote villages.

NEPAL: The end of the cold chain… Village health worker Tilrupa Pokharel, accompanied by a neighbour, pauses on a mountain pass in the foothills of the Himalayas. Tilrupa walks from village to village, visiting pregnant women and adolescent girls, supplying iron and folic acid supplements and administering tetanus and polio vaccines. The vaccines in the UNICEF cold storage box have been kept cold throughout their journey to the roof of the world - to retain their potency.

MalariaMalaria is one of the most virulent killers of children - two children die every minute from malaria in Africa alone. Malaria accounts for almost one death in ten worldwide and nearly one in five in sub-Saharan Africa. An increase in funding and the procurement of malaria treated bednets is having a positive effect. UNICEF procurement of nets is 20 times greater today than in 2000 and during this period the supply market has expanded to meet the increase in global demand.

PLANNINGThe use of long-lasting insecticidal nets (LLINs) can lower malaria-related child deaths by a quarter. Together with its partners in the Roll Back Malaria partnership and suppliers and governments, UNICEF has been instrumental in planning needs for LLINs, and the transfer of technology from Japan to Africa. The value of UNICEF’s procure-ment fell from $116 million in 2006 to $83 million in 2007 (18 million nets). This reflects the success-ful transfer of procurement capacity from UNICEF to the governments concerned.

The Measles Initiative has played a key role in the dist-ribution of LLINs. The Initiative also provides additional life-saving health interventions in campaigns, including vitamin A and de-worming medicine. These integrated community based health programmes are saving lives and need to be scaled up, as an estimated 250 million mosquito nets are required to achieve the 2010 coverage target of 80 per cent in sub-Saharan Africa

UNICEF procured nearly $17 million worth of ACTs, rep-resenting nearly 17 million treatments. UNICEF and its partners continue to support the implementation of new, more effective antimalarial treatments. For many coun-tries the cost of ACTs is a challenge, but in 2007 the average cost of the main ACTs decreased significantly, with adult doses now less than $1 per treatment.

DESTINATION OF UNICEF SUPPLIES


A major innovation has been the development, procure-ment and first deliveries of the new Multiple Micronutrient Powder, which is sprinkled on a child’s food to combat vitamin deficiency. The use of this new dietary supple-ment is expected to make an important contribution to child survival.

Medicines and medical equipmentUNICEF continues to support activities to increase access to medicines, procuring a total of $148 million worth in 2007. The products in highest demand were vitamin A, albendazole and mebendazole, iron and folic acid tablets and injectable and low-cost antibiotics like cotrimoxazole. Demand for zinc tablets to treat diarrhoea has increased significantly and UNICEF continues to engage key partners to increase access. The procure-ment of medical equipment and supplies reached $112 million in 2007.

NutritionUnder-nutrition exacerbates the impact of disease and is the main underlying cause of half of all under-five deaths. In 2007, Supply Division procured $50 million worth of nutritional products to fight acute malnutrition in children, focusing mainly on ready-to-use therapeutic food (RUTF) and therapeutic milk. Great progress has been made in increasing local production of RUTF in Africa to ensure sustainability and support local needs. Supplementary foods as emergency rations were procured for countries like Bangladesh, the Democratic Republic of the Congo, Madagascar and Somalia.

PROCUREMENT In-kind assistance is also handled as stringently as other kinds of procurement. In 2007, the Micro-nutrient Initiative and the Canadian Government provided in kind-assistance of some 440 million vitamin A capsules to UNICEF. Vitamin A supple-mentation is one of the most cost-effective health interventions available - strengthening children’s resistance to disease.

PROCUREMENTIn 2007, UNICEF doubled its procurement of RUTF to nearly $18 million, principally of the nutritional paste plumpy’nut® from Nutriset, and delivered to 41 countries. UNICEF has now qualified - and procures from - local Nutriset franchises in Niger, Malawi and Ethiopia. Working in partnership with various agencies, and in connection with programme scale-up for child survival, UNICEF advocates for a significant increase in the delivery of RUTF. Nutriset now intends to triple its production, in France and in its franchises. This boosts local capacity and helps governments address severe malnutrition in children.

Water and sanitationOver 1.1 billion people still do not have access to safe water. Simple, low-cost supply interventions at the household level and in schools are key to saving chil-dren’s lives. UNICEF procured some $68 million worth of water and sanitation supplies in 2007.

In coordination with Country Offices and governments, Supply Division has begun developing water and sanit-ation ‘technology packages’. These include guidance notes, tips, codes of practice, standard designs, cost components and specifications – with the emphasis on encouraging communities and local governments to take over the procurement of life-saving equipment and supplies themselves.


HIV/AIDS AND CHILDREN Less than 25 per cent of pregnant women with HIV/AIDS are offered services for the prevention of mother-to-child transmission (PMTCT) of HIV to their infants, and only 16 per cent of HIV-positive children in need of paediatric AIDS treatment receive it. Half of all HIV-infected infants die before the age of two. UNICEF continues to provide procurement and supply manage-ment support for PMTCT and treatment, including quality medicines and diagnostics at best prices, and shares information related to this complex market with partners.

DELIVERYThe delivery and clearance of HIV/AIDS medicines, as with other kinds of drugs, is one of the most critical steps in the supply chain. Almost all the medicines UNICEF supplies are procured through Supply Division for shipment to countries in the

developing world. The capacity of the drug regulatory authorities

in these countries varies from non-existent to well-developed. A lot of careful up-

stream work needs done to make sure the delivery can enter the coun-try smoothly. Every shipment of HIV/AIDS medi-cines must be accompanied by important documen-tation, such as the manufacturer’s certificates of analysis, to facilitate clearance at the country’s port of entry, which is where the real challenge begins. Ensuring repeated safe delivery to the beneficiaries is critical for the effective treatment of HIV/AIDS.

WHAT UNICEF BUYS INTERNATION-ALLY FOR ITS PROGRAMMES

SOUTH AFRICA: Thembi Ngubane, 19, and her boyfriend Melikhaya Mpumela (both HIV positive) stand with their 16-month-old daughter in Cape Town. Their daughter is HIV-negative thanks to Thembi’s participation in a PMTCT programme.

The procurement value of HIV test kits increased from $7.3 million in 2006 to $11 million in 2007. A similar growth has been observed in HIV/AIDS-related diagn-ostic equipment, reagents and consumables, which reached over $5 million in 2007.

The procurement of antiretrovirals (ARVs) almost doubled from $37 million to nearly $66 million worth of supplies in 2007. Almost all of this was for Procurement Services customers. (Supply Division also maintains a stockpile of ARVs that can be air freighted to prevent the interruption of treatment where governments experience a stock-out.) The number of treatment packs procured grew from 2.8 million to over 5 million in 2007, partly due to a substantial fall (19 per cent) in the price of the most common treatments.


EDUCATIONAccess to appropriate educational supplies is essential to the fulfilment of children’s right to education. In 2007, UNICEF shipped educational kits and sup-plies for over 12 million children and over 100,000 teachers, working closely with Country Offices and governments on planning, procurement, packing and distribution. Global procurement for education items reached $67 million.

There is a rapidly increasing demand from countries requiring education supplies. A key portion of the educational items procured by UNICEF are manu-factured in China. Supply Division has therefore established a warehouse hub in Shanghai – to reduce freight costs and lead times though closer proximity to suppliers.

END-TO-END CHAINDecentralised last-mile supply operations are a key element of sustainable delivery and distribution. A good example is the supply management of ‘go-to-school’ education programmes in countries like DR Congo (supplies for 4.5 million children) and Sudan (supplies for 1.3 million children). The project is carefully planned a year in advance, and appropri-ate packing sites (such as Goma, Lubumbashi and Kinshasa in DR Congo, and Juba in Sudan) are identified. Goods are then procured, quality control-led, shipped, received at the packing sites, packed into kits with local labour and finally distributed for the school-start. Besides being cost-efficient this also builds capacity in the country.

QUALITY ASSURANCE AND COMPLIANCE UNICEF makes quality a priority, ensuring that all supplies procured comply with international norms set by regula-tory agencies such as WHO (e.g. for pharmaceuticals, biological products, medical devices and bednets). All suppliers for standard pharmaceutical and medical items are pre-qualified either by Supply Division or by WHO.

Supply Division staff undertook Good Manufacturing Practice (GMP) inspections of pharmaceutical manufacturers premises as well as a number of other factory inspec-tions for products such as bednets, educational supplies and water equipment.

Supply Division continues to enhance the efficiency and effectiveness of its operations by developing its quality management system (QMS), in prepa-ration for ISO certification. Staff were trained in the principles and practices involved in maintaining a QMS.

INSPECTIONGoods received in the Copenhagen warehouse are inspected by quality assurance staff (over 2,500 inspections in 2007), and inspection companies conducted over 280 pre-delivery inspections at supplier premises of goods being shipped directly to the field. As part of the procurement tendering process the quality of over 700 product samples was also evaluated.

ALBANIA: A girl looks up from reading, in a class in Halit Coka Compulsory and High School in Bathore, a suburb of Tirana. Some 4,000 children attend the school, which was built for 1,000 and classes are taught in three shifts. The school is without heat and had no library. With support from IKEA, the Swedish home-furnishings retailer and UNICEF’s largest corporate donor, UNICEF has donated new books to the school through the Albania Reads initiative, which will reach more than 200,000 children.

WarehousingUNICEF maintains a stock of some 750 items and produces 37 different kits, including the school-in-a-box, the emergency health kit and safe-delivery kits. UNICEF central warehouse throughput reached $93 million in 2007, representing a 27 per cent increase over 2006. This is the sixth year of growth and reflects increases in all commodity areas.

Supply Division managed over 8,500 international shipments, to a total freight value of $64 million – including 26 charter flights (mainly for vaccines, medicines, tents, therapeutic food and other emer-gency relief supplies).

LogisticsImprovements in the downstream portion of the supply chain was a focus area in 2007. A particular effort is being made to strengthen UNICEF exper-tise in logistics, to enable us to clear bottlenecks and maintain a smooth flow of necessary goods to children and communities.

Supply Division has employed logistics experts to focus on supply chain needs in key commodity areas and to support the training of UNICEF staff in logistics. This includes UNICEF staff members in nearly 30 countries enrolled in the Institute of Logistics and Transport/Fritz Institute Logistics Certification course.

UNICEF KITSOver 350,000 kits (an increase of 43 per cent over 2006) were shipped either from the Copenhagen warehouse or field locations with supervision and training provided by Supply Division warehouse experts.

IN-COUNTRY LOGISTICS UniTrack, a UNICEF developed stand-alone warehouse management software, is tailored for environments where UNICEF is asked to manage or support in-country logistics. In 2007, over 150 staff were trained, and UniTrack was installed in 9 countries - bringing to 14 the number of countries equipped with UniTrack. The roll-out will continue through 2009.

BANGLADESH: A woman carries a UNICEF family kit, distributed by the NGO South Asia Partnership, in Borguna District. The kit contains essential household items such as clothing, cooking utensils, a plastic sheet and soap. UNICEF distributed 10,000 such kits to families who lost their homes in Cyclone Sidr in November 2007.

Working closely with the World Food Programme (WFP), the UN High Commission for Refugees (UNHCR) and several NGOs, UNICEF plays an active role in the Humanitarian Logistics Cluster that is led by WFP.

Emergencies Supply Division assisted 66 countries to respond to emergencies in 2007 - notably in DR Congo, Sudan and Pakistan - shipping goods with a value of $48 million. Most emergency supplies were sent to 34

countries in Sub-Saharan Africa. In addition, UNICEF logisticians were deployed to provide urgent support in-country. With a network including the Copenhagen warehouse and hubs, suppliers, other UN and NGO partners (such as WFP Humanitarian Response Depots), UNICEF ensures a global reach in its emergency response, and can deliver emergency supplies to cover the needs of a population of 250,000.

WAREHOUSING, LOGISTICS AND EMERGENCIES


AFGHANISTAN: Two girls operate a UNICEF-provided hand pump, while their classmates wash their hands, at Phool-e-Rangeena Government School in Herat. Some 7,000 children attend class in three shifts. UNICEF supports the school with water points, latrines, teacher training and school supplies. The International Organization for Migration (IOM) has donated 30 tents to relieve the pressure on the main building.

UTILISATIONAs part of a broad intervention in Ethiopia’s entire health supply chain, UNICEF received Procurement Services orders worth over $58 million from the Gov-ernment. These consisted mainly of Health Post and Centre kits, as well as other essential immunization, HIV/AIDS, medical and malaria supplies. This comple-ments the training of over 17,000 female extension workers (of a target total of 30,000). Kit packing is done in Ethiopia as part of capacity building activities.

UTILISATION UNICEF Procurement Services is supporting the Government of Malawi in its fight against AIDS. The project reached a value of $31 million for HIV-related items and services in 2007. A record 100,000 HIV-positive people are now receiving antiretroviral therapy through a partnership between the Govern-ment, UNICEF, donors and other agencies.

Procurement Services In coordination with UNICEF Country and Regional Offices, Supply Division Procurement Services works with governments and other partners to help them pro-cure supplies for mothers and children. The growth in new funding mechanisms such as GAVI and the Global Fund for Aids, TB and Malaria, represents another avenue for governments to use UNICEF Procurement Services to address the needs of children.

In 2007, Procurement Services transactions totalled $671 million worth of supplies shipped to almost 100 countries. Following steady increases in the volume of Procurement Services, allowing economies of scale, UNICEF’s handling fees were reduced in 2007.

PARTNERSHIPSAt the mid-point of the Millennium Development agenda, UNICEF is rallying partners – including governments, UN agencies, global financing mechanisms, NGOs, universities and the corporate sector - to Unite for Children. Partnerships are critical at every stage of the supply chain – from assessing the needs of children in country, all the way through to end-user monitoring and evaluation. For example, UNICEF worked with the Government and a range of partners on in-country logistics for health commodities in Ethiopia in 2007.

Almost 80 per cent of UNICEF procurement was undertaken with inter-UN agency collaboration in 2007, and some 31 per cent of Supply Division’s procurement was on behalf of external partners. UN coherence and external partnerships leverage resources for children and share technical and supply expertise.
