Welcome to St. Joseph’s Health Centre, Guelph (SJHCG), your next home.

Our goal is to provide care that will meet your individual needs in a warm, caring and home like atmosphere. We have a sincere interest in the well being of every person and will do everything possible to ensure your comfort and safety.

On the day you arrive, you may expect the following:

• ourphotographwillbetakensothatitcanbeusedforidentification purposes.

• Anursewillmeetwithyoutodiscussyourmedicalandother health needs.

• Yourfamilywillbeinvitedtojoinyouforyourfirstmealwhile they help you to settle into your room.

• Ourstaffcanalsoprovideyouwithinformationabout:a. designatingaPowerofAttorney(tomakedecisions

about your personal care if – for some reason - you are unable to do so for yourself); and,

b. making Emergency Care Plan (that is, making decisions about how you would like to be treated if you were facing a life-threatening situation).

• Laundry personnel will come to your room to label your clothing.

Finally, this booklet will give you some general information about our programs and living arrangements.

Key Family Information for Long Term Care the First 72 Hours

1. Please do not bring in any furniture or electronics until it has been approved.Thisprocesswillbeexplainedtoyouduringadmission.

2. Label every belonging other than clothing. St. Joseph’s Health Centre Guelph will label all clothing items.

3. YouareresponsibletocontactBellforaphoneconnectionor internet hookup. Contact the Business Office, if you have questions.

4. Every electrical device, including lights, must be approved by maintenancebeforeuse.InformationTechnology(IT)departmentmust approve the use of any phone.

5. Visiting hours are “anytime” as long as it does not bother other residents/patients.

6. Obtain a telephone contact number for the nurse or staff.

7. Should your loved one pass away, it is the responsibility of the family to pack and remove the client’s personal possessions within 24hours(furnitureandequipmentwithin7days).Ifyouareunable to pick up the items within 24 hours, please contact the TeamLeadtomakealternatearrangements.

8. Familycelebrationsareencouraged.Thefamilyspacescanbebooked by calling Environmental Services at ext 4412.

9. Parking cards (3 per family) are available for access to the parking lot.Arefundabledepositisrequiredforeachcard.

10.Youcanjoinyourfamilymemberforameal.Thecostis$5permeal.Thefirstmealisfreeforonefamilymemberondayofadmission.YoumaypurchaseyourmealticketatSwitchboard.

11. St.Joseph’sHealthCentreGuelphisafragrancefreeorganization.Do not wear perfume or cologne when visiting or provide fragrant products for your loved one. Do not bring lilies, lavender or freesia flowers for your loved one.

First Things First

IndividualizedLifePlanningYourpersonalcareneedsareveryimportantbothtoyouandtous,however,werecognizethatthereareotherequallyimportantaspectsof you and your life that make you unique. We want to learn more about your personal history, life-long interests, significant relationships andpreferences.Inthisway,wearebetterabletohelpyoutoliveadignified and meaningful life in your new home at St. Joseph’s Health Centre Guelph.

Within six weeks after you move in, you will be invited to a meeting called“LifePlan–GettingtoKnowYou”.Staffrepresentativesfromvariousdepartments(Food&NutritionServices,Nursing,RecreationTherapyandSocialWork)willmeetwithyouandyourfamilytodiscussyour abilities, needs, preferences and routines in order to develop a “LIFEPLAN”.

AdoctorwillberesponsibleformanagingandprescribingyourmedicalcareandmayworkwithaNursePractioner(NP)inyourcare.The“Lifeplan”willhaveallofyourindividualneedsalongwiththedetails of how, when, where and by whom each aspect of your care willbeprovided.Acopy is shared with the team working in the neighbourhood where you will be living.

Thestaffwillcontinueto meet with you and/or your family on an annual (or as needed) basis to review and update your Lifeplan.

Ifyourneedschangeat any time, and you require admission to another facility, or you wish to return home, a Case Manager from the CommunityCareAccessCentre(CCAC)oraSocialWorkerwillassistyouin making the necessary arrangements.

AccommodationSt. Joseph’s Health Centre Guelph offers both single (private) and double (basic) occupancy rooms.

Rooms are furnished with a bed, a bedside table, a chest of drawers, a flat screen television mounted on the wall, an over the bed light and onechairforvisitors.Youalsohaveyourownindividualcloset.

During the admissions process the furniture policy will be reviewed. Please do not bring in any furniture or electronics until it has been approved as it may be returned, at your own expense, if it doesnotmeettheorganizationspolicyoryourcareneeds.Anyelectricalequipmentneedstobeapproved by Environmental Services before they can be brought into your room.

Each bed and every washroom has a call bell for you to contact nursing staffifyouneedhelp.Nursingstaffwillshowyouhowthecallbellworks.

Weencourageyoutopersonalizeyourownlivingspacewiththingslike personal photos, wall decorations and a favourite bedspread or comforter.Thememoryboxesareawonderfulwaytohelpothersgettoknowyou–andtheyaregreatconversationpieces.Thisisaspacewhich you can use to display any items or memorabilia that have special significancetoyou.Theaddedbonusisthatyouruniquememoryboxmay help you locate your room more easily while you are learning your way around your new home.

Undersomecircumstances,e.g.illnessorchangeinaresident’scondition,aresidentmayhavetobetransferredtoanotherroom.Youand/or your family will be notified if this has to occur and we will assist you in settling in.

Personal BelongingsSt. Joseph’s Health Centre Guelph takes every effort to make your stay with us as comfortable and safe as possible. With that in mind, the following points will help you and your family determine what to plan on bringing in with you and what restrictions, for safety reasons are necessary.

ClothingItwillbeeasierforyouifyourclothingiswashableandcomfortabletowear.Ifyouneedassistancetogetdressed,itisalsoeasierforyourcaregivers if clothing is loose-fitting or your caregiver might suggest purchasing adaptive clothing.

Consider bringing:• enoughclothingtolastfortwoweek(suitedfortheseason).This

could include:- dresses and slips or pants and trousers- underwear, including brassieres and/or undershirts- socks and/or pantyhose- shoes and/or slippers- cardigans and/or sweatshirts- nightgowns and/or pajamas- housecoat- coat or jacket and hat (winter and sun)- a scarf, mitts/gloves or boots (in winter)

St. Joseph’s Health Centre Guelph is not responsible for loss or damage of personal belongings or valuables kept in your room.

Due to limited storage space, we ask that “out-of-season” clothing be stored with family.

We also rely on family to remove clothing that is no longer being used

andtopurchasenewclothingwhenthisisnecessary.Nursingstaffcanhelp identify clothing that needs to be replaced.

Finally,SJHCGorganizesregularvisitsfromcompaniesthatspecializeinadaptive clothing (i.e. designed to make dressing/undressing easier for peoplewithspecialphysicalneeds).PleaseaskaRecreationTherapistfor additional information.

LaundryLaundry service for personal clothing is provided once a day. Repairs of personal clothing is not provided. SJHCG does not provide dry cleaning services. Wool, suede and leather clothing is not recommended.

Familiesmaywishtolaundertheirlovedone’sclothingathome.Youare also free to use the client washer and dryer on Level One main buildingoroneachoftheunitsintheNorthWingtolaunderpersonalclothing; however, you must bring your own detergent. Please remember that detergent and fabric softener must be scent free.

PersonalHealthCareItemsItisimportantthatyoubringany:• eyeglasses• hearingaids• dentures

Please make sure that these items are marked for identification prior to the day of your admission.

Youshouldalsobring:• any prescribed medications, over-the-counter drugs and supplements

that you currently use in their original labeled containers.

Please ask your pharmacist for a list or “print-out” of all your medications to bring with you as well.

Once staff has reviewed your medications and the list from your pharmacist, you will be asked to send your medications home and St. Joseph’s pharmacy provider will then be responsible for dispensing your medications including any over the counter medications.

Ifyoudonotalreadyhaveawheelchairorwalkerbutyouthinkyoumightneedone,pleasewaituntiltheRestorativeCareTeamatSJHCGhascompleted their assessment to assist in obtaining the most appropriate piece of equipment.

Personal Belongings ToensurethatSt.Joseph’sHealthCentreGuelphprovidesasafebuthome-like health care environment for all clients and staff, SJHCG must balancetherightofclientstopersonalizetheirlivingspaceagainstallapplicable legal safety standards. For this reason, SJHCG must approve the use of all personal furniture and electrical equipment before it is moved into the client’s living space.

Memory BoxesAlockedglass“memorybox”islocatedoutsideofeachbedroom.Thisis a space which you can use to display items or memorabilia that has specialsignificancetoyou.Thememoryboxesareawonderfulwaytohelpothersgettoknowyou–theyaregreatconversationstarters.Theadded bonus is that your unique memory box may help you to locate your room more easily when you are first learning your way around the neighbourhood.Ifyouhaveverylargepicturesthatyouwouldliketodisplay, staff will arrange to have them mounted on your wall.

ItemsYouMayBringOnceApprovedforUseAllclientroomsincludeadresser/night-table,bedandguestchair.

All personal furniture/equipment items must be approved before being brought into the facility.

Please complete “Furniture/Equipment Application Form” to request approvalforthesepersonalitems.Askthestaffforacopyoftheapplication form. Please be advised that the healthcare team will require at least two weeks to assess your care needs as well as the bestwaytoset-upyourroomtomeetthoseneeds.Inaddition,aninspection of all electrical equipment is required.

Youmayapply to have the following furniture/equipment in your room:

• Comfortablechairthatmeetsthefollowingsafetyspecifications.Chairs should be constructed of upholstery that can be easily wipedclean(leather,vinyl,PVC).Themaximumdimensionsshouldbe 28 inches wide and 28 inches deep. Stationary chairs are acceptable, however swivel rockers, gliders and recliners pose too great a risk for resident falls.

• SJHCGhasasupplyofcomputerdesksthatcanbepurchasedtogoin resident rooms.

RemovingFurniture&EquipmentUndersomecircumstances(suchasachangeinyourhealth),somefurniture or equipment may need to be removed so that we can meet yournewcareneeds.Ifitemsarenotsafeforyoutouseand/ormakethe environment unsafe for you and/or your care providers, you will be asked to have them removed.

When a resident is discharged or passes away, it is the responsibility of the family/SDM to pack and remove the resident’s personal possessions within 24 hours. Furniture and equipment must be removed within 7days.Ifyouareunabletopickuptheitemswithin7days,pleasecontact SJHCG to make alternate arrangements.

PleaseNote:Allfurniture/equipmentmustbedeliveredandremovedduring regular business hours, i.e. Monday- Friday from 8:00AM-4:00PM.

TelevisionsTherearetelevisionsmountedonthewallineachresidentroom.Youcan arrange to have cable services by contacting the Business Office and providing the required information. Once cable is in service, the BusinessOfficewillbillyouthroughtheResidentTrustaccount.

Please note that television services cannot be provided by an external company such as Rogers Cable or Bell Canada.

Telephones Therearetelephonesineachneighbourhoodthatyou may use; however, you can also arrange to have a personal line installed in your bedroom.

Ifyouwouldlikeyourowntelephone,youcanactivatetheserviceinyour room by contacting Bell Canada and our Business Office directly. Once the telephone is in service, Bell Canada will bill you directly for the cost of installation and any subsequent monthly or long-distance charges.Tomakealongdistancecallfromaneighbourhoodphoneyouwill need a calling card.

IfyouarepurchasingacordlessphoneyoumusthaveitapprovedbytheInformationTechnologyDepartmentatextension4100.Certainphonescancauseinterferencewiththeorganization’swirelessphonesystem which creates a safety/risk issue for residents and patients.

Ifyouwouldlikeyourown

telephone line (for use with

a land line and/or portable

telephone), you must contact

Bell Canada directly to

arrange for installation.

Other Important Information About Our Centre

Our HistorySt. Joseph’s Hospital and Home was opened in 1861 by the Sisters of St. Joseph, Hamilton.ThreeSistersprovidedcareinthe original sixteen bed structure. Over the years many additions were made to the old building to accommodate the changing needs of our community.

In1992,itwasdeterminedthatSt.Joseph’sHospital and Home would transfer its acute care services to the Guelph General Hospital (GGH) and our role would be redefined so that “St. Joseph’s Health Centre” would become a “long term care centre of excellence”.

The1990swereaperiodoftransitionforSt.Joseph’sthatculminatedinthefinaltransferofallacutecareservicestoGGHinAugust2001.TodaySt.Joseph’sisafullyaccreditedinstitutionthathouses64medically complex and palliative residents, 240 long-term care residents and 27 patients in rehabilitation.

St. Joseph’s also offers excellent community Outreach Programs for people living in the community such as:• Out‘NAboutSeniorsDayProgram• AlzheimerDayProgram• AcquiredBrainInjuryProgram

Inaddition,wehavenumerousoutpatientorambulatorycareservices such as:• OutpatientPhysiotherapyandOccupationalTherapy• SpeechLanguagePathologyservices(forallagegroups)• Clinicalservices,includingOpthamology,BoneDensitometryandAmbulatoryCareClinics

• OntarioTelemedicineNetwork(OTN)connectingpatientsandresidents of our community with physicians from other areas of the province remotely

Enhanced Living ModelTheEnhancedLivingModel(ELM)representsashiftawayfromaprimarily medical focus (where care is planned in response to an individual’s disease or health problems) to a social model that is more focused on the resident we serve. Care is planned after considering all aspects of a person’s life and health, including physical as well as intellectual, emotional, social, cultural and spiritual.

Residents’ Bill of RightsThefollowingBillofRightsrelatestoallresidentsofLongTermCareHomes in Ontario; however, the management and staff of St. Joseph’s Health Centre Guelph embraces these rights as privileges appropriately extended to all residents and outpatients who access services provided within our facility.

Because long term care homes are essentially homes for the residents who live there, these facilities are expected to operate in a way that values and encourages efforts to meet the physical, emotional, social, culturalandspiritualneedsofeachresident.Itisalsoassumedthateach resident will be given the opportunity to contribute to the physical, emotional, social, cultural and spiritual needs of others – to the extent of his or her ability to do so.

Thefollowingrightsofresidentsaretobefullyrespectedandpromoted:

1. Every resident has the right to be treated with courtesy andrespectandinawaythatfullyrecognizestheresident’sindividuality and respects the resident’s dignity.

2. Every resident has the right to be protected from abuse.

3. Every resident has the right not to be neglected by the licensee or staff.

4. Every resident has the right to be properly sheltered, fed, clothed, groomed and cared for in a manner consistent with his or her needs.

5. Every resident has the right to live in a safe and clean environment.

6. Every resident has the righttoexercisetherightsofacitizen.

7. Every resident has the right to be told who is responsible for and who is providing the resident’s direct care.

8. Every resident has the right to be afforded privacy in treatment and in caring for his or her personal needs.

9. Every resident has the right to have his or her participation in decision-making respected.

10. Every resident has the right to keep and display personal possessions, pictures and furnishings in his or her room subject to safety requirements and the rights of other residents.

11. Every resident has the right to:

i) participate fully in the development, implementation, review and revision of his or her plan of care,

ii) give or refuse consent to treatment, care or services for which his or her consent is required by law and to be informed of the consequences of giving or refusing consent;

iii) participate fully in making any decision concerning any aspect of his or her care, including any decision concerning his or her admission, discharge or transfer to or from a long-term care home or a secure unit and to obtain an independent opinion with regard to any of those matters, and,

iv) have his or her personal health information within the meaning of the Personal Health Information Protection Act,2004keptconfidentialinaccordancewiththatAct,and to have access to his or her records of personal health information, including his or her plan of care, in accordance withthatAct.

12. Every resident has the right to receive care and assistance towards independencebasedonarestorativecarephilosophytomaximizeindependence to the greatest extent possible.

13. Every resident has the right not to be restrained, except in the limitedcircumstancesprovidedforundertheLongTermCareHomesAct,2007andsubjecttotherequirementsprovidedforunderthatAct.

14. Every resident has the right to communicate in confidence, receive visitors of his or her choice and consult in private with any person without interference.

15. Every resident who is dying or who is very ill has the right to have family and friends present 24 hours per day.

16. Every resident has the right to designate a person to receive informationconcerninganytransferoranyhospitalizationofthe resident, and to have that person receive that information immediately.

17. Every resident has the right to raise concerns or recommend changes in policies and services on behalf of himself or herself orotherstothefollowingpersonsandorganizationswithoutinterference and without fear of coercion, discrimination or reprisal, whether directed at the resident or anyone else,

i. the Residents’ Council,ii. the Family Council,

18. Every resident has the right to form friendships and relationships and to participate in the life of the long-term care home.

19. Every resident has the right to have his or her lifestyle and choices respected.

20. Every resident has the right to participate in the Residents’ Council.

21. Every resident has the right to meet privately with his or her spouse or another person in a room that assures privacy.

22. Every resident has the right to share a room with another resident according to their mutual wishes, if appropriate accommodation is available.

23. Every resident has the right to pursue social, cultural, religious, spiritual and other interests, to develop his or her potential and to be given reasonable assistance by the licensee to pursue these interests and to develop his or her potential.

24. Every resident has the right to be informed in writing of any law, rule or policy affecting services provided to the resident and of the procedures for initiating complaints.

25. Every resident has the right to manage his or her own financial affairs unless the resident lacks the legal capacity to do so.

26. Every resident has the right to be given access to protected outdoor areas in order to enjoy outdoor activity, unless the physical setting makes this impossible.

• Therearesmallsittingareasattheendofeachneighbourhoodcorridor where you can stop, chat and enjoy a hand-quilted mural or glassed wall display.

• Eachneighbourhoodhasacommunaldiningroom;however,residents can be seated at an individual table to enjoy a meal or snack with family and friends.

• Thereareopenairbalconieswhichcanbeaccessedonthesecond,third, and fourth levels of each building.

• AFamilyDiningRoom(locatedonLevelOnemainbuilding)is available for special events such as birthdays or holiday celebrations.Thisroomisequippedwithakitchenanddining/living space, a small area for coats, and a washroom. Some of our residents are frail or elderly, have chronic health conditions (such as diabetes) or take medications that make them more susceptible to infections (such as steroids), they are particularly vulnerable to contagiousdiseases.Notonlyaretheymorelikelytocontracta“cold”or the “flu”, but their symptoms are likely to be much more severe.

As guests enter and leave the building we request that they use the hand gel at the entrance of the facility. Thisisthesinglemostimportant way to prevent the transmission of viruses and other germs.

Preventing infection takes on even more importance during “flu” season–extendingfromlateNovemberuntillateMarchorearlyApril.Each fall, residents, staff and volunteers are offered vaccination against influenza.FamilyandfriendswhoregularlyvisitSt.Joseph’sarealsoencouraged to receive the vaccine.

How Family and Friends Can HelpMany people ask us how they can continue to support and provide care for a family member or friend who has moved to our Centre.

Here are some suggestions for activities in which extended families and friendscanplayanimportantrole.You,thefamilymember,may:

• Helpyourlovedoneorfriendatmealtime.Yourcompanyandassistance makes it more likely that residents enjoy their meals and are adequately nourished.

• AttendeventsandcommunityoutingsthatareorganizedbytheRecreationTherapyProgramsuchas“pubnights”,concerts,shopping trips or other special events. Recreation Calendars are printed monthly and are readily available.

Family Gathering Spaces Ifyouwouldliketogettogetherwithyourlovedoneforagatheringand a meal contact Environmental Services at ext 4412 for the many optionsavailable.Astaffmemberwillworkwithallareasandstafftofind the perfect location for your gathering or celebration.

Family Room - Our quaint family room is located on the first floor of the main building and is set with all appliances for a sit down meal. Food Services will provide a cart with all the necessary supplies needed for your gathering.

Recreation Kitchen - We have a room located on the third floor of the main building with a full kitchen and large table. Environmental Services or the Recreation Staff would be happy to assist you with this space if you are interested.

Oasis Lounge - Our lounge is located on the second floor of the main building and it is a nice atmosphere to have a more informal gathering. Thoughitisnotafullkitchentherearesmallappliancesavailableforyouifneeded.Agreatplacetobringintakeoutorhaveacelebration!

Bistro - Located on the first floor of the main building, our Bistro/Cafeteria is open for lunch through the week (excluding some holidays) and is a great spot if you are having a large gathering and need the space especially during the evenings and weekends.

Van ServiceSt. Joseph’s has a wheelchair accessible van that can be used to transport residents locally to medical appointments, home visits or other special activities.

Ifresidentsrequireanyassistanceenrouteoraftertheyarriveattheirdestination, a family member or friend must go with them.

Toarrangefortheuseofthevan,pleasecontactyournurse;and please note that: 1. Itisimportanttobookearlytoensurethatboththevananda

volunteer driver are available; and,

2. Because volunteer drivers provide this service, not all bookings can be accommodated.

Thevanmaynotbeavailableoryouneedanothertypeoftransportation then the staff will assist you in arranging the following;

VolunteersVolunteers play a crucial role in enhancing the quality of life for residents.TheDepartmentofVolunteerServicessupportsover200volunteerswhomakeupthemembershipoftheVolunteerAssociationat St. Joseph’s Health Centre Guelph.

Drivers, volunteer visiting, neighbourhood support, providing meal companionship, and assisting with recreational programs are just some of the many ways that volunteers choose to contribute.

Medical Services Residents may choose to continue their care with a previous family physician.Eachfamilyphysicianvisitsregularly.Twenty-fourhourphysician coverage is available to residents. St. Joseph’s Health Centre Guelph also has a core group of physicians that are available to provide careifyoudonothaveaphysician.ThereisaNursePractitionerinLongTermCare,whoworkscloselywiththephysicianstoprovidemedicalcare.

Nursing Nursingstaffarededicatedtoprovidingahighstandardofcareandattention to the residents at SJHCG. Our goal is to promote quality of life for our residents through maintaining their dignity and independence.

Categories of Staff:TheDirector and Assistant Directors are responsible for the overall nursing care and are available Monday to Friday or a message may be left on voice mail.

Registered StaffconsistofRegisteredNurses(RN)andRegisteredPracticalNurses(RPN).TheyarereferredtoasTeamLeaders.Theyoverseetheresidentcareonaspecificneighbourhood.ARegisteredNurseisonduty24hoursaday.Registeredstaffwilladministermedicationsandcarryoutphysicianorderedtreatments.Theyalsocoordinate the care provided by Personal Support Workers (PSW) to our residents and patients.

Personal Support Workers (PSW) are on duty 24 hours a day and provide the resident’s personal care such as bathing, toileting, dressing andassistingwithmeals.Theybecomefamiliarwiththedailyroutinesofeachresident.Inreturntheresidentandtheirfamilywillbecomefamiliar with the staff as they come to know each other on a first name basis.

FoodandNutritionServices TheDepartmentofFoodandNutritionServicesisresponsiblefor providing meals and nutrition services to our residents and families.Therearemaincourseoptionsavailableateachmeal,and every effort is made to provide a menu that offers variety and choices.

Restorative Care PhilosophySt. Joseph’s Health Centre Guelph has a restorative care philosophy that provides services to maintain or improve a resident’s functional and cognitive abilities in all aspects of daily living. Members of the care team at SJHCG include nursing, therapy services, social work, spiritual and religious services and recreation therapy.

TherapyServices AllnewresidentsareassessedbyaPhysiotherapistand/orOccupationalTherapisttodeterminetheircurrentlevelofindependence.Theappropriate mobility and therapy programs are recommended to promote and maintain each resident’s level of independence. Our staff oftrainedRehabAssistantswillprovidebothindividualandgrouptherapy programs on each neighbourhood to promote and maintain your level of independence.

Aphysiotherapistwilldoanassessmenttodeterminewhetheryoursafety, mobility or independence could be improved with the use of assistive devices such as a wheelchair or walker. Our care staff will alsoassistinarranginganOccupationalTherapy(OT)consultationifneworadditionalseatingorassistiveequipmentisrequired.Aswell,our care staff will assist in arranging a Speech Language Pathologist consultation to assess communication and swallowing issues if needed.

Spiritual and Religious Care Our Spiritual and Religious Care staff are available to offer a caring presence whenever you feel the need for emotional or spiritual support. Our staff value and respect the beliefs, religious practices and culture of all our residents and patients. Spiritual and Religious Care staff encourage and support maintaining your connections with your own faith community.

Roman Catholic Mass is celebrated daily and Ecumenical services are held every Sunday in the Chapel. Seasonal celebrations are held throughout the year, and memorial services are held regularly. Also,theChapelisalwaysopenforyouoryourfamilytouseforprayerormeditation.TheChapelislocatedinthemainlobbyonleveltwo.

HorticulturalTherapyTheHorticultureTherapistenablesresidentstobeinvolvedinplantandgardenrelatedprogramsandtasks.Thiscanbedonewithindividualsintheirrooms,orinspecificgroups.ExamplesofHorticultureTherapygroups include Men’s Greenthumbs, Women’s Sunflowers, Food Adventurers,andaFamilySupportedHorticultureGroup.

The“Tranquility Garden” and the “Elmwood and Ashley Garden” are beautiful outdoor spaces that you are welcome to visit and enjoy on your own or with family. Other Information

LeavesofAbsenceUndertheLong Term Care Homes Act, 2007, any resident of a long term care home may take 30 days of medical leavewithoutlosinghis/herbed.Youmaytakeamedicalleavewheneveryourequirehospitalcare.Inaddition,a60dayleaveof absence for psychiatric care may be taken as often as required.

Youarealsoentitledto48hoursof“casualleave”perweekaswellas21 days of vacation per calendar year.

End of Life CareEndoflifecareemphasizestherighttochoiceforthoseresidentswhono longer look towards a cure. Our goal is to respect the resident’s wishes and beliefs and to provide comfort and dignity through pain control and caring staff. Family members are encouraged to be present as much as they wish.

ImmunizationSt. Joseph’s encourages all residents to maintain and update their immunizationagainstpneumococcalpneumonia,tetanus,diphtheriaandinfluenza.Ifyourimmunizationisnotup-to-dateonadmission,youwillbeofferedimmunizationagainstpneumoccalpneumoniaand/ortetanusanddiphtheriaatthattime.Asnotedpreviously,influenzavaccine or “flu shots” are offered to all residents and patients each fall. Theseareorderedbyyourphysician.

MedicationsFor long-term care residents, all prescription medications and over-the-counter, non-prescription drugs must be obtained from the SJHCG contracted pharmacy. Medications are prepared in separate packages and are delivered to the facility. Emergency drugs are kept on hand in the facility and where needed special deliveries could be made. Medications including over-the-counter, non-prescription drugs should not be taken without consulting with the registered staff and/or your physician. Please feel free to ask questions about your medications and treatments.

SmokingResidents may only smoke outside in the smoking shelter provided at the front of the building. Residents must be able to get to the shelter independent of staff and have the ability to smoke safely. Visitors and family members are not permitted to use this shelter.

AlcoholicBeveragesAphysician’sorderisrequiredforallalcoholicbeverages.

Youmaykeepyourownsupplyofliquorontheneighbourhoodwhereyoulive.Allpersonalliquormustbekeptinalockedcompartment.

Alcoholisalsoservedatscheduled“HappyHours”heldintheOasisLounge (see calendar for specific dates), residents will require a doctors order to consume alcohol at these events.

AlcoholisalsoservedatPubNight,timesanddatescanbefoundonthe Recreation Calendar.

MailandNewspapersResident mail is delivered by volunteers and residents each week-day (except holidays). Outgoing mailboxes are located at the front entrance.

Stamps are available from the Silver Maple Coffee and Gift Shop.

NewspapersandmagazinesarealsoavailableattheCoffeeShop.Ifyou wish to subscribe to a daily newspaper, you may pick up your paper attheBusinessOffice(locatedintheTownSquaremainbuilding.

Hair Care TheVolunteerAssociationoperatesaHairCareCentreforbothwomenandmen.ItislocatedonLevelTwomain building, just outside the Whispering Oaks neighbourhood.Thecostforthisservicemaybechargedto your monthly bill. Gift certificates are also available.

AppointmentsmaybemadedirectlyattheHairCareCentreorthroughthe nursing staff outside regular business hours.

Fire and SafetyResident safety is of paramount importance at St. Joseph’s Health Centre Guelph. We have a variety of codes that require staff response from missing resident to environmental spills. Staff are trained to respond quickly and appropriately in the case of codes such as a fire and, if necessary, to evacuate all occupants of the building.

We believe that knowing and understanding our fire plans will provide you with a sense of confidence and security in your new home.

NewDesignFeaturesHigher construction standards mean that new health care facilities are designed to aid staff in the quick identification and confinement of fires.Thisshouldalsoresultinareducedneedtoevacuateoccupantsinthe case of a fire emergency.

Some of the fire safety features in our building are outlined below:• Fire&SmokeBarriers: Thefiredoorsthatseparateeachareaor“zone”inthe

building will prevent fire from spreading beyond that point for approximatelyTWOHOURS.Smokedoorswilldelaythespreadof smoke for about 30 minutes (an important feature in terms of improved visibility and ventilation).

Fire and smoke doors will close automatically when the fire alarm is triggered.

• AutomaticSprinklerSystem: Thereareover2000sprinklerheadsinourfacility–atleastonein

every room. Sprinklers are activated by heat and can prevent or delay the spread of fire beyond the immediate area of the fire.

• EmergencyPowerandLighting: Intheeventofapowerfailure,anemergencysystemwillensure

lighting in certain areas (to improve visibility) as well as an electrical power source for essential medical equipment.

• AutomaticFireAnnouncementSystem: Whenever a sprinkler, smoke detector or manual “pull station”

is activated, the fire alarm will automatically begin to sound (continuous rings). Shortly thereafter, a computer-generated announcement is made indicating the location of the fire.

• FireFighterElevator: Assoonasthefirealarmistriggered,allsixelevatorsinthebuildingwillautomaticallyreturntoLevelTwo,thedoorswillopen to let anyone inside to get off, and then the elevators will be “locked down” so that no one can use them.

However, elevators in each lobby (one in the main lobby and two in the new addition lobby) are designated as “Fire Fighter Elevator(s)”.Ifnecessary,firefighterscanuseaspecialdevicetore-activate this elevator (even when other electrical devices are not operational) in order to evacuate residents to another level.

• Ifthereissmokeinaresident’sroom,thecallbellsystemwillbetriggered and the light over the door will alternately flash red and amber to identify the exact location of the fire.

Fire PreventionInspiteofthespecialconstructionfeaturesnotedabove,preventionisalways the real key to fire safety.

Youcanhelptopreventafireinthefollowingways:• KnowandobeytheSt.Joseph’sHealthCentreGuelph’ssmoking

policy.

• Donotuseextensioncordsinyourroom.

• Haveallelectricalequipment(neworused)approvedforusebyour Environmental Services staff before plugging it in.

• Keepexitsandfiredoorsclearofobstruction.

• Ifyouobserveotherswhoarenotobeyingfiresafetyrules,informa staff member immediately.

HowtoRespondIfYouDiscoveraFireorHeartheFireAlarmIfyouoryourvisitordiscoverafire,donothesitatetocallforhelpand to activate the nearest manual pull station (located near any illuminatedred“EXIT”sign).Pullstations(orfireexitsigns)cantypically be found near the stairwells located at the end of each neighbourhood and next to the elevators on each level.

Fire Fighter Elevators can be used to move occupants on Level ThreeandFourdowntoLevelOneandTwo.

Fire DrillsFire drills are scheduled regularly without prior notification, and all staff, visitors, patients and residents are expected to participate. Visitors may stay with the resident/patient during the alarm, or go to the nearest lounge so that corridors are clear for the fire department personnelandstaff.Yourcooperationduringfiredrillsisessentialtothesafetyofallresidentsandpatients.Youwillknowwhenitissafeto return to your normal activities when you hear an announcement of“AllClear”.SJHCGCodeRedFireplanisreviewedregularly.AnEmergency Plan is also in place and a “mock evacuation” will be conducted from time to time to ensure our emergency measures are adequate.
