Our second method of notification will be U.S. mail to residents adjacent to the right-of-way. (attachment B) When practical, an effort will be made to personally notify the individual property owner when working in area. Also see the annual notice that is mailed to all GMP customers.(attachment C).

The undersigned accepts full responsibility for all statutes and regulations of the State of Vermont and

understands that any authorization is limited to the described materials, locations and time periods stated herein.

The undersigned further understands that weekly spray and dusting operations must be reported to the Vermont

Department of Agriculture. Such written report shall be on forms furnished by the Commissioner of Agriculture

and placed in the mail not later than the close of business on the Monday following the week's operation.

2018 Integrated Vegetation Management Program

Type of Treatment

Selective Low Volume Foliar -

This refers to selective treatment of identified uncompatible brush during the growing season

in a manner to preserve desirable shrub and plant species. Most effective application

technique to target individual plants for control while minimizing usage rates.

General Comments:

In the foliar season, both the selective basal and foliar methods of brush control will be used.

In the dormant season the primary mode of treatment will be selective basal (a).

Stump treatment (b) will be used only in isolated sensitive areas such as roadsides,

hedgerows, near residences and where tall isolated clumps must be cut. Stump treating may

be done throughout the spray season.

Control Details

Garlon® 4 Ultra

Manufactured by Dow Agrosciences, EPA Registration # 62719-527

Garlon® 4 Ultra is a new low-odor, non-petroleum, plant-derived methylated seed oil (MSO) solvent with

demonstrated increased efficacy on multiple woody species.

Garlon® 4 Ultra will be applied with basal oils such as Arborchem Basal Oil, Arborchem NPD or HY-

GRADE I™. The oils act as a carrier and penetrant to cut stump surfaces. It may also be applied as a

Selective Stem Basal Treatment. Garlon® 4 Ultra is applied to stumps and stems via low volume wand.

Garlon® 4 Ultra is also designed to be used on a woody plant species during the dormant season. It will not

be applied to frozen or snow covered ground.

Escort® XP

Manufactured by Bayer, EPA Registration #432-1549

Escort® XP is commonly used in tank mixes with other foliar herbicides for broad spectrum species control.

This product will be mixed with a drift control and/or surfactant such as Thinvert®.

Polaris®

Manufactured by NuFarm, EPA Registration #228-534

Polaris® is commonly used in tank mixes with other foliar herbicides for broad-spectrum species control.

This product will be mixed with a drift control and/or surfactanct such as Thinvert®.

Rodeo®

Rodeo® is applied to the foliage in the summer and early fall.

Rodeo® will be tank mixed with Polaris® and/or Escort® XP for both low and ultra low volume foliar

Thinvert® RTU (Ready to Use)

Is a combination of phyto-bland paraffinic oil blended with emulsifier, surfactant and water to form a

thin invert emulsion that is applied withULV nozzles. It replaces water as the carrier of herbicide

products. THINVERT® keeps the herbicide solution wet on the plant for a long period of time,

ensuring penetration and a high percentage of control all while minimizing the potential of drift

NOTIFICATION FOR VEGETATION MANAGEMENT ON

RIGHT-OF-WAY

Contractor X has been contracted by Green Mountain Power to

perform routine vegetation maintenance along the Transmission

right-of-way for the purposes of providing safe and reliable

electrical service. This work is located adjacent to your property

and is scheduled to commence in the coming months. A variety of

Integrated Vegetation Management (IVM) methods will be used;

including hand cutting, mowing, and selective herbicide

application. The work will be performed by experienced and

licensed Lewis Tree Service personnel.

IVM is essential to provide safe and reliable electrical service.

Tall-growing vegetation is targeted for removal to prevent it from

growing into overhead lines resulting in outages and loss of

electrical service. In addition, vegetation is managed within the

right-of-way to allow access for routine line maintenance,

inspections, and service restoration efforts during storm events.

By implementing IVM methods we encourage stable, low-growing

plant communities that require minimal maintenance and

disruption of the environment. These plant communities provide

important wildlife habitat, especially for those species requiring

open fields, meadows and shrubs.

Work Types taking place within the Right-Of-Way:

Crews hand cut tall hardwoods within the right-of-way, as well

as clearing around poles, structures, guy wires, and access to

right-of-way.

Crews will return in the summer months to selectively apply

herbicides (where applicable) to incompatible hardwood &

softwood species utilizing a foliar, cut stump or basal

application technique.

Important Annual Notice Regarding Herbicide Use in the Maintenance of Electric Utility Rights-of-Way

The Vermont Public Utility Commission has set forth rules under PUC 3.600 pertaining to the use of herbicides in the maintenance of electric utility rights-of-way. Each spring, herbicide applications may begin on or after April 1st. These rules afford you important rights and duties. Vermont electric utilities maintain electric line rights-of-way with several methods, including the selective use of herbicides on trees and brush. They also encourage low-growing shrubs and trees which will crowd tall-growing species and, thus, minimize the use of herbicides. Methods of herbicide applications may include stump, stem Injection, basal, soil, and foliar. Only electric utility rights-of-way that have tall-growing tree species with the potential of threatening the electric utility system are treated.

If you reside on or own property in Vermont within 1000’ of an electric utility right-of-way:

1. Sign up to receive written notification from your local electric utility of plans to apply herbicide on any ROW within 1000’ of your property or the property where you reside. Check nearby poles for tags identifying the utility and/or pole number, complete the form below and submit it to your local electric utility by mail before February 15

th, 2018 to be added to the

notification list. If determined to be qualified, you will receive notification from the utility at least 30 days prior to scheduled herbicide application.

2. You are responsible to make your local electric utility aware of the location of any potentially affected water supply, and of any other environmentally sensitive area where herbicide application ought to be avoided.

3. Watch and listen for public service announcements in newspapers and radio ads noting upcoming herbicide applications. 4. Check with your local electric utility regarding the vegetation management cycle near your particular line. 5. You have the right to request, in writing, that the utility refrain from applying herbicides in the process of clearing the right-

of-way, and the utility may offer alternatives such as herbicide stump treatment or herbicide stem injections. 6. You have the right to refuse, in writing, the use of herbicides whatsoever at no cost to you if the type of lines in the right-

of-way are distribution lines, bringing electric service directly to individual customers. 7. You have the right to refuse, in writing, the use of herbicides whatsoever by paying a $30 administration fee if the type of

lines in the right-of-way are transmission lines or sub-transmission lines, bringing electricity to or between substations.
