The operating table is tilted 30° so that the knee to be operated on is lower than the contralateral knee for the posterior approach.



The medial border of the medial head of the gastrocnemius muscle is identified, and the interval between it and the semimembranosus tendon is dissected to expose the posterior joint capsule.



The bone block of the posterior cruciate ligament graft is fixed with a screw and washer with the knee in 70° of flexion.



A long leg splint is used for two to three weeks postoperatively to hold the knee in extension.
