Amaç

Radyonüklid tedavi planlama, uygulama, kalite kontrol ve klinik dozimetri prosedürlerinde radyasyondan korunmanın optimizasyonu prensiplerinin uygulanmasının sağlanması.

Onkolojide radyonüklid tedavinin prensibi

Normal dokularda minimal veya geri dönüşümlü hasar ile tümöre letal doz verecek terapötik radyonüklidlerin malign alanlara taşınmasıdır.

Responsibilities (BSS: Interim Edition)

Radyofarmasötiklerin uygulandığı terapötik radyolojik prosedürler için nükleer tıp hekiminin tıbbi fizikçi ve radyofarmasist ile birlikte çalışarak, her hasytaya uygun radyofarmasötiği, uygun aktivite seçilerek primer ilgilenilen tümör alanına yeterli dozu verilirken vücun diğer alanlarında dozun ulaşılabilecek en düşük düzeyde tutulmasına gayret edilmesidir.

Radioiodine easily crosses the placenta and therapeutic doses can pose significant problems for the fetus, particularly permanent hypothyroidism.

In women, thyroid carcinoma comprises over 80% of cancer of the head and neck diagnosed between the ages of 15-45 years. Thyroid cancers are relatively unaggressive compared to most other cancers. As a result both surgical and radio-iodine treatment are often delayed until after pregnancy.

Menstrual history is often not adequate to ensure that a patient is not pregnant. In most developed countries, it is common practice to obtain a pregnancy test prior to high-dose 131 I scanning or therapy for women of childbearing age unless there is a clear history of prior tubal ligation or hysterectomy precluding pregnancy.

In spite of the above, it still happens that pregnant women are treated, either because of false histories or because the pregnancy is at such an early stage that the pregnancy test is not yet positive.

İlk gün eğer mümkün ise toplu taşımadan kaçının. Eğer kullanılması zorunlu ise birkaç saatten daha az süre olması konusunda süreyi kısıtlamaya çalışın.

Eğer mümkün ise birkaç gün işe gitmeyin ve evde kalın. İşin özelliklerine bağlı olarak bu süre 0-5 gün arasında değişebilir.

Mümkünse birkaç gün ayrı banyo ve tuvalet kullanın, ayrı havlular kullanın.

Günlük duş alın. Kullandıktan sonra banyo ve lavoboyu durulayın.

İdrarınızı oturarak yapın ve sıçramaları önlemeye çalışın, sonrasında 2-3 kez sifona basın.

Radioactive Waste

• Faeces, urine and other liquids should be disposed of via the toilet.

• Contaminated clothing, linen, food items etcwhich can not go into the toilet should be stored in a separate plastic bag labeled ’RADIOACTIVE’, and should be removed daily to the designated radioactive waste storage facility.

• Disposable cutlery and dishes should be used. If not, they should be washed in the patient’s room and reused by the patient.

Optimization of Protection in Medical Exposure

Emergency Procedures:Death of Patient

Emergency Surgery of Patients Who have Received Therapeutic Amounts of Radionuclides 1. If surgery is performed within the first 24

hours following the administration of iodine-131, fluids (e.g., blood, urine) will be carefully removed and contained in a closed system.

2. Protective eyewear will be worn. 3.The radiation safety staff will direct personnel in

methods to keep doses as low as reasonably achievable in a practical fashion.

4.If an injury (such as a cut) or a tear in a glove occurs, the individual will be monitored to see whether contamination occurred and whether any action is necessary.

Emergency Autopsy of Patients Who have Received Therapeutic Amounts of Radionuclides

1. Immediately notify the authorized user in charge of the patient and the RSO upon death of a therapy patient.

2.An autopsy will be performed only after consultation and permission from the RSO.

3. Protective eyewear will be worn. Consider the need for protection against exposure from high-energy beta rays in cases involving therapy with phosphorus-32 and yttrium-90.

4. Remove tissues containing large activities early to help reduce exposure. Shield and dispose of tissues in accordance with license conditions.

5. If an injury (such as a cut) or a tear in a glove occurs, the individual will be monitored to see whether contamination occurred and whether any action is necessary.

In the event of death of a patient who has recently received a therapeutic dose of a radionuclide care has to be taken toensure that personnel receive as low a dose as possible at all stages prior to the burial or cremation.
