Quality audit Report of

Hazrat Nizamuddin, Coaching Depot, N Railway

1.0 Introduction - In terms of instructions issued vide Railway Board letter no. 2012/M (W)

814/5 dated 20.03.12, QA/Mechanical/RDSO/Delhi has conducted the quality audit of

Coaching Depot, Delhi of Delhi division of Northern Railway.

1.1 Audit team – Shri Satish Singh, Director/I&L/RDSO/Delhi, Shri A K Sharma, AIE-

I/I&L/RDSO/Delhi, Shri Arun Kumar, SE-1/I&L/Delhi, Shri S D Pandit, JE-

I/I&L/RDSO/Delhi.

1.2 Dates of audit - 02.04.12, 10.04.12, 13.04.12 and 14.04.12

1.4 Scope of audit – Primary Maintenance of Coaching Depots, Sick Line attention, Roller

bearing section, Wheel and axle section, bogie and brake gear, coupler and buffer, air

brake, availability of infrastructure M&P facilities, test benches, availability of stores

and its management, painting, etc.

2.0 Brief about depot – Nizamuddin coaching depot is located about 1 km from the

station. It has a holding of 402 coaches. It takes up primary maintenance of 14 trains,

secondary maintenance of 22 trains and 9 trains on RPC-IV.

2.1 There are 6 washing lines with WL no. 1 and 2 having 24 coaches capacity. Washing

line no. 1 is under the process of rebuilding, from April-11. WL 3, 4 and 5 have a

capacity of 22 coaches whereas WL 6 is having a 26 coaches capacity.

2.2 Sick line has three lines with holding capacity of 19 coaches & working capacity of 13

coaches. Two EOT cranes with capacity of 25/6 T & 10/3 T is available in shed. At an

avg. 7 coaches (Including 2 AC DVS) are attended in sick lines including wheel

changing & heavy repairs. At an avg. 30 coaches for special schedule in sick line are

being attended.

3.0 Strengths

3.1 System has been implemented to have a direct dialogue with the passenger, who has

recorded his complaint, on the phone number provided by the passenger and

ascertaining the exact grievance and informing the passenger regarding the corrective

action taken.

3.2 A system of testing BP/FP hoses on 100% basis using pressure at 10 kg/cm2 before

fitment in coaches is being followed.

3.3 Check of phonic wheel and speed sensor in LHB coaches has been picked up as a

mission area.

4.0 Weaknesses

4.1 Condition of catwalks on washing lines is atrocious at many locations on WL 2 to 5. It is

either having only the width of the supporting beam available or nothing at all with

gaping holes, making it impossible to attend the coaches even with minimal safety.

4.2 Illumination needs to be improved at many of the areas since it is making inspection

and attention very difficult.

4.3 Cleanliness and general maintenance of pit lines is not satisfactory. Lack of monitoring

of cleanliness, throwing of garbage randomly all over is further deteriorating the

condition.

4.4 High pressure jet cleaning pipeline with plant for cleaning at primary pit line is not

available.

4.5 Drainage system is not effective and water logging exists at many locations in the

depot.

4.6 System of provision, replacement, testing of Fire extinguishers on trains from NZM

needs to be revisited and streamlined.

4.7 External agency is carrying out the work of coach cleaning, which appeared to be

much below expected/ tolerable standards in more than 50 % of the coaches.

4.8 Washed linen, dirty linen and condemned linen are lying in the same room. The

room is in a broken condition in one corner with lot of dust and gravel, which was

found to be affecting washed linen also.

4.9 Evidence of physical stock verification of linen could not be made available. SSE/

Linen could not confirm the last date of verification. It was also seen that a large

number of blankets which were told to be condemned did not bear any

documentary evidence.

4.10 Blanket sanitization was not found to be yielding any result. Room meant for the

purpose had only a small domestic room convection heater with a few naphthalene

balls in front of it, with isolated blankets hung here and there in folded condition.

4.11 Records of D2, D3 and D4 schedules of LHB coaches need to be organized and

streamlined.

4.12 The problem of short supply of LHB spares and Wheel Sets is being faced in the depot.

This is being followed. Rolling-in lights are provided at TKD end and DLI end for all seven platforms, except on PF 1 TKD end. FL fittings were not in working condition at four locations. Only one rolling–in hut is provided towards TKD end.

8.1 Site shall provide unobstructed view of under gear from both sides.

Site is conducive for conducting Rolling-in-examination. However, for trains permitted from platforms directly, one side view is possible.

8.2 Should have adequate space for fixing the lighting arrangement and for staff

Adequate space is available. Illumination needs to be improved at many of the areas since it is making inspection and attention very difficult.

8.3 As the train passes the nominated point, C&W staff should watch out vigilantly for loose/hanging/broken under gear parts of the coaches, any unusual sound coming from the coaches or any other abnormality in the coaches.

This is being followed. However, defects/ discrepancies noted are being informed orally to officials concerned. System of record keeping was inevident.

8.4 Temperature of the axle boxes should be measured preferably with the help of the electronic temperature measuring device.

9. Revised maintenance pattern for coaching trains circulated vide Railway Board’s letter No.95/M(C)/141/1 dt.31.01.07

iii) All missing passenger amenity fittings must be replaced and the rake must be turned out as ‘Zero-Missing-Fitting’ rake.

-do- Passenger Amenities are being fitted to turn out the rake with Zero-missing amenity, to a great extent, the only constraint being occasional unavailability.

iv) Intensive cleaning of coach toilets and lavatory. -do- This is being followed. In the last six months, 1184 AC coaches (including LHB) and 437 non AC coaches have been cleaned intensively. Records are being maintained. Inspection of Rakes of 12412 and 12430 after cleaning revealed that toilet and lavatory cleaning standards need to be improved. External agency is carrying out the work of coach cleaning, which appeared to be much below expected/ tolerable standards in more than 50 % of the coaches. This was highlighted to JE-on duty, and Sr.CDO/HNZM.

v) Clear maintenance time of 6 hours on the pit as per train schedule.

-do- This is being followed, except train No. 14085. 12412 and 12430 have a pit time slot from 0800-1400 hrs. on W/L no. 2 and 1400-2200 hrs. on W/L no. 6 respectively.

vi) Provision of proper washing cum maintenance pit line facility with adequate testing equipment and high pressure jet cleaning plant at primary end.

-do- All washing lines, 1 to 6 are pit type. Washing line 1 is under rehabilitation. Welding facility is available on all lines. Condition of catwalks on washing lines is atrocious at many locations on WL 2 to 5. It is either having only the width of the supporting beam available or nothing at all with gaping holes, making it impossible to attend to coaches even with minimal safety. High Pressure Jet cleaning arrangement is not available on any Washing line. Pit/ Column lights are not available.

9.1.2 The Other End

i) Whenever the lie-over is more than 2 hours at the

platform or the rake is stabled in the yard, the rake

should be locked and positive security should be

provided.

-do- This is being followed. Verification done on rake of Train no. 12430 and the conditions found complied with.

ii) The minimum infrastructure to be provided at the platforms from where trains are returned without secondary maintenance should be available

-do- Partially available. Drains’ covers & concrete path way are not available. No watering arrangement on PF no. 3 and 4. Water pressure becomes low at times on all the platforms. It fluctuates from 12-40 lpm on PF 1 to 6 on opening 10 hydrants and from 5-22 lpm on opening 20 hydrants. Water pressure at PF 6 was adequate. Inspection lights along the track are also not available.

iii) A joint safety certificate covering each clause of mandatory conditions will be issued by mechanical and operating branches at divisional level prior to changing over any train to revised coaching maintenance pattern.

-do- Sr.CDO/HNZM informed that no such joint safety certificate is being issued at divisional level

10. Rake maintenance link has provision of maintenance time (pit attention) of 6 hrs at primary end to enable intensive attention.

11. Internal cleaning, Passenger amenity attention and watering is done at platform line or nominated stabling line provided stipulated facilities are available at such line is followed

Stipulated RPC-4 conditions are available on stabling line no. 22. It was highlighted by Sr.CDO/ HNZM that keeping in view the workload of HNZM depot; such facilities are required on more stabling lines.

12. Washing and cleaning of coaches 109 of Chg.Manual

12.2 It should be ensured that the rake/coach is protected with proper board/signal for safety of the staff working on washing/cleaning job to prevent movement/disturbance in the activity. Scotch blocks with locking arrangement should protect lines and keys should be kept with Engineer (C&W) till the time rake is under maintenance

Rakes are being protected properly with scotch block & locking arrangement. Verification done in Rakes of 12412, Gondwana Express and 12430, Bangalore Rajdhani.

12.3 External cleaning /washing: Check that guidelines as detailed in CAMTECH manual are being followed.

12.4 Cleaning of Toilet: Check that guidelines as detailed in CAMTECH manual are being followed.

This is being followed. Toilet cleaning needs to be improved further. Toilets appeared to be splashed with water than cleaned. Distinct marks could be observed on the wash basins and pans in majority of the coaches of 12430.

12.5 Internal cleaning of upper class AC and sleeper coaches: Check that guidelines as detailed in CAMTECH manual are being followed.

This is being followed. Betel stains and dried left-over food stain marks in the passages and vestibule area could be observed. Please refer item 12.3 above for coach numbers.

12.6 Internal cleaning of GS, SLR: Check that guidelines as detailed in CAMTECH manual are being followed.

12.7 Cleaning of buffers and screw couplings 109(f) of Chg.Manual This is being followed. (i) Buffer plungers should be scrubbed with a scraper

to remove dirt and muck. Thereafter, they should be wiped clean with cleaning oil and rubbed with coir rope.

ii) Primary maintenance of the rake should be done in one hook without splitting

due date of testing had passed long back, but these had been continued to remain on coach without fresh testing, though stock was available in stores. System of provision, replacement, testing of Fire extinguishers on trains from NZM needs to be revisited and streamlined.

15. Similarly, other brake van equipment, which Mechanical Train Examining staff is responsible to supply, should be provided according to the instructions of each Railway. Portable control telephones, portable train lighting equipments, portable fire extinguisher, wooden wedges/skids and stretcher are to be provided.

111(g)(i) of Chg.Manual

DCP Fire Extinguisher and wooden wedges are being provided by C&W, same is being ensured by maintenance C&W engineer. Other items being supplied by their respective Department. The rake of 12430 Bangalore Rajdhani checked for availability of such equipments on dt. 14/04/12 and equipments were found available.

16. Watering and cleaning of rakes

Mechanical department of each railway will nominate the watering and the cleaning stations on the railway.

17. Deficiency Rolling Stock (DRS) for Coaching Stock

111(i) (i) of

(i) Railway should devise system for detecting deficiencies. Reports of deficiencies/ defects in Rolling Stock (DRS) reports in the proforma given in Annexure 1.2, should be prepared for each mail/express/passenger originating train in duplicate by the Engineer (C&W)/Electrical (TL) and should be signed jointly with the RPF representatives.

Chg.Manual DRS card is being prepared by C&W Department and provided in outgoing rake. However, Joint check with RPF is not being carried out. DRS card is not being received back.

(ii) On arrival of the train at the destination station,, the Engineer (C&W) staff and electrical staff shall check the rakes jointly with a representative of RPF and comparison may be made with original report.

111(5)(b) of Chg.Manual

Joint report is made in reported case of theft only.

18. Coach Maintenance History Card 111(k)(i) of

(i) Every coaching depot shall have computers for maintaining the coach maintenance history in a software programme which should be compatible with the programme of the coaching workshop.

Chg.Manual Though computers are available, coach maintenance management software is not available with Depot. Coach History Cards up to depot level are being maintained.

(ii) The complete history book of each coach, consisting of maintenance history cards, date card, trial card, etc. will, however, be maintained by the base workshops

Washed linen, dirty linen and condemned linen are lying in the same room. The room is in a broken condition in one corner with lot of dust and gravel, which was found to be affecting washed linen also.

Evidence of physical stock verification of linen could not be made available. SSE/ Linen could not confirm the last date of verification. It was also seen that a large number of blankets which were told to be condemned did not bear any documentary evidence. Gate Pass no. 29 of 19/03/12 showed movement of condemned bed sheets to SSB, but it was informed that same were not received there and returned.

ii.(b) At least 5% of new linen should be checked by SSE/Mechanical for: (a) bundles being sealed and having batch nos., manufacturer’s name, year of manufacture, quantity, size indicated, (b) Linen items having marking of month / year of manufacture, Railway, etc. (c) dimensional check, colour, feel and workmanship, etc.

ii.(c) Monthly report of inspections / test checks of the linen washing contractor’s premises by officers /

supervisors, should be prepared and payment should be made after recovery of all penalties / dues

This is being followed. Records of linen check at supervisory level were seen. Super-check records at Depot officers’ level is being carried out. It was informed that system exists for rejection/ return of any soiled/ dirty linen entering the circuit after washing. System exists for forwarding necessary advices in regard to imposing deductions from the contractor’s bills in case of default.

ii.(d) Enough buffer stock of linen to last for 2 to 3 days is available

This is being followed. As per the daily position of linen availability on 12/04/12, 28588 bed sheets, 19684 pillow covers, 42311 face towels and 1392 blankets were available at the depot.

Procedure for rolling – in examination: As train passes the nominated point, C&W staff should watch out vigilantly loose / hanging / broken under gear parts of the coaches special attention should be paid to air springs in case of coaches provided with air springs at secondary suspension stage. It should be ensured all the air springs are in inflated condition by noting that the levelling valve lever is in horizontal position. In case it is not feasible to inflate the air springs of any particular coach, these should be isolated by air spring isolating cock and speed of train should be reduced to 60 Kmph or the coach should be detached.

LOCTITE 243 or equivalent glue should be used to ensure rigid connection between internal and external thread. The Phonic Wheel screws should be tightened with 21N-m torque.

Water pressure becomes low at times on all the platforms. Illumination needs to be improved at many of

the areas since it is making inspection and attention very difficult.

instructions are being complied with during maintenance.

12. Performance of critical spare: This is a cause for concern.
