•Need a picture of yourself to apply online Or a picture of self to send with paper application (A school picture would be perfect)

•Address, Phone number, etc.

•Online: Credit Card•Paper: Check (get paper from Guidance Office)

IMPORTANT!!! If you do not have a PHOTO ID, you need

to ask Mrs. Linkes or Mr. Zablocki to get you a school ID!!!

Do NOT ask day before test, it will be unlikely that you will be able to get the photo ID; therefore you will not be able to take the test.
