This brief report provides statistical data for each of Champlain’s 10 recommended Health Link Areas. Selected indicators are provided that describe each area’s population, some of its high-needs patients and overall hospital use by all residents.

Population estimates were based on the 2011 census. Akwesasne First Nation, in HLA2, did not participate in the census and is excluded from the counts. Totals in Tables 1 and 2 may differ due to rounding.

Table 2: Population Characteristics by Residence (Health Link Area) Population and Percent Seniors:

Population estimates were based on the population in 2011. Percent aged 65 years or over provided as a percentage of the total population. Totals in Tables 1 and 2 may differ due to rounding.

Percent French Mother Tongue:

Proportion of residents who included French as one of their first official languages spoken languages in the 2011 census. Totals in Tables 1 and 2 may differ due to rounding.

All reported rates are based on hospital records for the 2011-12 fiscal year.1 Rates are per 1,000 persons, based on area of residence irrespective or where people received care in Ontario. Rates are crude, not age-adjusted. Population estimates to calculate rates were obtained from the 2011 census.

People with High Needs (composite indicator):

Includes those who met the definition of one or more of the following in 2011-12:

• People with a high cost inpatient stay

• People with multiple inpatient stays

• People with 5+ ED visits

People with a High Cost Inpatient Stay:

Includes people who had at least one acute inpatient stay in 2011-12 that was in the top 10% in terms of cost (based on CMG+ Resource Intensity Weight). Select very high cost groups considered likely to receive most of their care in the acute care setting only are excluded (people who received transplants, premature babies and victims of serious burns).2 Designated (scheduled) mental health beds, designated rehabilitation beds and complex continuing care beds are excluded.

People with multiple Inpatient stays:

People who had two or more acute care hospitalizations in 2011-12. May involve more than one Ontario hospital. Includes patients admitted for any reason. Designated (scheduled) mental health beds, designated rehabilitation beds and complex continuing care beds are excluded.

People with 5+ ED Visits:

People who had five or more planned or unplanned emergency department visits for any reason, within 2011-12. May involve multiple Ontario hospitals.

People with a readmission within 30 days

People who had an acute care admission within 30 days of being discharged from an earlier acute care stay in 2011-12. May involve more than one Ontario hospital.

Rates are crude, not age-adjusted. Population estimates to calculate rates were obtained from the 2011 census.

Equivalent Acute Bed Use:

The values are based on the total length of stay for patients discharged from acute care hospitals in Ontario. Patients can have multiple discharges from one or more hospitals. Each is counted separately. The number of beds is estimated using a 90% occupancy assumption.4 Designated (scheduled) mental health beds, designated rehabilitation beds and complex continuing care beds are excluded.

Equivalent ALC Bed Use:

The values are obtained on the alternative level of care (ALC) length of stay for patients discharged from acute care hospitals in Ontario. The number of beds is estimated using a 90% occupancy assumption.4 Patients are designated “alternative level of care” when they no longer require an acute bed but are not able to be discharged because the care they require elsewhere (e.g. in a long-term care facility) is not available. Designated (scheduled) mental health beds, designated rehabilitation beds and complex continuing care beds are excluded.

ED Visits:

The values are based on the count of planned and unplanned visits to emergency departments during 2011-2012. An individual may make multiple visits and may visit more than one emergency department within the year. Each visit is counted separately.

Table 5: Top 20 Case Mix Groups Among Champlain Residents with High Cost Acute Inpatient Stays (Top 10%) in 2011-12 CMG Cost:

The top 20 Case Mix Groups (CMGs) are obtained for the entire Champlain area based on the most common CMGs for people with high cost inpatient stays (i.e. the top 10% - see definition above) for 2011-12. Average costs are then calculated based on patients’ Resource Intensity Weights (RIW) and the

average cost per weighted case according to the Ontario Cost Distribution Methodology (OCDM). A volume-weighted average for all Champlain hospitals ($5,968 per weighted case) is derived from the most recent OCDM estimates available (2010-11).

Table 7: High / Low Rate Summary for the Top 10 Case Mix Groups by Patient Residence (Champlain Residents with High Cost Inpatient Stays, 2007-12) Each Health Link Area (HLA) is compared to the average per capita rate of discharges for each of the top 10 CMG's from 2007 to 2012. Five years of data is used in order to attain greater statistical precision. Those with CMG-specific discharge rates 40% above the Champlain average are considered “high” for that CMG and displayed in red. Those 40% below the regional average are considered “low” for that CMG and displayed in blue.
