Check List for PHCs
1) Number of patients used the out-patient services in the past quarter:
a. How many of them are from SC, ST, and other backward classes?
b. How many of them are women?

2) Availability of Medicines in the PHC:
a. Is the Anti-snake venom regularly available in the PHC?

d. Is the facility for internal examination for gynaecological conditions
available at the PHC?
e. Is the treatment for gynaecological disorders like leucorrhoea,
menstrual disorders available at the PHC?

the last three years?
i. Malaria

iii. Gastroenteritis (diarrhoea and vomiting)

v. Fever with loss of consciousness / convulsions
b. If yes, did the PHC staff respond immediately to stop the further spread of
the epidemic?
c. What steps did the PHC staff take?
d. How is the behaviour of PHC staff with the patient?

specific instances.
i. Are women patients interviewed in an environment that ensures privacy
and dignity?
