How to proceed:

1. Upon agreement with a contact person from Mondi SCP, you will take the order/work

order to visit work permit issuer

2. Join inspection of the workplace with issuer

3. Issuer will give you the necessary forms

4. In the PtW and in the annexes you will assessment the activities performed in

cooperation with your group

5. Fill in the relevant fields in the PtW form

6. Discuss risk assessment (RA) together with issuer

7. You will take over the work and the workplace against the signature, the issuer will

hand over and you will get the copy of the PtW respectively annexes

Should the conditions agreed in RA/ PtW change during

work:

1. Visit the issuer with a copy of PtW

2. Add the information in the PtW original and copy together with the

issuer

3. Ensure the implementation of measure together with the operations

personnel

4. Instructs colleagues about changes

5. Continue the work

Arrival of a new member of the work group:

1. Instruct new member about content of PtW

2. Visit the issuer with the copy of PtW

3. Add the worker´s name

4. The worker signs both copies of PtW

5. The worker may start working

Finalizing of work:

1. Prepare the workplace for handover

2. Finish all activities (all workers)

3. Sign off the end of work in PtW

4. The issuer accepts the workplace against his/ her signature

Interruption of work

1. Prepare the workplace for handover

2. Finish all activities (all workers)

3. Sign off the interruption of work in point 19 of Annex 5

4. If hot work is carried out inspection work and consider whether the need for

further inspection recorded in paragraph 19 of Annex 5

5. The issuer accepts the workplace against his/ her signature

PtW can be interrupted for 7 days

Permitting work after interruption

1. Reassess conditions and consider necessity of additional measures.

2. You and members of work group confirm familiarization with PtW against

Lockout and release of energy

Ensure that:

• Every one member of work group locked his personnal

yellow padlock before the start of the work

• Each worker keeps the key to the personal yellow lock

• After completion of work, each worker unlocks his/ her

personal yellow lock

• Padlock will be returned to issuer

Work on live equipment is work that cannot be carried out, if the equipment

has been de-energised. This means that in order to carry out specific work,

the equipment has to energized or has to be in motion.

Examples include diagnostic work, setting, performance of tests, corrections

on conveyor belts running aside and the like.

You will develop a safe operational procedure for working on a live

equipment that will be approved by the manager of the Mondi SCP

operation against the signature.

A written work procedure is not required for the work on electrical equipment

with voltage under 400V, if the work is performed by persons qualified to

perform activities on electric equipment according valid legislation.

Leader or member of fire protection patrol have to check

workplace 30 minutes after completion of hot work.

Employee conducting hot work specifies if follow-up

inspection is necessary. If yes he/she writes down time of

follow-up inspection

Leader

Group leader:

Ensures, that risk assessment is done and all threats are recorded in PtW

Determines which effect the performed work may have on others at

workplace.

Together with ISSUER ensure, that all necessary measures are recorded

in PtW.

Ensures that all members of workgroup know about risks and taken

measures, as well as all requirements written in PtW and that all workers

are professionally qualified for performed work and have required

certificates (these have to be available for inspection at place of work).

Ensures, that all new members of workgroup are familiar with conditions

written in PtW.

Group leader:

Ensures that joint inspection with Issuer (or any other person

appointed by him/her) and Acceptor will be done prior to

issuing of PTW

Ensures that work will be not performed out of date and time

written in PtW

Ensures that PTW will be at workplace.

Ensures that type and scope of the work is not different from

what is written in and agreed

Ensures that when the conditions change, the work will be

suspended and reassessed together with Issuer.

Leader

Group leader:

Confirms validity of PtW and fulfillment of measures and take

over the workplace by his/her signature

Ensures that work will start after:fulfillment of all measures written in the PtW

familiarization of all workers with PtW

Approval of work by Issuer

Ensures that commencement of activities starts only after

taking all the measures given in WP and only after issuer

permits commencement of work.

Ensures performance of the inspections related to

observation of the RA and WP terms.

Confined space

Supervisor (stand-by person) may supervise several work

groups in one confined space after a mutual agreement of

group leaders. In this case, she/he uses a register in the

Annex to PTW of his/her work group for all persons in CS.
