Dear Colleagues,

It gives me great pleasure to welcome you to the inaugural Oesophageal Neoplasia Masterclass 2016 Live Endoscopy Course at University College Hospital. It also gives me great pleasure to welcome our esteemed international and national faculty members and thank them for taking the time to come to this course. For some it is their fi rst time at UCLH, for most they are returning after previous visits and it is great to welcome old friends back.

With the ever increasing annual repertoire of live endoscopy courses, the vision behind the ONM programme is to bring in under one umbrella all the key innovative and state of the art developments involved in the diagnosis and management of Oesophageal Neoplasia.

I hope that during the course of today through a combination of lectures, compelling debates and live demonstrations we will be able to share with you some of these developments.

As always a huge amount of work goes into organizing and coordinating events such as this and with that in mind I would like to thank my colleagues at UCLH and all the team that have been involved in, no doubt, making today a tremendous success. I hope you enjoy the course.

Pradeep Bhandari leads the early cancer services at Portsmouth. His initial research was related to the role of cyclooxygenase in upper gastrointestinal infl ammation and cancer. In 2004, he went to National cancer center in Tokyo on a visiting fellowship and trained in the principles of early cancer diagnosis and endoscopic resection of superfi cial neoplasia.

He was appointed as a Consultant Gastroenterologist in Portsmouth in 2005. He developed an early cancer service providing advanced endoscopic diagnosis and resection for upper and lower gastrointestinal neoplasia. This service provides the basis of various research projects and advanced training program apart from providing a tertiary referral service for UK.

His contribution to UK endoscopy was recently recognized by RCP(Edinburgh) by awarding him Freeland Barbour Award. He was awarded the Hopkins Endoscopy prize by the British Society of Gastroenterology in 2013 and his research work received the ASGE crystal award for his endoscopic work. He is a specialist advisor to NICE and various BSG committees. He is a member of BSG, ESGE and ASGE and has been instrumental in producing various International Guidelines.

He was trained in Gastroenterology in Den Bosch and Amsterdam and was appointed Consultant Gastroenterologist, at Academic Medical Center in Amsterdam in 2001 as Associate Professor in 2005, and was appointed as Professor of Gastrointestinal Endoscopy in 2011. Dr. Bergman is the head of the AMC esophageal research team, which conducts endoscopic imaging studies using high defi nition endoscopy, optical chromoscopy techniques, autofl uorescence endoscopy, volumetric laser endomicroscopy, and spectroscopy techniques. In addition, the group has a strong focus on the endoscopic treatment of early neoplasia in the upper digestive tract using endoscopic resection and endoscopic ablation techniques.

The team currently consists of three physicians/endoscopists, twelve research fellows, three research nurses, a database manager, and 2 physician assistants. The group has extensive experience with multicenter clinical trials and coordinates international studies in Europe, the United States and China.

Dr. Bergman is a permanent member of International Working Group on Classifi cation of Oesophagitis, member of the International Editorial board of ³Gastrointestinal Endoscopy², member of the International Editorial board of ³Endoscopy², Special Section Editor of ³Gastroenterology², and is the secretary of the Dutch Society of Gastrointestinal Endoscopy.

Dr. Bergman has authored and co-authored on over 250 peer reviewed publications and text book chapters and has lectured at many national and international meetings.

Stefan Seewald obtained his medical degree at the University of Halle/Saale, Germany in 1991. He qualifi ed as a specialist and lecturer for internal medicine at the University of Bonn from 1991 to 1999. At this time his research work was focused on intracellular signalling pathways.

In 1999 he started his training in gastroenterology and interventional endoscopy at the Medical Center Hamburg-Eppendorf under the leadership of Prof. Nib Soehendra. He became board certifi ed gastroenterologist, specialist in interventional endoscopy, stuff member, professor of medicine and member of the gastroenterologist credentialing board. He extended his skills in ESD at the National Cancer Center in Tokyo and in POEM at Showa University in Yokohama, Japan.

His main focus of research has been on endoscopic techniques for the resection of early gastrointestinal malignancies, pancreatic fl uid collections, biliopancreatic fi stula, treatment of varices and achalasia.

In 2008 Stefan Seewald became partner of the Center for Gastroenterology at the Hirslanden Clinic in Zurich, Switzerland, one of the major leading center for interventional endoscopy in Switzerland. He is course director and organizer of “EndoSwiss-2017 Live” an international meeting on therapeutic endoscopy taking place every two years.

During his training in internal medicine he followed the doctoral program at the doctoral school of KU Leuven and obtained a PhD in 2005 after defending his thesis in basic gastrointestinal physiology entitled “Ligand and electrically induced activation patterns in myenteric neuronal networks. Confocal calcium imaging as a bridge between basic and human physiology”. Since 2005, he has been working as a staff member at the department of gastroenterology and was appointed associate head of clinic in 2006 and associate professor in 2007 at the KU Leuven, faculty of medicine. In October 2013 he became head of endoscopy, which serves as a multidisciplinary activity center hosting services for the departments for gastroenterology, hepatology, pneumology and pediatrics, with a yearly performance of over 30.000 endoscopic procedures.

His main focus of research lies currently in advanced endoscopic imaging (NBI, BLI, I-scan, CLE) and therapeutics, with a main interest in premalignant conditions such as Barrett’s esophagus, HNPCC, ulcerative colitis. After participating in a training program organized by prof Bergman in Amsterdam, Raf Bisschops has deployed one of the major recruiting centers in Belgium for endoluminal treatment of Barrett’s neoplasia. He is also actively involved in the motility clinic with a special interest in the link between gastrointestinal motility and endoscopy, hence his interest in peroral endoscopic myotomy.

Raf Bisschops is a member and co-editor of the Editorial board of Endoscopy. He has authored and co-authored several peer reviewed papers.

Dr. Haidry is a Consultant Gastroenterologist and interventional endoscopist at University College Hospital, London. He trained at St Mary’s Hospital in London and completed his Specialist Gastroenterology training in North East London.

He is the director of endoscopy for the division of gastroenterology at University College Hospital and leads the upper GI interventional endoscopic unit. He also sits on the BSG oesophageal committee His main interests are pre-malignant and malignant disorders of the upper gastrointestinal tract, with a particular interest in Barrett’s and Squamous neoplasia, oesophageal cancer and advanced interventional endoscopy. He also hosts several live endoscopy workshops each year at UCH focusing on novel and innovative endoscopic imaging techniques and therapeutic endoscopic courses focusing on endoscopic resection, radiofrequency ablation, cryo-ablation, EUS and complex stricture and fi stula management. He sits on the BSG oesophageal committee.

He has an academic position at UCL as an honorary senior lecturer exploring existing and novel endoscopic imaging technologies for detecting early cancers. He supervisors several MD and PhD projects at present. His other main research focus is minimally invasive techniques for treating cancers of the upper gastrointestinal tract and continues to lead and be involved actively on several national and international clinical trials in these fi elds. Recent innovative studies that he is leading on include include Duodenal Mucosal resurfacing for management of Type 2 DM, Cryo-ablation for oesophageal neoplasia.

Laurence Lovat is Professor of Gastroenterology & Biophotonics at UCL and Honorary Consultant Gastroenterologist at UCLH. He heads the GENIE Research lab at UCL where he is also the Head of the Bloomsbury Campus for the Division of Surgery & Interventional Science. He was appointed as Senior Lecturer in 1999.

His major contributions to oesophageal neoplasia management include research into and popularisation of minimally invasive endoscopic therapy as well as research into enhanced endoscopic diagnosis. He helped pioneer the use of photodynamic therapy for dysplastic Barrett¹s oesophagus, leading to NICE approval. When this was superseded by radio frequency ablation, he set up and continues to oversee the UK National RFA Registry which reports on outcomes for more than 1500 patients nationally.

In the diagnostic space, he has trialled multiple spectroscopic and imaging techniques including elastic scattering and mid infra red spectroscopies, confocal endomicroscopy and OCT. He enrols over 500 patients per year into clinical trials. He is an advisor to NICE, the Department for Health and a scientifi c advisor to multiple charities and commercial companies. He has supervised more than a dozen PhD students.

Dr. Charlie Gordon is a Consultant Gastroenterologist and interventional endoscopist at Royal Bournemouth Hospital, Dorset. He trained at St George’s Hospital in London and completed his Specialist Gastroenterology training in South West London. He was appointed as a consultant in 2008. He is the lead for endoscopy and upper GI intervention at Royal Bournemouth Hospital, and is chair of the Dorset SSG for upper GI cancers. His main interests are Barrett’s screening and endotherapy (EMR, RadioFrequency Ablation) and interventional upper GI endoscopy.

Since 2008 he has built up an HALO RFA service serving the Dorset region, and has contributed >100 patients to the national HALO database.

Matthew is a Consultant Gastroenterologist at University College London Hospital NHS Trust (UCLH), with an academic appointment as an Honorary Senior Lecturer at The National Medical Laser Centre, University College London (UCL).

He was Director of endoscopy at UCLH from November 2008 to April 2014, having formally been Director of Endoscopy at Chelsea & Westminster Hospital from July 2005 to October 2007. His current appointments include: British Society of Gastroenterology (BSG) Endoscopy Committee, BSG Endoscopy Research Committee, and Endoscopy Section editor, Faculty of 1000 (F1000). He has written chapters and reviews on endoscopy and is editor of the text book ‘Endoscopy in the Cancer Patient’.

He has also written numerous original papers, chapters and general reviews. He undertook an EUS and therapeutic endoscopy fellowship in Sydney in 2004 and a sabbatical in Tokyo’s prestigious National Cancer Centre in 2007 training in ESD. He currently provides a Barrett’s service at UCLH undertaking RFA & EMR, but also undertakes ESD, laser, EUS and POEM. His research interests include GORD, Barretts, obesity and cancer progression, and the pre-malignant stomach.

Dr Rami Sweis acquired his Bachelor¹s degree from the University of Illinois at Chicago in 1994 and his medical qualifi cation from University of Edinburgh in 1999. He trained in gastroenterology at Guy¹s & St Thomas¹ Hospitals, Kings College Hospital and South East Thames, and completed his PhD from Kings College London in 2012. He was appointed at UCLH in March 2014 as a Consultant in Upper GI Medicine. He is the Upper GI Physiology Lead at UCLH and an Honorary Clinical Senior Lecturer at UCL.

Rami Sweis¹ research focused on advancing the methodology and utility of the technology used to investigate refl ux and swallowing disorders, specifi cally catheter and wireless pH-monitoring (Bravo) as well as High Resolution Manometry. He also has a particular interest in endoscopic therapy and in investigating and managing complex benign disorders (including POEM for Achalasia) as well as Barrett¹s and early cancer.
