APPROVE SINGLE PLAN FOR STUDENT ACHIEVEMENT 2014-2015 Approve the Single Plan for Student Achievement (SPSA) for Stanford Middle School. The plan has been reviewed by Equity, Access, and College & Career Readiness staff for compliance. The plan contains the components of effective planning based on student needs, assessment data, use of staff, parent involvement, and allocation of resources. The schools’ approved SPSA for the 2014-2015 school year will also serve as an interim plan until the following school year’s SPSA is revised and approved. The funding for all programs and personnel outlined in these SPSAs is reflective of the current fiscal year only. 2. RATIFY APPLICATION FOR THE FRESH FRUIT AND VEGETABLE PROGRAM Ratify LBUSD’s application for the Fresh Fruit and Vegetable Program, which includes five elementary schools: Chavez, Lee, Smith, Stevenson, and Webster. The purpose of the program is to provide an additional free fresh fruit or vegetable snack to students during the school day as a supplement to the school breakfast and lunch programs, and to teach students about good nutrition. The schools will serve fruits and vegetables three times per week during morning recess. The fresh fruits and vegetable will be available to all students. 3. APPROVE P.L.A.Y. PILOT PROGRAM AT THREE SCHOOLS Approve the Physical Learning Activities for Youth (P.L.A.Y.). pilot programs for Tincher, Bixby and Burbank schools. The pilot programs are free and are run by the YMCA for up to three weeks beginning in late-May or June 2015. The program combines physical fitness, health education and character development. P.L.A.Y. is aligned with the California Physical Education Content Standards. The curriculum designed to meet the needs of each school, taking into consideration the unique characteristics of each student population. There is no cost to the District. 4. APPROVE APPLICATION FOR WORKFORCE INVESTMENT ACT, YOUTH
ACADEMY PROJECT Approve LBUSD’s application for the Workforce Investment Act, Youth Academy Projects as administered by the City of Long Beach/Pacific Gateway. The grant provides supplemental funds for youth services that help students to become employable, productive, and responsible citizens, and workers. LBUSD’s proposal focuses on assisting students to transitional employment for high school aged youth designed to retain employment, provide pre-employment skills training, job development and placement and follow-up services. The proposal seeks $312,500 in funding.

Subject: Adoption of Tentative Agreements, California School Employees Association, Long Beach Enclosures: None Chapter #2, Unit A and Unit B __________________________________________________________________________________ Category: New Business Reason for Board Consideration: Action Date: May 26, 2015 BACKGROUND The Long Beach Unified School District (“District”) and the California School Employees Association, Long Beach Chapter #2 (“CSEA”), recently concluded reopener negotiations for Collective Bargaining Agreements covering both Unit A and Unit B. CSEA informed the District on May 21, 2015 that their membership had ratified the Tentative Agreements providing for the modification of the Unit A and Unit B contracts through October 31, 2015. Ratification resulted in revisions to the Unit A and Unit B bargaining unit contracts, including a 5% increase in all salary schedules for bargaining unit members, retroactive to July 1, 2014. In addition, language was modified to align both Unit A and Unit B health benefits to those of other district employees. Revisions also resulted in clarified language related to the reporting of absences for both Unit A and Unit B, and modification in the language for bus driver workdays in Unit B. In compliance with collective bargaining agreement public disclosure requirements of Government Code, Section 3547.5, the “Public Disclosure of Collective Bargaining Agreement” forms for CSEA Unit A and Unit B have been displayed for public review since May 12, 2015. These forms have also been reviewed by the Los Angeles County Office of Education according to compliance with AB1200 (Chapter 1213/1991), Government Code 3547.5 and CCR, Title V, Section 15449. The Superintendent or designee shall ask parents/guardians to provide emergency contact information in order to facilitate communication in the event of an accident or illness.
District staff shall appropriately report and document student accidents.
Resuscitation Orders
The Board believes that staff members should not be placed in the position of determining whether or, not to follow any parental or medical "do not resuscitate" orders. Staff shall not accept - or follow any such orders.
The Superintendent or designee shall ensure that all parents/guardians are informed of this policy.
Automated External Defibrillators (AED)
The Board authorizes the placement of automated external defibrillators (AEDs) at designated school sites for use by designated personnel who have volunteered to receive training in the use of AEDs.
The Superintendent or designee shall develop procedures/guidelines for employees regarding the use of these devices and shall ensure that employees receive training on their proper use and handling. The guidelines shall also specify the placement, security, and maintenance of the AED.
BP 5141.1 The authorization of AEDs in district schools shall not be deemed to create a guarantee or obligation to use the AED in the case of an emergency, nor any expectation that an AED or trained employee will be present and/or able to use an AED in an emergency or any expectation that the AED will operate properly.

In addition, Education Code 56301 mandates that the District establish a continuous "child find" system which addresses the relationships among identification, screening, referral, evaluation, planning, implementation, review, and triennial assessment. Identification and Evaluation of Individuals for Special Education The Governing Board recognizes the need to actively seek out and evaluate district residents from birth to age 21 who have disabilities in order to provide them with appropriate educational opportunities in accordance with state and federal law. (cf. 0430 - Comprehensive Local Plan for Special Education) (cf. 6164.41 - Children with Disabilities Enrolled by Their Parents in Private School) (cf. 6164.6 - Identification and Education Under Section 504) The Superintendent or designee shall establish a comprehensive system that includes procedures for the identification, screening, referral, and regular and triennial assessment of individuals eligible for special education, as well as procedures for the planning, implementation, and review of the education and related services provided to such individuals. (Education Code 56301) The district's identification procedures shall include methods for utilizing referrals from parents/guardians, teachers, appropriate professionals and others, and shall be coordinated with school site procedures for referral of students whose needs cannot be met with modifications to the regular instructional program. (Education Code 56302) The Superintendent or designee shall notify parents/guardians, in writing, of their rights related to identification, referral, assessment, instructional planning, implementation, and review, including the right to consent to any assessment concerning their child. In addition, the Superintendent or designee shall notify parents/guardians of procedures for initiating a referral for assessment to identify individuals who need special education services.
